    Title translation Evaluation of the effectiveness of a school-based alcohol and cannabis prevention programme among adolescents.
    Abstract Objective: Alcohol and cannabis consumption in young people aged 14 to 18 years show high and stable prevalence and incidence. The Instituto de Adicciones de Madrid Salud (IAMS) within its prevention strategy aimed at adolescents and young people in Madrid, implements an universal 3-sessions school-based prevention program. To evaluate the effectiveness of a program aimed at alcohol and cannabis from the Theory of Planned Behavior (TPB, Ajzen, 1991), we analysed the changes in consumption and intention of consumption, attitudes and subjective norm as well as in the risk perception.
    Methods: Quasi-experimental design. Participants: Over 1,529 students belonging to 12 high schools in Madrid (M=15.36 years, DT=1.13, range: 13-18, 51.8% boys) Experimental group: N=790, intervention period October-December 2018. Control group: N=469. The Student's t test was used for the repeated means test. When the difference between the means was significant, the effect size was calculated using Cohen's d index.
    Results: Alcohol: consumption (t=2.08, p<0.05, d=0.07), intention (t=3.01, p<0.01, d=0.12), risk perception (t=-502, p<0.001, d=0.20), attitudes (t=5.77, p<0.001, d=0.21), subjective norm (t=2.17, p<0.05, d=0.08). Cannabis: intention (t=2.98, p<0.01, d=0.11), risk perception (t=-3.99, p<0.001, d=0.14), attitudes (t=6.19, p<0.001, d=0.23).
    Conclusions: Students who received the intervention reduced the favourable attitudes and intention of alcohol and cannabis consumption and alcohol consumption, and increased the risk perception on both substances, with significant differences, but low effect sizes. Related to control group and regarding cannabis there were no significant differences. The program has been shown to be potentially effective, but would benefit from a review for fit to its theoretical framework.

    Abstract The aim of this study was to evaluate how different rearing systems (intensive or semi-extensive) and sexual hormones could affect stress parameters and the development of stress-related intramural coronary arteriosclerosis in Lidia bulls (n = 18) and Piemontese oxen (n = 17). At slaughter, hearts were sampled and submitted to histological investigations in order to evaluate coronary arteriosclerosis. Blood and hair samples were also collected to measure d-ROMS and 20β-dihydrocortisol levels, respectively. No significant differences were recorded for serum levels of d-ROMS between Piemontese oxen and Lidia bulls. On the contrary, Lidia bulls presented higher levels of hair 20β-dihydrocortisol compared to Piemontese oxen (p = .007). Arteriosclerosis was recorded in both the groups. In Lidia bulls, mild to moderate coronary arteriosclerosis was recorded in all the evaluated regions of the heart (right and left papillary muscle; interventricular septum; right and left ventricle free wall; right and left atrium). In Piemontese oxen, mild to severe coronary arteriosclerosis was observed, especially in the left papillary muscle. These results suggest that both cattle breed experienced stress during their rearing cycle. Further studies are needed to clarify the role of housing system and sexual hormones in the development of these subclinical vascular pathology.HighlightsRearing systems can affect animal welfare. Coronary arteriosclerosis, d-ROMS and hair 20β-dihydrocortisol levels can be affected by stress. Lidia bulls and Piemontese oxen experienced different levels of stress during their rearing cycle.

    Abstract Background/aims: Interventional radiologists have played a main role in the technical evolution of gastrostomy, from the first surgical/endoscopical approaches to percutaneous interventional procedures. This study evaluated the results obtained in a 12-year series.
    Methods: During the period December 1996 to December 2008, 254 new consecutive gastrostomies and 275 replacement procedures were performed in selected patients. All of the cases were treated by a T-fastener gastropexy and tube placement. The procedures were assessed by analyzing indications, patient selection, duration of the procedures, and mortality.
    Results: All 254 first gastrostomies were successful; replacement procedures were also successfully performed. One (0.2%) patient with severe neurologic disorders died after the procedure without signs of procedure-related complications, and seven (1.3%) major complications occurred (four duodenal lesions with peritoneal leakage, two gastric bleedings, and one gastric lesion). Minor complications were easily managed; three tube ruptures were resolved.
    Conclusions: This long-term series and follow-up showed that a group of interventional radiologist can effectively provide gastrostomy placement and long-term tube management. Percutaneous gastrostomy is less invasive than other approaches and it satisfies the needs even of high-risk patients.

