    Title translation Ultrasound-guided radiofrequency ablation of the sural nerve for chronic ankle pain: A case report.
    Abstract Chronic lateral ankle pain related to sural neuralgia is a rare pathology. The sural nerve innervates the sensitivity of the posterolateral border of the leg, as well as the dorsolateral border of the foot. On occasions, sural neuralgia is resistant to conservative treatment and can affect the patient's psycho-emotional and social sphere. We describe the case of a 54-year-old patient with neuropathic pain in the sural territory and a history of several ankle surgeries. After unsuccessful conservative treatment, ultrasound-guided ablative radiofrequency is performed in the sural nerve with subsequent complete cessation of pain without side effects. We propose to give importance to ecopalpation in the consultation of a rehabilitation physician, as well as to describe ultrasound-guided ablative radiofrequency as a safe and effective technique for sural neuralgia that does not respond to conservative treatment. However, more quality studies are needed to corroborate these results.

    Abstract Objetivo: Describir el perfil de multirresistencia (MDR), resistencia extendida (XDR) y panresistencia (PDR) a antibacterianos en aislados de muestras de pacientes de un hospital privado de tercer nivel en el norte de México.
    Método: Se realizó un estudio retrospectivo durante el periodo comprendido de febrero de 2016 a abril de 2019. A partir de 156 muestras clínicas de orina, heridas, sangre, expectoración y otros fluidos se aislaron 166 bacterias; 10 de las muestras incluyeron dos microorganismos. Los microrganismos aislados se clasificaron en MDR, XDR o PDR.
    Resultados: El 78% de los aislados gramnegativos y el 69% de los aislados grampositivos mostraron MDR, XDR o PDR. Staphylococcus epidermidis fue la bacteria grampositiva con multirresistencia más frecuentemente aislada. Escherichia coli y Klebsiella sp. se encontraron entre los gramnegativos MDR más frecuentes. En dos casos, los aislados clínicos de Pseudomonas aeruginosa procedentes de la unidad de cuidados intensivos neonatales mostraron PDR.
    Conclusión: Los servicios de terapia intensiva, cirugía y unidad de cuidados intensivos neonatales merecen especial atención por la alta proporción de aislados MDR y la presencia de PDR a causa de P. aeruginosa.
    Objective: To describe the profile of multidrug-resistance (MDR), extensively resistance (XDR) and pandrug-resistance (PDR) to antibacterial drugs in isolates from patient samples from a third level private hospital in the North of Mexico.
    Method: A retrospective study was carried out during the period from February 2016 to April 2019. From 156 clinical samples of urine, wounds, blood, expectoration and other fluids, 166 bacteria were isolated; 10 samples included two microorganisms. Isolated microorganisms were classified into MDR, XDR or PDR.
    Results: 78% of the Gram negative and 69% of the Gram positive isolates showed MDR, XDR or PDR. Staphylococcus epidermidis was the most frequently isolated MDR Gram positive bacteria. Escherichia coli and Klebsiella sp. were among the most frequent MDR Gram negative. In two cases, the clinical isolates of Pseudomonas aeruginosa from the neonatal intensive care unit showed PDR.
    Conclusions: The intensive care, surgery and neonatal intensive care unit services deserve special attention due to the high proportion of MDR isolates and the presence of PDR due to P. aeruginosa.

