Abstract Background: Social media are widely used information tools, including the medical/health field. Unfortunately, the levels of misinformation on these platforms seem to be high, with a medium-low quality of the proposed content, as evidenced by previous studies. You Tube is one of the most important platforms for audio/video content. It shows content to users through a recommendation algorithm system.
Materials and methods: We have classified in two cohorts the first results obtained by researching "bladder tumor treatment" on You Tube through two different user profiles: "Cohort A" with a not logged-in session in incognito mode (46 videos enrolled) and "Cohort B" with a logged-in session with a physician profile (50 videos enrolled). The videos were evaluated using validated instruments such as DISCERN and PEMAT-AV Furthermore, we used a Likert's scale for the evaluation of levels of misinformation.
Results: Overall quality of information was moderate to poor (DISCERN 3) in 54% of Cohort A and 24% of Cohort B. Moreover, a high degree of misinformation (Likert score 3) was found in 52% of Cohort A cases and 32% of Cohort B.
Conclusions: Levels of misinformation in both cohorts are positively correlated to the number of views per month. Globally, the levels of information quality, understandability and actionability are lower for the results obtained from searches performed with anonymous user profile (Cohort A).

Abstract Respiratory weaning after cardiac surgery can be difficult or prolonged in up to 22.7% of patients. The inability to wean from a ventilator within the first 48 h after surgery is related to increased short- and long-term morbidity and mortality. Risk factors are mainly non-modifiable and include preoperative renal failure, New York Heart Association, and Canadian Cardiac Society classes as well as surgery and cardio-pulmonary bypass time. The positive effects of pressure ventilation on the cardiovascular system progressively fade during the progression of weaning, possibly leading to pulmonary oedema and failure of spontaneous breathing trials. To prevent this scenario, some parameters such as pulmonary artery occlusion pressure, echography-assessed diastolic function, brain-derived natriuretic peptide, and extravascular lung water can be monitored during weaning to early detect hemodynamic decompensation. Tracheostomy is considered for patients with difficult and prolonged weaning. In such cases, optimal patient selection, timing, and technique may be important to try to reduce morbidity and mortality in this high-risk population.

Title translation New perspectives in second-line treatment in non-squamous non-small cell lung cancer.
Abstract The treatment options for patients with non-squamous non-small cell lung cancer (NSCLC) in second-line have been limited for a long time. Docetaxel, pemetrexed and erlotinib are the standard second-line treatment. Docetaxel is frequently used although it is associated with an unfavorable safety profile. Recently, the results from the trial CheckMate 057 with nivolumab, an anti-PD-1 inhibitor, has increased the therapeutic options for patients with non-squamous NSCLC in the second-line setting. The results of this randomized phase III trial comparing nivolumab vs docetaxel have deeply changed our current clinical practice. This review explores the recent findings about nivolumab in this setting by analyzing the results of CheckMate 057 trial findings and discussing their implications in clinical practice. Particular attention has been given to efficacy, patients selection, toxicity profile, and quality of life.

Abstract The emergence of a collective behavior in a many-body system is responsible for the quantum criticality separating different phases of matter. Interacting spin systems in a magnetic field offer a tantalizing opportunity to test different approaches to study quantum phase transitions. In this work, we exploit the new resources offered by quantum algorithms to detect the quantum critical behavior of fully connected spin-1/2 models. We define a suitable Hamiltonian depending on an internal anisotropy parameter γ that allows us to examine three paradigmatic examples of spin models, whose lattice is a fully connected graph. We propose a method based on variational algorithms run on superconducting transmon qubits to detect the critical behavior for systems of finite size. We evaluate the energy gap between the first excited state and the ground state, the magnetization along the easy axis of the system, and the spin-spin correlations. We finally report a discussion about the feasibility of scaling such approach on a real quantum device for a system having a dimension such that classical simulations start requiring significant resources.
