    Abstract PREGUNTA: Tras los negativos resultados del estudio SYMPLICITY HTN-3 (Renal Denervation in Patients With Uncontrolled Hypertension), ¿qué nuevos ensayos permiten pensar de nuevo en la denervación renal (DR) como opción terapéutica? RESPUESTA: Los resultados del estudio SYMPLICITY HTN-31 fueron del todo inesperados, sobre todo porque los que teníamos cierta experiencia con la técnica habíamos vivido una realidad completamente distinta. Las limitaciones del estudio se resumen en una discutible selección de los pacientes (con tratamiento farmacológico no optimizado que se ajustó durante el seguimiento y que generó una respuesta inesperadamente positiva en el grupo control), una falta de experiencia por parte de los operadores (la mayoría de ellos realizaron las primeras intervenciones dentro del estudio) y una falta de conocimiento sobre cómo realizar la intervención para optimizar los resultados. Tras el análisis detallado de los resultados del SYMPLICITY HTN-31 se diseñaron dos estudios, con un dispositivo mejorado para realizar la intervención, cuyos resultados se han presentado recientemente. El primer estudio, el SPYRAL HTN-OFF MED2 fue un ensayo clínico aleatorizado, simulado (con grupo sham control) como prueba de concepto sobre la eficacia de la DR para disminuir la presión arterial (PA) en pacientes sin tratamiento farmacológico concomitante.     Abstract QUESTION: After the negative results of the SYMPLICITY HTN-3 (Renal Denervation in Patients With Uncontrolled Hypertension) study, what new studies are dealing with renal denervation (RD) as a possible therapeutic option? ANSWER: The results of the SYMPLICITY HTN-3 study1 were totally unexpected, particularly because those of us who had some sort of experience with this technique had lived a completely dif-ferent reality. The limitations of the study have to do with the arguable selection of patients (with non-optimized pharmacological treatment that was adjusted during follow-up and generated an unexpectedly positive response in the control group), the operators’ lack of experience (most of them conducted their first procedures within the study), and the lack of knowledge on how to conduct this procedure in order to optimize the results. After the detailed analysis of the SYMPLICITY HTN-3 study,1 two different studies were designed with an improved device to conduct the procedure and their results have recently been made public. The first study, the SPYRAL HTN-OFF MED2 was a randomized sham-controlled clinical trial (with a sham control group) as the proof-of-concept on the efficacy of RD to reduce arterial blood pressure (BP) in patients without concomitant pharmacological treatment.     Abstract Electrocardiogram (ECG) synchronization is useful to avoid the effects of cardiac motion in medical measurements, and is widely used in standard medical imaging. A number of medical equipment include embedded commercial synchronizers. However, the use of independent synchronization modules is sometimes needed when several non-integrated instruments are used, or in the development of new medical instruments and procedures. We present a simple low-cost ECG synchronizer module based on an Arduino controller board that converts the ECG signal into a transistor-transistor-logic (TTL) one, allowing real-time medical measurements triggered at specific phases of the cardiac cycle. The device and conversion algorithm developed is optimized in vitro using synthetic and human ECG signals, and tested in vivo on three swine specimens. Error rates during the in vivo testing stage remain below the 2% of the cycles in all animals and critical false positives are less than 1%, which is sufficient for most applications. Possible algorithm updates are discussed if its performance needs to be improved.

    Abstract Background: Sacubitril/valsartan (Sac/Val) is superior to angiotensin-converting enzyme inhibitors in reducing the risk of heart failure hospitalization and cardiovascular death, but its mechanistic data on myocardial scar after myocardial infarction (MI) are lacking. The objective of this work was to assess the effects of Sac/Val on inflammation, fibrosis, electrophysiological properties, and ventricular tachycardia inducibility in post-MI scar remodeling in swine.
    Methods: After MI, 22 pigs were randomized to receive β-blocker (BB; control, n=8) or BB+Sac/Val (Sac/Val, n=9). The systemic immune response was monitored. Cardiac magnetic resonance data were acquired at 2-day and 29-day post MI to assess ventricular remodeling. Programmed electrical stimulation and high-density mapping were performed at 30-day post MI to assess ventricular tachycardia inducibility. Myocardial samples were collected for histological analysis.
    Results: Compared with BB, BB+Sac/Val reduced acute circulating leukocytes (
    Conclusions: After MI, compared with BB, BB+Sac/Val was associated with reduced acute systemic inflammatory markers, reduced total scar and border zone mass on late gadolinium-enhanced magnetic resonance imaging, and lower ventricular tachycardia inducibility.
