    Abstract Objective: The objective of this study was to explore diet patterns in children with tympanostomy tube placement (TTP) complicated by postoperative tympanostomy tube otorrhea.
    Study design: Cross-sectional survey and retrospective cohort study.
    Methods: Caregivers of children (0-12 years old), at a tertiary-care pediatric hospital who underwent TTP within 6 months to 2 years prior to enrollment were included. Children with a history of Down syndrome, cleft palate, craniofacial syndromes, known immunodeficiency, or a non-English-speaking family were excluded. Our primary outcome variable was the number of otorrhea episodes. The primary predictor was diet patterns, particularly dessert intake, which was captured through a short food questionnaire.
    Results: A total of 286 participants were included in this study. The median age was 1.8 years (IQR, 1.3, 2.9). A total of 174 (61%) participants reported at least one episode of otorrhea. Children who consumed dessert at least two times per week had a higher risk of otorrhea compared to children who consumed one time per week or less (odds ratio [OR], 3.22, 95% Confidence Interval [CI]: 1.69, 6.12). The odds ratio increase continued when considering more stringent criteria for otorrhea (multiple episodes or one episode occurring 4 weeks after surgery), with a 2.33 (95% CI: 1.24, 4.39) higher odds of otorrhea in children with dessert intake at least 2 times per week.
    Conclusions: Our pilot data suggest that episodes of otorrhea among children with TTP were associated with more frequent dessert intake. Future studies using prospectively administered diet questionnaires are necessary to confirm these findings.

    Abstract Objective: To determine whether publically funded 'reablement services' have any effect on patient health or use of services.
    Design: Systematic review of randomized controlled trials and non-randomized studies in which reablement interventions were compared with no care or usual care in people referred to public-funded personal care services. Data sources included: Cochrane Central Register of Controlled Trials, EPOC register of studies, trials registers, Medline, EMBASE, and CINHAL. Searches were from 2000 up to end February 2015.
    Setting: Not applicable.
    Participants: Investigators' definition of the target population for reablement interventions.
    Main outcome measures: Use of publically funded personal care services and dependence in personal activities of daily living.
    Results: We found no studies fulfilling our inclusion criteria that assessed the effectiveness of reablement interventions. We did note the lack of an agreed understanding of the nature of reablement.
    Conclusions: Reablement is an ill-defined intervention targeted towards an ill-defined and potentially highly heterogeneous population/patient group. There is no evidence to suggest it is effective at either of its goals; increasing personal independence or reducing use of personal care services.

    Abstract As a feature of the Fish Revolution (1400-1700), the early modern "invention" of the Grand Banks in literary and cartographical documents facilitated a massive and unprecedented extraction of cod from the waters of the north Atlantic and created the Cod/Sack trade Triangle. This overlapped with the southern Atlantic Slave, Sugar, and Tobacco Triangle to capitalise modern European and North American societies. In 1719, Pierre de Charlevoix claimed that the Grand Banks was "properly a mountain, hid under water," and noted its cod population "seems to equal that of the grains of sand which cover this bank." However, two centuries later in 1992, in the face of the collapse of the fishery, and fearing its extinction, a moratorium was placed on five centuries of harvesting Grand Banks cod. The invention and mining of its waters serves as a bellwether for the massive resource extractions of modernity that drive the current leviathan and "wicked problem" of global warming. The digital environmental humanities narrative of this study is parsed together from 83 pieces of Grand Banks charting from 1504 to 1833, which are juxtaposed through Humanities GIS applications with English and French cod-catch records kept between 1675 and 1831, letters regarding Cabot's 1497 voyage, Shakespeare's

    Abstract Trade-offs involving land use change, cultural values, water resources and jobs are critically important to understand the opportunity cost of resource extraction. Stated preference techniques can be particularly useful in eliciting the non-market values expressed as trade-offs. This study assesses preferences over the management of groundwater released from deep mining operations in Western Australia. A discrete choice experiment is used to investigate the trade-offs Australians are prepared to make for remote economic, ecological and cultural goods against costs. The results suggest that there is heterogeneity of preferences as indicated by a three-class structure of a latent class model. One class supports the use of released groundwater across a range of economic, ecological and cultural uses modelled: extending town water supply, restoring rangeland habitat, creating jobs for Aboriginal Australians and preserving cultural waterholes. The smallest class supports all these uses except job creation and the final class only supports preserving cultural waterholes. These results illustrate public attitudes towards cultural values as well as wider environmental policy tensions between instrumental and intrinsic values.