    Abstract PURPOSE OF THE STUDY Laboratory methods are central to prosthetic joint infection (PJI) diagnosis. Most research teams focus on detection of specific inflammatory markers, causative pathogens, or on assessment of the tissue response. This study sought to determine the optimal cut-off values and diagnostic performance of selected synovial markers in relation to the diagnosis of hip or knee PJI. The studied markers were synovial level of glucose, lactate, coefficient of energy balance (CEB) and NGAL (neutrophil gelatinase-associated lipocalin). MATERIAL AND METHODS This prospective study includes 89 patients who underwent revision total knee or hip arthroplasty for septic or aseptic reasons in the period from 2014 to 2017. Among these 89 patients, there are 2 cases of prosthetic hip infection, 22 cases of prosthetic knee infection, 31 aseptic revision total hip arthroplasties and 34 aseptic revision total knee arthroplasties. The diagnostic characteristics of the studied methods were set in relation to the reference standard, the 2013 MSIS (Musculoskeletal Infection Society) criteria. The cut-off values were calculated using the ROC (receiver operating characteristic curve) analysis. RESULTS The synovial glucose test is considered positive if the glucose level drops below 2.65 mmol/L. The area under the curve is 0.813, sensitivity 75.0%, specificity 83.1%. The synovial lactate test is considered positive if lactate level rises above 8.87 mmol/L. The area under the curve is 0.882, sensitivity 70.8%, specificity 95.4%. Synovial NGAL is considered positive if its level exceeds 998 μg/L. The area under the curve is 1.000, sensitivity 100.0%, specificity 100.0%. CEB is considered positive if its value is lower than +4.665. The area under the curve is 0.883, sensitivity 91.7% and specificity 69.8%. Combining of these tests with other synovial markers does not improve the diagnostic performance of the studied tests. CONCLUSIONS The glucose and lactate levels and CEB undoubtedly reflect the presence of an inflammatory process in a prosthetic joint. However, the diagnostic characteristics of these tests are not better than those of other modern diagnostic techniques. As opposed to these tests, synovial NGAL shows excellent diagnostic performance. Nonetheless, the potential of this method shall be verified on larger cohorts of patients.     Abstract Aim: Pilonidal sinus or Pilonidal Disease (PD) is a relatively common, benign but challenging condition. Although commonly encountered in practice, its ideal treatment is controversial. One of the most validated treatments is video-assisted surgery. In this context, very similar endoscopic techniques have been published under different names. The aim of this systematic review is to assess the differences among these proposed techniques and their outcomes.
    Methods: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed during all stages of this systematic review. A systematic search of the English literature was performed on multiple databases from 1 January 2014 to 3 April 2022. The primary outcome measure was the IDEAL framework stage of innovation. The key secondary outcome measures were the adherence to the IDEAL reporting guidelines, the Blencowe framework and the core outcome set (COS) for studies evaluating the introduction and evaluation of novel surgical techniques, the qualitative assessment using appropriate tools, the procedural variations and outcomes of each technique.
    Results: A total of 38 articles were included reporting a very similar technique under eight different acronyms. The number of patients varied from 9 to 250. Mean follow-up ranged from 1 to 60 months. There was only one published study of IDEAL 3. The majority (58%) were IDEAL 2a studies. Reporting of domains in the IDEAL reporting guidelines and Blencowe framework was poor, with most studies not reporting the component steps of procedures or efforts to standardize them. Half of COS domains were markedly underreported. The quality of the evidence was categorized as having a risk of bias from moderate to critical level in all nine comparative non-randomized series. Postoperative complications occurred in 0%-6% of cases, including surgical site infection, poor or failed wound healing bleeding, granuloma, haematoma, and pain requiring intervention. The recurrence rate varied from 0% to 22%.
    Conclusion: The study demonstrates that reporting on technical aspects of interventions for PD is poor, thus warranting a better-quality control of surgical techniques. It is advisable to group all endoscopic procedures under the umbrella term of 'endoscopic sinusectomy', thus embracing the two main principles of this technique, that is, video assistance and PD ablation.

