    Title translation About the safety of application of nasal decongestants in the practice of pediatricians.
    Abstract Undesirable effects of the application of the intransal vasoconstricting medications are a frequent occurrence in the pediatric practice. The objective of the present study was to evaluate the role of the intranasal vasoconstricting medications in the structure of the means and methods currently available for the treatment of toxicological pathologies based at a multi-field clinical hospital. The retrospective analysis of the medical histories of the patients admitted to the toxicological department and annual reports for the period from 2015 to 2016 was undertaken. The study has demonstrated that intoxication associated with the use of the intranasal vasoconstricting medications was the most common cause of hospitalization of the children in the toxicological departments. Intoxication of this origin accounted for 15-20% of the total number of toxicological pathologies among the children. The cases of intoxication are most frequently documented in the group of children at the age between 1 and 3 years. The risk of the undesirable serious complications is especially high after the application of naphazoline-based intranasal vasoconstricting medications (71.7-77.4% of all the cases of intoxication with these products). It is concluded that the use of intranasal vasoconstricting medications in the pediatric practice should be carried out under the strict control, with the naphazoline-based preparations being totally excluded from the application.

    Abstract During the pandemic, an urgent task has become to develop new vaccine platforms that will help fight the infection caused by SARS-CoV-2 and quickly respond to newly emerging pathogens. Plant viruses can make a significant contribution to the solution of this problem. Phytoviruses, having the properties of any viral particles (self-assembly, immunogenicity, nanosize), are safe for humans since plants and mammals have no common infectious agents. As a result of thermal rearrangement of the tobacco mosaic virus, spherical particles of a protein nature have been obtained, which have unique immunostimulation and adsorption properties and can play the role of a universal adjuvant platform to create vaccines. Based on these particles, a scheme for obtaining vaccine preparations is proposed. This technology resembles a toy construction set for children. The basis is spherical particles, on the surface of which there are toy blocks-antigens. The "blocks" can be removed, added, or replaced, and this does not take much time and resources. Based on spherical particles, a polyvalent vaccine candidate against COVID-19 has been created as an adjuvant platform.

    Abstract Hemostasis system in patients with schizophrenia and schizophrenia spectrum disorders was examined using "Fibrinodynamics" technology that allows evaluating the overal coagulation, fibrinolysis, and hemostasis potentials. All indices, except for the parameter characterizing the hemostasis balance, were statistically higher (p<0.0001) in patients than in healthy volunteers (control). In most patients, the hemostasis system remained balanced despite changes in individual coagulation and fibrinolysis parameters. The obtained results have a fundamental importance, expand the understanding of the pathogenesis of endogenous mental disorders, and are useful for the development of personalized approaches to the treatment of these patients.

    Abstract Trigeminal autonomic cephalgias is a group of primary headaches, including cluster headache, paroxysmal hemicrania and hemicrania continua, as well as two forms of short- lasting unilateral neuralgiform headache attacks, the complexity of diagnosis of which is determined by the low prevalence and some similarity of clinical manifestations both among themselves and with other diseases in particular with migraine and trigeminal neuralgia. Despite the rather short duration of headache attacks, the intensity of the pain syndrome reaches a severe and very severe degree, and the high frequency of paroxysms per day significantly complicates abortion treatment and leads to a pronounced professional and social maladjustment. At the same time, the possibility of using effective specific prophylactic therapy determines the importance of accurate diagnosis and, therefore, the knowledge of neurologists on this issue.

    Abstract The lessons of COVID-19 have confirmed the effectiveness of measures aimed at strict self-isolation and quarantine. In the People's Republic of China, where any violation of the regime requirements was qualified as a criminal offense, the virus was neutralized when the pandemic was only gaining momentum in Europe and America. However, without proper organization of a restrictive regime, self-isolation can lead to negative consequences in terms of deterioration of health and standard of living. The pandemic has undoubtedly significantly complicated our lives, has taken millions of lives, caused disability, deterioration of material status, rupture of marital ties. One of the reasons for this development of events was the unsatisfactory organization of leisure during self-isolation. The inability to calmly wait out, endure COVID-associated adversities in most countries caused mass popular unrest, gave rise to panic moods. Only those who managed to curb the negative consequences and direct them in the right direction managed to come out of the pandemic with dignity. The authors have developed an organizational technology of the forced self-isolation regime based on the complex of measures and proposed this technology for introduction to daily life of the population during the period of restrictive regimes. The authors are sure that it is especially important to organize leisure and create the most comfortable conditions of the isolated living for the elderly, who are the most sensitive to changes in life stereotypes. The authors propose a set of measures to efficiently organize leisure during self-isolation with physical therapy, cognitive training, as well as measures to fit the body and develop psychological relief, making it possible to reverse the threat of the pandemic in higher health potential and better family relations.

    Abstract Objective: This work is to develop and use a new approach to the analysis of differences between the velocity profiles of registered light signals in groups of patients with schizophrenia and healthy donors.
    Material and methods: The present study involved 62 patients (all female) with schizophrenia or schizoaffective disorders in the acute period (observational study - 2016-2017). 44 patients were diagnosed with paranoid schizophrenia with an attack-progredient type of course (F20.01 according to ICD-10) or with a continuous type of course (F20.02). The fibrinodynamics test (FD) was performed on the T-2 thrombodynamics device (Hemacore LLC, Moscow), which makes it possible to monitor the processes of coagulation and fibrinolysis in the cuvette channels filled with fresh blood plasma. The result of the test are the brightness profiles of the clot.
    Results: In the proposed approach, each profile is characterized by a vector of distances between it and other analyzed profiles. At that the distances between the profiles are calculated as the sum of the modules of differences at different points of the observation interval. This method makes it possible to overcome a certain loss of information that occurs when using a standard method based on the calculation of several parameters that characterize the analyzed velocity profiles. The method includes the construction of all kinds of statistically significant piecewise linear regression models that link the distances to two profiles. The velocity profiles corresponding to the points lying on opposite sides of the break point of such models have significantly different shapes. In practice, it turns out that for many piecewise linear regression models, the break points also significantly separate the velocity profiles for the compared groups of donors and patients with schizophrenia.     Conclusion: The developed method makes it possible to visually establish the relationship between the shape of the velocity profile and the presence of the disease.