    Abstract Introduction: In 2008, the US FDA issued an alert about an increased risk of psychiatric events associated with montelukast. Recent national pharmacovigilance analyses in Sweden, France and Spain detected a potential increase in reporting risk of the association.
    Aim: Our objective was to analyse spontaneous reports of psychiatric events in children and adolescents worldwide treated with montelukast.
    Methods: We conducted a retrospective analysis of Individual Case Safety Reports (ICSRs) recorded up to 1 January 2015 in the World Health Organization (WHO) database (VigiBase(®)), in which montelukast was associated with 'psychiatric disorders'. We used the Bayesian Confidence Propagation Neural Network (BCPNN) approach for signal generation.
    Results: A total of 14,670 ICSRs for montelukast were recorded, of which 2630 corresponded to psychiatric disorders in people aged <18 years. The main symptoms reported for infants (aged <2 years) were sleep disorders, for children (aged 2-11 years) the main symptoms were depression/anxiety, and for adolescents (aged 12-17 years) they were suicidal behaviour and depression/anxiety. Suicidal behaviour was over-represented in all age groups with information component (IC) values that reached 5.01 in children and 3.85 in adolescents. Unexpectedly, completed suicides were reported more frequently for children (IC: 3.15; IC025: 1.98) than for adolescents (IC: 3.11; IC025: 2.61) or the total population (IC 1.95; IC025: 1.73).
    Conclusions: Neuropsychiatric disorders as side effects of montelukast were more frequently reported for children than for adults. Infants and children seem to be more prone to sleep disturbances, whereas adolescents present symptoms of depression/anxiety and psychotic reactions more often. Suicidal behaviour and completed suicide appear to be more frequently reported than previously thought in practice. Risk management plans and epidemiological studies are needed to quantify the risk. Practitioners should be aware of the risk of neuropsychiatric events associated with montelukast use, and should advise the patient and report new cases.

    Abstract Purpose: To evaluate the effects of reducing the number of segments in which the intervertebral disc (IVD) can be subdivided on the accuracy in estimating its sGAG content by computation of the parameter DeltaT1 from delayed Gadolinium-Enhanced MRI of Cartilage (dGEMRIC) protocol.
    Materials and methods: Twenty-three herniectomy patients underwent dGEMRIC acquisitions for IVD. Thirty-one tissue samples were obtained at herniectomy from the same patients and biochemically analysed for their sGAG content. Eleven different division schemes (DS) were applied by processing dGEMRIC images, and DeltaT1 values of the segments related to the surgical sampling locations were computed and correlated to the corresponding biochemical data. For each DS, the linear regression and Pearson's coefficient were computed.
    Results: Reducing the number of segments from 48 (4 annular rings and 12 angular sectors) to 12 (2 rings and 6 sectors), correlation with sGAG biochemical data did not decline (r > 0.7).
    Conclusion: A 12-segment DS provided the best compromise between preserving accuracy and reducing the number of segments.

    Abstract Purpose: To retrospectively evaluate the outcome of carotid artery stent placement (CAS) without the use of embolic protection devices (EPDs) in a large cohort of patients.
    Materials and methods: Institutional review board approval and informed consent from all patients were obtained. Preprocedure color Doppler ultrasonography (US), magnetic resonance (MR) imaging, or computed tomography (CT) were used to evaluate stenosis severity (70% or greater). Clinical findings and combined 30-day complication rates in 400 patients (289 men, 111 women; mean age, 73 years +/- 8 [standard deviation]) who underwent unprotected CAS for asymptomatic (n = 156; 39%) or symptomatic (n = 244, 61%) stenoses were analyzed. Follow-up at 30 days included neurologic evaluation and color Doppler US.
    Results: Self-expanding stents were successfully deployed in 397 of 400 (99.25%) patients. Among the 397 patients, nine (2.27%) major complications (all in patients with prior symptoms) had occurred at 30 days, including three (0.76%) major (all in patients who had stopped antiplatelet prophylaxis) and six (1.5%) minor strokes--three intraprocedural and three delayed. Minor complications included 16 (4%) transient ischemic attacks, four in asymptomatic and 12 in symptomatic patients. The 30-day combined adverse outcomes (transient ischemic attack, ipsilateral stroke, death) were significantly correlated with prior presence of symptoms (symptomatic, 8.6%; asymptomatic, 2.6%; P < .03).
    Conclusion: Stent placement without EPD was performed with a high technical success rate. For asymptomatic patients, the combined 30-day adverse-outcomes rate was within the limits recommended by the American Heart Association for carotid endarterectomy and compared favorably with results reported for CAS with EPD. When a transient ischemic attack is excluded, the 30-day combined death and stroke rate among patients with prior symptoms also compared favorably with published results.