    Abstract Handgrip strength is a robust indicator of the biological health of children and adolescents. Objecti ves: to identify the anthropometric characteristics and body composition related to handgrip stren gth, and to describe the main characteristics of the protocols used for its evaluation.
    Methodology: A scoping review was developed; the search was carried out in 1.) Medline; 2.) Web of Science; 3.) Science Direct; 4.) Scielo; and 5.) EBSCO's Sportdiscus. Original investigations were included if they have handgrip strength data, anthropometric characteristics, and body composition.
    Results: 59 reports published between 2005 and 2020 were selected. Among them, nutritional classification was the most studied characteristic (n = 25; 42.3%), followed by body mass index (n = 23; 38.9%). In 47.5% (28 reports), the number of tests performed on each individual to estimate the handgrip strength value was not clear. The dynamometer brand Takei Scientific Instrument Co. Ltd. was the most used, with 49.2 %. The most widely used indicator was absolute handgrip (86.4%, 51 reports).
    Conclusions: There is a wide variety of protocols used for handgrip strength assessment with va riations in body position, arm and hand selection, number of repetitions, and intervals between measurements. A proportional pattern of values was found between absolute handgrip strength and body mass index, Σ skinfolds, however, when adjusted by mass (relative handgrip strength), the relationship is inverse.

    Abstract The aim of this work was to establish local diagnostic reference levels (DRLs) for retrograde wedge portography (RWP) performed on pediatric patients assessing the usefulness of radiation dose monitoring software in the establishing process. Between September 2016 and April 2020, 66 consecutive RWP were performed at a transplantation institute and were included in our study. Patients were divided in three groups according to age: n = 25 infants, n = 20 middle childhood and n = 21 early adolescence. The third quartile of both Air Kerma at the reference point (Ka,r) and air kerma-area product (PKA) were evaluated to establish local DRLs (lDRLs). In addition, to control high Ka,r levels during procedures, the software notified to operators if Ka,r exceeded the dose 'alert' threshold set at 2 Gy. lDRLs were established for all three groups using PKA and Ka,r: infant group: 5.6 Gy.cm 2 and 0.034 Gy; middle childhood: 6.4 Gy.cm2 and 0.018 Gy and early adolescence: 12.8 Gy.cm2 and 0.059 Gy. The dose threshold 'alert' was never encountered (alert quota: 0%). The dose monitoring system supports the feasibility of accurate and easier lDRLs' establishment.

    Abstract Introducción. Internet ha logrado imbricarse de manera vertiginosa e insospechada en todas las esferas de la vida cotidiana; las formas de ser y ejercer como madres y padres en la contemporaneidad no han quedado al margen de estos avatares. Las tecnologías digitales adquieren un lugar protagónico en la forma como mujeres y hombres configuran y agencian sus parentalidades, incidiendo en sus significados, saberes, sentires y prácticas. Método. Este estudio realizó una revisión sistemática de la producción científica que explora las trayectorias y las transformaciones de las subjetividades parentales que se despliegan a partir de mediaciones digitales en la contemporaneidad durante los años 2010-2020. Se realizó una búsqueda en tres bases de datos: Scopus, EBSCOhost y ScienceDirect, recuperando, una vez aplicados los criterios de inclusión y exclusión, un total de 55 artículos. Resultados. Se encontraron tres líneas principales de estudio sobre el tema: (1) Pesquisas parentales, en el que las madres, y más recientemente, los padres, se sirven de las mediaciones digitales para la gestión instrumental, simbólica y emocional de sus parentalidades, especialmente en momentos de transición, crisis y situaciones adversas. (2) Disputas discursivas de las parentalidades hegemónicas, donde las prácticas maternas y paternas, reservadas tradicionalmente a la esfera privada, se hacen públicas a través de lo digital para su significación, debate y renegociación. (3) Emergencias en los usos y apropiaciones parentales, que remite a los mayores usos de Internet en madres y padres en torno a la sociedad de consumo, la conciliación laboral, las preocupaciones por la privacidad y el activismo social. Discusión y conclusiones. Las interacciones facilitadas por Internet permiten la comprensión de fenómenos culturales y sociales más amplios que permean las subjetividades parentales, las cuales requieren seguir investigándose en sus diferentes aristas y posibilidades.
