    Abstract The exact pathogenesis of inflammatory bowel disease (IBD) is still not completely understood. It is hypothesized that a genetic predisposition leads to an exaggerated immune response to an environmental trigger, leading to uncontrolled inflammation. As there is no known causative treatment, current management strategies for inflammatory bowel disease focus on correcting the excessive immune response to environmental (including microbial) triggers. In recent years, there has been growing interest in new avenues of treatment, including targeting the microbial environment itself. Fecal microbiota transplantation (FMT) is a novel treatment modality showing promising results in early studies. The article discusses the rationale for the use of FMT in inflammatory bowel disease and the yet-unresolved questions surrounding its optimal use in practice.

    Abstract The aim of the study was to check the integument and alimentary tract of Theromyzon for the occurrence of fungi. Moreover, the mycological purity of water sources from which leeches were collected was analyzed. The potential role of the duck leech Theromyzon maculosum in transmitting pathogenic fungi has not been studied until now. Mycological analyses were performed according to the procedure approved in mycological diagnostic laboratories, using classical diagnostic methods (macro- and microcultures, microscopic observations). In total, 22 species of yeasts were detected. The specimens belong to eight genera: Candida, Citeromyces, Debaryomyces, Kluyveromyces, Lodderomyces, Pichia, Saccharomyces and Saccharomycodes. Overall, 18 species of fungi were isolated from leeches Theromyzon maculosum, including seven species from the integument and 11 species from the gastrointestinal system. Among fungi isolated from body integument, fungi of the genus Candida were dominant, whereas in the gastrointestinal system, the genus Saccharomyces dominated. From the river waters, nine fungi species were isolated, and yeasts of the genera Candida and Kluyveromyces dominated. Isolation of the same species of microfungi from the integument of leeches and the river water suggests that Theromyzon maculosum may be a potential vector of pathogenic yeasts for vertebrates living in the same ecosystems and for humans.

    Abstract Glioblastoma multiforme (GBM) is the most common malignant primary brain tumor in adults. Despite several investigations in this field, maximal safe resection followed by chemoradiotherapy and adjuvant temozolomide with or without tumor-treating fields remains the standard of care with poor survival outcomes. Many endeavors have failed to make a dramatic change in the outcomes of GBM patients. This study aimed to review the available strategies for newly diagnosed GBM in the neoadjuvant setting, which have been mainly neglected in contrast to other solid tumors.

    Abstract Purpose: The aim of this study was to evaluate objective results of short-term treatment with the use of electromyography and to determine changes in muscle recruitment during the prone hip extension test in individuals with chronic low back pain who underwent the McKenzie treatment.
    Methods: The studied group consisted of 17 male office workers aged 33-55 who underwent two experimental sessions comprising of 3 measurements and the McKenzie method of mechanical diagnosis and therapy. Electromyographic examination of selected muscles was performed during the test in order to assess changes in their activity before and after the McKenzie method and the Visual Analogue Scale was used to assess the level of pain.
    Results: There was a tendency towards significance at the beginning of activation between the first and second test for the left erector spinae muscle ( p = 0.0684). In 18% of patients, the onset activation time decreased, which is indicative of correct direction of changes. No statistically significant changes were observed in the remaining muscles. Statistical significance ( p = 0.0131) was observed between the first and third test for the left erector spinae muscle. A decrease in the onset activation time was observed in 75% of the subjects. Effectiveness increased by as much as 57%. The level of pain decreased in the third test compared to the first one ( p = 0.0240). However, the change in pain sensations between the first and second test was not statistically significant.
    Conclusions: The study showed objective changes in the activation sequence of selected muscles and subjective changes in pain following short term McKenzie therapy.

    Abstract This study aimed to assess refractive astigmatism, in phaco-canaloplasty (PC) vs phaco-non-penetrating deep sclerectomy (PDS) in a randomized, prospective study within 24 months. Patients were randomized pre-operatively, 37 underwent PC and 38 PDS. The following data was collected: BCVA, IOP, number of antiglaucoma medications, refraction with autokeratorefractometry. The assessment of astigmatism was simple arithmetic and vector analysis (calculations included cylinder with axis in form of centroids) and included double angle plots and cumulative refractive astigmatism graphs. Pre-operative mean BCVA in PC was 0.40 ± 0.43 and was comparable to BCVA in PDS 0.30 ± 0.32logMAR (P = 0.314). In the sixth month follow-up, mean BCVA showed no difference (P = 0.708) and was 0.07 ± 0.13 and 0.05 ± 0.11, respectively. However, 2 years after the intervention mean BCVA was better in PC 0.05 ± 0.12 than in PDS 0.12 ± 0.23 and it was statistically significant (P = 0.039). Mean astigmatism in PC at baseline was 1.13 ± 0.73Dcyl, at 6 months it was 1.09 ± 0.61 and at 2 years 1.17 ± 0.51. In PDS at baseline 1.35 ± 0.91 at 6 months 1.24 ± 0.86 and at 2 years 1.24 ± 0.82. There were no differences between the groups in mean astigmatism throughout the study. Centroids (mean of a cylinder with axis) in PC were pre-operatively 0.79D@172˚ ± 1.10Dcyl, at 6 months 0.75D@166˚ ± 1.01 and at 24-months 0.64D@164˚ ± 1.11 and in PDS pre-operatively 0.28D@10˚ ± 1.63D at 6 months 0.26D@11˚ ± 1.5 and at 24-months 0.47D@20˚ ± 1.43. The direction of mean astigmatism was against the rule in all analyzed time points. The mean baseline IOP in PC was 19.4 ± 5.8 mmHg and 19.7 ± 5.4 mmHg in PDS(P = 0.639). From the 6-month IOP was lower in PC, at 24-months it was 13.8 ± 3.3 mmHg in PC and 15.1 ± 2.9 mmHg in PDS(P = 0.048). In both groups preoperatively patients used median(Me) of three antiglaucoma medications(P = 0.197), at 24-months in PC mean 0.5 ± 0.9 Me = 0.0 and 1.1 ± 1.2 Me = 1.0 in PDS(P = 0.058). Both surgeries in mid-term observation are safe and effective. They do not generate vision-threatening astigmatism and do not even change the preoperative direction of mean astigmatism. Refractive astigmatism is stable throughout the observation.

    Abstract Purpose: Chronic low back pain is a common disorder affecting about 80% of the population, caused by a disorder in the muscular system. The main aim of this study was to assess muscle fatigue during the Biering-Sorensen Test in people with chronic low back pain who underwent the McKenzie treatment.
    Methods: Tests were conducted on 19 men (mean age of 41.4 years) with chronic non-specific lumbar-sacral pain syndrome, working in the seated position in front of a computer. Assessment of changes in fatigue of erector spinae, gluteus maximus and biceps femoris using surface electromyography during the Biering-Sorensen Test and subjective pain assessment using Visual Analog Scale were conducted on three test dates. Time-frequency representation of the electromyographic signal (Fourier transform) was used for the examination of muscle fatigue. The McKenzie method of diagnosis and therapy was applied before and between the tests.
    Results: The McKenzie therapy resulted in increased endurance (test duration) of the examined spinal muscles between the 1st and 3rd test date ( p = 0.043), and a systematic decrease in pain assessment on the three test dates ( p = 0.000-0.004). Correlations were obtained between slope coefficients of the simple regression of median frequency of electromyographic signals on the one hand and duration of the BST ( p = 0.000-0.012) and anthropometric parameters (body mass, height and body mass index, p = 0.001-0.020) on the other.
    Conclusions: The McKenzie method is an effective tool in reducing the level of lumbar pain and improving muscle endurance.
