    Abstract Introduction: This study examines the association between perceived stress, death anxiety, psychological resilience and the sociodemographic and clinical features of patients with Coronavirus Disease 2019 (COVID-19).
    Methods: 304 patients with COVID-19 diagnosis, who were admitted to İstanbul University İstanbul Faculty of Medicine Hospital "COVID-19 Patients Monitoring Center" were recruited. No sample selection was made, all the patients who were followed up and treated in the center were included. Data was collected by the researchers through face-to-face interviews using the Sociodemographic Information and Disease Progression Form, Psychological Hardiness Scale (PHS), Perceived Stress Scale (PSS), and Templer Death Anxiety Scale (TDAS).
    Results: Women scored higher in PSS and TDAS. Participants with chronic diseases reported higher death anxiety whereas perceived stress was higher in individuals with psychiatric disorders and ones without a history of intensive care unit stay. Participants without psychiatric disorders, who had longer hospitalization and who fully recovered scored higher in PHS-Control. Patients' report of negative attitudes from their relatives/friends was associated with lower scores in PHS. Perceived stress was correlated with death anxiety and psychological resilience.
    Conclusion: Being female, comorbid physical and mental illnesses, continuation of disease symptoms and low psychological resilience were found to be risk factors in terms of stress and death anxiety in COVID-19 patients. These vulnerable groups need to be closely evaluated with a bio-psychosocial approach and provided psychological support during the course of the disease. Health institutions are recommended to conduct medical treatment in cooperation with psychological care.

    Abstract Aim:We investigated the level of knowledge about influenza, pneumococcal, herpes zoster and tetanus vaccines recommended by the World Health Organization (WHO) for individuals aged 65 and over.Methods:A total of 147 patients aged 65 and over who attended to the Internal Diseases Outpatient Clinic in İstanbul University Faculty of Medicine Hospital were included in this study. A 21-item questionnaire was administered to the participants.Results:Fifty-three point seven percent of the elderly who participated in the study had been vaccinated. Influenza vaccine was the most known (56.5%), and tetanus vaccine was the most commonly used (32.7%) vaccine. When the 65-74 age group was taken as a reference, the percentage of adults aged 75 years and older who were not vaccinated was 2.56 times higher [confidence interval (CI): 1.20-5.44; p=0.014]; when those who received information about the vaccines were taken as a reference, in those who did not receive information, the percentage of adults who were not vaccinated was 2.48 times higher (CI: 1.19-5.18; p=0.016); when patients with chronic diseases were taken as reference, in patients without any chronic disease, the percentage of individuals who were not vaccinated was 6.30 times higher (CI: 1.60-24.85; p=0.009).Conclusion:It is important for the elderly population to have vaccines recommended by WHO, healthcare professionals to inform people about vaccines and to recommend these vaccines. New regulations should be made for the immunization of people aged 75 and over. In this context, family physicians may be recommended to follow the immunization status of this age group specifically or to provide immunization services to these people within the framework of home health services.

    Abstract The aim of this study was to investigate the reinfection rates and characteristics of SARS-CoV-2 in individuals with SARS-CoV-2 RNA present in their clinical specimens for COVID-19. Our data from the COVID-19 Laboratory of Istanbul University were analyzed for 27,240 cases between 27 March 2020 to 8 February 2022. Demographic characteristics, vaccination statuses, comorbidities, and laboratory findings were evaluated in cases with suspected reinfection, as determined by the presence of SARS-CoV-2 RNA at a rate of 0.3% in clinical specimens.     Abstract Objective: Coronavirus disease 2019 (COVID-19) is caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Renin-angiotensin-aldosterone-system (RAAS) inhibitors may increase the expression of angiotensin-converting enzyme 2, which is the receptor for SARSCoV-2 Spike protein. The consequences of using angiotensin-converting enzyme inhibitors (ACEi) and angiotensin receptor blockers (ARB) during the COVID-19 pandemic are unknown.
    Methods: A retrospective cohort study aiming to identify the odds of severe disease (defined as either hospitalization of ≥14 days, admission to the intensive care unit, or death) associated with exposure to ACEi or ARB was conducted. Adult patients (age ≥18 years) with COVID-19 admitted to the İstanbul Faculty of Medicine Corona Center between March 9 and May 11, 2020, were included. Chronic users of ACEi, ARB, or other antihypertensive drugs were matched according to age, sex, sick days before hospitalization, comorbidities, smoking, number of antihypertensive regimens, doxazosin use, furosemide use, and serum creatinine level. Odds ratios (OR) of having severe disease were calculated.
    Results: In total, 611 patients were admitted with COVID-19, confirmed by either reverse-transcriptase polymerase chain reaction or computed tomography (CT). There were 363 males, and the age ranged from 18 to 98 years, with an average age of 57±15 years. Of these, 165 participants had severe disease (53 deaths, case fatality rate: 8.7%). Among those with hypertension (n=249), ARB exposure was compatible with decreased odds (OR=0.60, 95% CI: 0.27-1.36, p=0.31) of severe disease though not statistically significant, while ACEi exposure significantly reduced the risk of severe disease (OR=0.37, 95% CI: 0.15-0.87, p=0.03). ACEi exposure was associated with milder infiltrations seen on baseline CT, lower C-reactive protein and ferritin, higher monocytes, shorter hospitalization, and less requirement for specific empirical treatments (favipiravir and meropenem).
    Conclusion: Our data suggest that exposure to ACEi drugs may have favorable effects in the context of COVID-19 pneumonia.

    Enhancing Medical Image Segmentation via Text-Guided Diffusion Model

    Abstract Large-scale, big-variant, and high-quality data are crucial for developing robust and successful deep-learning models for medical applications since they potentially enable better generalization performance and avoid overfitting. However, the scarcity of high-quality labeled data always presents significant challenges. This paper proposes a novel approach to address this challenge by developing controllable diffusion models for medical image synthesis, called EMIT-Diff. We leverage recent diffusion probabilistic models to generate realistic and diverse synthetic medical image data that preserve the essential characteristics of the original medical images by incorporating edge information of objects to guide the synthesis process. In our approach, we ensure that the synthesized samples adhere to medically relevant constraints and preserve the underlying structure of imaging data. Due to the random sampling process by the diffusion model, we can generate an arbitrary number of synthetic images with diverse appearances. To validate the effectiveness of our proposed method, we conduct an extensive set of medical image segmentation experiments on multiple datasets, including Ultrasound breast (+13.87%), CT spleen (+0.38%), and MRI prostate (+7.78%), achieving significant improvements over the baseline segmentation methods. For the first time, to our best knowledge, the promising results demonstrate the effectiveness of our EMIT-Diff for medical image segmentation tasks and show the feasibility of introducing a first-ever text-guided diffusion model for general medical image segmentation tasks. With carefully designed ablation experiments, we investigate the influence of various data augmentation ratios, hyper-parameter settings, patch size for generating random merging mask settings, and combined influence with different network architectures.
