    Abstract Endothelin (Et) and nitric oxide (NO) may serve as chemical mediators of hypoxia-induced pulmonary hypertension. Plasma levels of Et1 were elevated 2-fold while levels of nitrate, a NO metabolite, decreased in rats exposed to 10 days of hypoxia (10% O2). Administration of l-arginine, the precursor for NO, decreased Et, increased nitrate, and decreased right ventricular hypertrophy in hypoxic animals. By increasing plasma NO levels, the right ventricular hypertrophy and right heart failure seen in hypoxia-induced pulmonary hypertension in human patients may be prevented.

    Abstract Low-noise surfaces have become a common mitigation action in the last decade, so much so that different methods for feature extraction have been established to evaluate their efficacy. Among these, the Close Proximity Index (CPX) evaluates the noise emissions by means of multiple runs at different speeds performed with a vehicle equipped with a reference tire and with acoustic sensors close to the wheel. However, signals acquired with CPX make it source oriented, and the analysis does not consider the real traffic flow of the studied site for a receiver-oriented approach. These aspects are remedied by Statistical Pass-By (SPB), a method based on sensor feature extraction with live detection of events; noise and speed acquisitions are performed at the roadside in real case scenarios. Unfortunately, the specific SPB requirements for its measurement setup do not allow an evaluation in urban context unless a special setup is used, but this may alter the acoustical context in which the measurement was performed. The present paper illustrates the testing and validation of a method named Urban Pass-By (U-SPB), developed during the LIFE NEREiDE project. U-SPB originates from standard SPB, exploits unattended measurements and develops an in-lab feature detection and extraction procedure. The U-SPB extends the evaluation in terms of before/after data comparison of the efficiency of low-noise laying in an urban context while combining the estimation of long-term noise levels and traffic parameters for other environmental noise purposes, such as noise mapping and action planning.

    Abstract Purpose of review: Patients with slowly progressive and/or symptomatic oligometastatic radioactive iodine refractory (RAIR) differentiated thyroid carcinomas (DTCs) are candidates to receive locoregional treatment to delay the start of systemic therapy with multikinase inhibitors. Information provided by the recent literature has not been extensively reviewed in previous published works, thus we aim to bridge this gap.
    Recent findings: We present for each metastatic site the different locoregional treatment options, contraindications and potential adverse events. Some techniques can be combined together, whereas others are discouraged in certain situations, requiring a high level of expertise and multidisciplinarity in the treatment algorithm.
    Summary: Different techniques of radiation therapy and interventional radiology allow to control the metastatic spread. However, as no clinical trials are available to compare the treatment schemes in terms of safety and potential impact on the prognosis, the most appropriate option for each patient should be selected within a multidisciplinary decision making, taking into account the clinical conditions and the pattern/rapidity of metastatic disease.

    Abstract The development of deintensified therapies aiming at reducing the treatment-related toxicity while not impairing the outcome is an unmet need in HPV+ oropharyngeal carcinoma patients. Several deintensification approaches have been explored in this setting, including induction chemotherapy with reduced chemoradiation dose in responding pts. The findings of the studies conducted so far show that the available deescalated therapy should be based on the disease risk profile. At present, we still lack robust high level data to infer that the cited interventions are equivalent or superior to standard of care treatments. However, literature data suggest that approximately 70% of low-risk patients might receive a deintensified approach by including either surgery or induction chemotherapy. In the context of high-risk disease, more than 80% of patients may undergo a deintensified loco-regional treatment by neoadjuvant systemic therapy. Epidemiological data suggest that in the next decades, most of the HPV-related OPSCC patients will be made of old and possibly frail individuals. However, this patient population was excluded from most of the studies conducted so far. Therefore, there is a strong need for clinical trials to define risk-based deescalation strategies in this population and in younger patients as well.
