    Abstract Background: Conventional thoracotomy (CT) often leads to systemic inflammatory response syndrome (SIRS), which induces several clinical complications. CT remains widely used in low-income institutions. Although minimally invasive surgical procedures, such as robotic surgery (RS), have been used to prevent many of the complications inherit from the surgical procedure. Here, we investigated the protective effect of vagus nerve stimulation (VNS) in a pre-clinical model during CT or RS and postoperative period (POP) relative to clinical complications and inflammatory control. The objective was to compare hemodynamic features and cytokine levels in the blood, lung, and bronchoalveolar lavage (BAL) fluids of animals subjected to CT or RS with or without VNS.
    Methods: Twenty-four minipigs were subjected to 12 animals CT and 12 animals RS, with or without VNS, and accompanied 24 h later by pulmonary lobectomy. Blood samples for evaluating the hemodynamic parameters were collected before the surgical preparation, immediately after the beginning of VNS, and every 4 h until 24 h after the lobectomy. BAL fluid and lung tissue were collected at the end of the experiment. Cytokine levels were evaluated in the blood, BAL fluid, and lung tissues.
    Results: VNS maintained a more stable heart rate during POP and decreased the incidence of overall cardiac complications while preventing increase in IL-6 levels 12 h after lobectomy, compared to sham animals. No differences were found in cytokine expression in the BAL fluid and lung tissue in any of the studied groups.
    Conclusions: Taken together, our data suggested that VNS should be considered a non-pharmacological tool in the prevention of the exacerbated inflammatory response responsible for severe clinical complications, especially in more aggressive surgical procedures.

    Abstract Objetivo: Analisar e comparar a sobrevida de pacientes com metástases pulmonares e pleurais em um serviço inicial de cirurgia torácica. Métodos: Estudo longitudinal retrospectivo de 19 pacientes com metástases pulmonares e pleurais submetidos à cirurgia entre 2016 e 2020. Sobrevida analisada através da curva de Kaplan Meier. Resultados: A maioria do sexo feminino, 84,2%, com idade média de 64 anos e 68,4% não eram tabagistas. Comorbidades foram detectadas em 73,7% dos pacientes, sendo a hipertensão arterial a mais prevalente. Os tumores primários mais comuns foram, câncer de mama e pulmão. A abordagem mais frequente (52,7%) foi através da videotoracoscopia. O tratamento quimioterápico abrangeu 84,2% dos pacientes. Dos seis pacientes que evoluíram com complicações pós-operatórias durante internação, 33,3% tiveram pneumonia e 1 foi a óbito. A sobrevida global aos 30 meses foi 57,9%. Pacientes com metástase pulmonar mostraram maior sobrevida em 30 meses, comparativamente àqueles com metástase pleural (75% vs. 50%; p=0,017). Em relação ao tumor primário, pacientes com carcinoma de mama tiveram sobrevida de 85% em 12 meses, enquanto que dentre os pacientes com câncer colorretal a sobrevida foi de 50%. Conclusão: De acordo com nosso estudo os pacientes portadores de metástase pleural têm pior prognóstico quando comparados aos pacientes portadores de metástases pulmonares. A localização primária do tumor associa-se à sobrevida do paciente.

    Abstract Over one million cases of the SARS-CoV-2 virus have been confirmed worldwide, with the death toll exceeding 50,000 people. An important issue to be addressed concerns the exposure of health professionals to this new virus. The first reports from Wuhan province, China, described infection rates of up to 29% among healthcare professionals before the use of personal protective equipment (PPE) was fully regulated. There are several protocols on the correct use of PPE during aerosol-generating procedures. However, there is no specific guidance on how to proceed in cases of need for chest tubes in patients with positive COVID-19 active air leak. The objective of this work is to assist surgeons of the most diverse specialties during the chest drainage of a patient with COVID-19 and to avoid a risk of contamination to the professional and the environment.