    Abstract Our hypothesis was that the enhanced MRI of cartilage (dGEMRIC) imaging protocol could be used in patients to quantify the sulfated glycosaminoglycan (sGAG) in intervertebral discs (IVD). To test this hypothesis, 23 patients with degenerative disc pathology scheduled for surgery were studied by a specific dGEMRIC protocol: each patient underwent two MRI scans, before and 3.5 hr after Gd(DTPA)2-injection of a nonconventional dose of 40 mL. Then, T(1PRE-ENH) and T(1POST-ENH) parametric images of the disc were obtained, from which a new index DeltaT(1) of the molecular status of the IVD was computed (T(1PRE-ENH) - T(1POST-ENH)). A total of 31 tissue samples (one or two from each patient) obtained at herniectomy were collected and biochemically analyzed for sGAG content and used as the gold standard for comparison. DeltaT(1) values in correspondence to degenerated sectors were higher (158 +/- 36 ms) compared to normal sectors (80 +/- 13 ms). Linear regression analysis between MRI-derived and biochemistry-derived measurements resulted in a significant correlation (r = 0.73, P < 0.0001). The DeltaT(1) parametric images, calculated using the modified dGEMRIC technique, provided noninvasive quantitative information about sGAG content within discal tissue in vivo, which resulted in agreement with biochemical analysis. The application of this new MRI method could provide diagnostic information for standard treatment of lumbar discopathy and for innovative therapies of regenerative medicine.

    Abstract Several animal models are currently used for the surgical implantation of either biologic or biopolymeric scaffolds in order to provide in vivo assessment of tissue-engineered heart valves. The Vietnamese pig (VP) is herein proposed as a suitable recipient to test the function of novel bioengineered valve substitutes, in the reconstruction of the right ventricular outflow tract (RVOT). This review aims to provide a complete and exhaustive panel of physiological parameters and methodological information for preclinical studies of tissue-engineered heart valves in the VP animal model.

    Abstract Objectives: Rising trends in eosinophilic esophagitis (EoE) have been repeatedly linked to declining Helicobacter pylori (H. pylori) infection, mostly in retrospective studies. We aimed to prospectively evaluate this inverse association.
    Methods: Prospective case-control study conducted in 23 centers. Children and adults naïve to eradication therapy for H. pylori were included. Cases were EoE patients, whereas controls were defined by esophageal symptoms and <5 eos/HPF on esophageal biopsies. H. pylori status was diagnosed by non-invasive (excluding serology) or invasive testing off proton pump inhibitor (PPI) therapy for 2 weeks. Atopy was defined by the presence of IgE-mediated conditions diagnosed by an allergist.
    Results: 808 individuals, including 404 cases and 404 controls (170 children) were enrolled. Overall H. pylori prevalence was 38% (45% children vs. 37% adults, p 0.009) and was not different between cases and controls (37% vs. 40%, p 0.3; odds ratio (OR) 0.97; 95% confidence interval (CI) 0.73-1.30), neither in children (42% vs. 46%, p 0.1) nor in adults (36% vs. 38%, p 0.4). Atopy (OR 0.85; 95%CI 0.75-0.98) and allergic rhinitis (OR 0.81; 95%CI 0.68-0.98) showed a borderline inverse association with H. pylori infection in EoE patients. This trend was not confirmed for asthma or food allergy.
    Conclusions: H. pylori infection was not inversely associated with EoE, neither in children nor in adults. A borderline inverse association was confirmed for atopy and allergic rhinitis, but not asthma of food allergy. Our findings question a true protective role of H. pylori infection against allergic disorders, including EoE.
