    Abstract Introduction Glanzmann's thrombasthenia (GT) is an autosomal recessive disorder of platelets caused by a deficiency in the glycoprotein IIb-IIIa. Bleeding from the skin, mucous membranes, and ecchymosis are symptoms manifested starting in early childhood. There may also be major bleeding conditions as a result of surgical procedures or trauma. The treatment is based on platelet transfusions, antifibrinolytic agents, and recombinant activated factor VII (rFVIIa). Objective To describe the demographic and clinical characteristics as well as the main treatment strategies used during bleeding events or procedures for patients diagnosed with GT that required inpatient attention at a university hospital, a referral center specialized in hemostasis, in the city of Bogota. Materials and methods A descriptive retrospective cohort study was done over a period of 10 years that included nine patients over 18 years of age diagnosed with GT. Results A total of 34 admissions were reported, 23 due to bleeding and 11 for scheduled surgery. Some of the admissions for bleeding (38%) (n=13) required surgical procedures. Overall, 23 surgical procedures were done, six of which were classified as major. Seventy-seven percent of the patients were women with a median age of 37. Their most common symptoms were mucosal and genitourinary bleeding. The use of antifibrinolytics was registered in 28 events, followed by the use of platelet transfusion in 19, and the use of rFVIIa in 17. The average hospital stay was eight days. Conclusion The characteristics registered and the treatments established for this cohort of Colombian patients with GT are similar to those reported in other hospitals around the world. GT presents diagnostic and therapeutic challenges and, therefore, acquiring more knowledge about this pathology is needed within this context.

    Abstract Background: Several MRI findings of optic neuritis (ON) have been described and correlated with specific underlying etiologies. Specifically, optic nerve enhancement is considered an accurate biomarker of acute ON.
    Objective: To identify differences in MRI patterns of optic nerve enhancement in certain demyelinating etiologies presenting with acute ON.
    Methods: Retrospective analysis of enhancement patterns on fat-suppressed T1-weighted images from patients presenting clinical and radiological acute ON, treated at our institution between January 2014 and June 2022.     Results: Fifty-six subjects met eligibility criteria. Mean age was 31 years (range 6-79) and 70% were females.     Abstract Background: Approximately 3/4 of ovarian cancers are diagnosed in advanced stages, with the high-grade epithelial ovarian carcinoma (EOC) accounting for 90% of the cases. EOC present high genomic instability and somatic loss-of-function variants in genes associated with homologous recombination mutational repair pathway (HR), such as BRCA1 and BRCA2, and in TP53. The identification of germline variants in HR genes in EOC is relevant for treatment of platinum resistant tumors and relapsed tumors with therapies based in synthetic lethality such as PARP inhibitors. Patients with somatic variants in HR genes may also benefit from these therapies. In this work was analyzed the frequency of somatic variants in BRCA1, BRCA2, and TP53 in an EOC cohort of Brazilian patients, estimating the proportion of variants in tumoral tissue and their association with progression-free survival and overall survival.
    Methods: The study was conducted with paired blood/tumor samples from 56 patients. Germline and tumoral sequences of BRCA1, BRCA2, and TP53 were obtained by massive parallel sequencing. The HaplotypeCaller method was used for calling germline variants, and somatic variants were called with Mutect2.
    Results: A total of 26 germline variants were found, and seven patients presented germline pathogenic or likely pathogenic variants in BRCA1 or BRCA2. The analysis of tumoral tissue identified 52 somatic variants in 41 patients, being 43 somatic variants affecting or likely affecting protein functionality. Survival analyses showed that tumor staging was associated with overall survival (OS), while the presence of somatic mutation in TP53 was not associated with OS or progression-free survival.
    Conclusion: Frequency of pathogenic or likely pathogenic germline variants in BRCA1 and BRCA2 (12.5%) was lower in comparison with other studies. TP53 was the most altered gene in tumors, with 62.5% presenting likely non-functional or non-functional somatic variants, while eight 14.2% presented likely non-functional or non-functional somatic variants in BRCA1 or BRCA2.

    Title translation O que contribui para a resolubilidade na Atenção Primária à Saúde? Revisão integrativa da literatura, 2010-2020.
    Abstract Primary Health Care (PHC) intends to rearrange services to make it more effective. Nevertheless, effectiveness in PHC is quite a challenge. This study reviews several articles regarding the effectiveness improvements in PHC between 2010 and 2020. Ninety out of 8,369 articles found in PubMed and the Virtual Health Library databases search were selected for thematic analysis using the Atlas.ti® 9.0 software. There were four categories identified: strategies for monitoring and evaluating health services, organizational arrangements, models and technologies applied to PHC. Studies concerning the sensitive conditions indicators were predominant. Institutional assessment programs, PHC as a structuring policy, appropriate workforce, measures to increase access and digital technologies showed positive effects. However, payment for performance is still controversial. The expressive number of Brazilian publications reveals the broad diffusion of PHC in the country and the concern on its performance. These findings reassure well-known aspects, but it also points to the need for a logical model to better define what is intended as effectiveness within primary health care as well as clarify the polysemy that surrounds the concept. We also suggest substituting the term "resolvability", commonly used in Brazil, for "effectiveness".

    Abstract A significant part of the worldwide population is affected by some mental disorder. Previous research conducted with the general population has revealed poor knowledge when it comes to mental health. Therefore, it is imperative to assess mental health literacy using robust assessment tools. Thus, this study aimed to translate, adapt and assess the psychometric properties of the Mental Health Knowledge Questionnaire in a sample of higher education students in Portugal. This study used a sample consisting of 2887 participants. Internal consistency for the psychometric study was estimated using Cronbach's alpha coefficient. Construct validity was tested using exploratory and confirmatory factor analysis, convergent validity, and discriminant validity. After data analysis, the final Portuguese version of the Mental Health Knowledge Questionnaire was composed of 14 items. The goodness-of-fit indices were adequate, confirming the quality of the model's fit to the empirical data (χ²/df = 2.459, GFI = 0.983, CFI = 0.969, RMSEA = 0.032, RMR = 0.023, SRMR = 0.032). This assessment tool is valid and reliable to assess higher education students' mental health literacy in Portugal. Analyses to confirm the scale's external validity, measurement equivalence and replicability are still required.

    Abstract RESUMEN El presente escrito se vincula a dos proyectos institucionales de investigación científica, en los que se ha propuesto indagar sobre prácticas de cuidado en salud mental durante la pandemia de Covid-19. El objetivo de este trabajo es describir y analizar prácticas de salud/salud mental centradas en el sostenimiento del vínculo en la atención y el fortalecimiento de redes productoras de cuidados en contexto de pandemia, desarrolladas en el Área Metropolitana de Buenos Aires, Argentina. Se trata de un estudio exploratorio y descriptivo que utiliza técnicas cualitativas para la recolección y análisis de datos. A partir de la reconstrucción de cuatro breves relatos vivenciales de nuestra propia práctica profesional, se abordan diversos núcleos temáticos de caracterización y análisis de la experiencia. Se concluye que estas prácticas recuperan la perspectiva de cuidados en salud/salud mental a través de la humanización del vínculo y la inclusión de la dimensión afectiva en la atención. Se trata de prácticas que, desde territo- rios institucionales muy diversos y por medio de dispositivos flexibles y creativos, han logrado sostener procesos participativos, afianzar abordajes interdisciplinarios y fortalecer las redes comunitarias de cuidados en un contexto de excepcionalidad y alta complejidad.
