    Abstract Perforation of a gastrointestinal tract as a complication of intubation is unusual, and only few cases have been reported. Prompt recognition and management of gastrointestinal tract perforation are needed to limit the morbidity and mortality of this condition. We presented a case of an acutely ill patient who developed gastric perforation following difficult intubation to remind clinicians of a life-threatening complication that can develop following a life-saving procedure.

    Abstract The diagnosis of pulmonary embolism is challenging particularly when patients present with vague and/or non-specific symptoms and signs. Misdiagnosis of pulmonary embolism can lead to death or severe morbidity. We reported a case of a 60-year-old woman presented with recurrent syncope who later was diagnosed as submassive pulmonary embolism. This case report highlights the importance of early diagnosis and management of pulmonary embolism to prevent life-threatening sequels. Pulmonary embolism should be considered as a differential diagnosis of patients presenting at an emergency department with syncope.

    Abstract Pneumocystis jirovecii pneumonia (PJP) is one of the most common causes of respiratory failure in patients with human immunodeficiency virus (HIV) infection, for which the mortality rate is approximately 10%. Spontaneous pneumothorax as a presentation of PJP has been reported with rising frequency, but tension pneumothorax as a presentation of PJP is rare. We reported a case of a middle-aged male with HIV infection who presented to our hospital with acute worsening shortness of breath and was later diagnosed with tension pneumothorax due to PJP. Physicians should suspect pneumothorax in patients with a clinical presentation of PJP who show rapid progressive respiratory deterioration.

    Abstract Pericardial effusion is a common cardiac condition that can be lethal if left untreated. Patients who have pericardial effusion often present with dyspnea, chest discomfort, chest tightness, and cough. Cardiac arrhythmia as an initial presentation of pericardial effusion is not common. While the most common cardiac arrhythmia associated with pericardial effusion is atrial fibrillation, atrial flutter as a presenting symptom of malignant pericardial effusion has been described rarely. Herein, we present the case of an elderly man who had developed atrial flutter due to malignant cardiac effusion, which subsequently led to the diagnosis of lung cancer. Clinicians should broaden the differential diagnosis of patients who present with atrial flutter. Also, point-of-care ultrasound (POCUS) may help determine the etiology of a new-onset atrial flutter.

    Abstract Hyperemesis gravidarum (HG) is the most common cause of in-patient hospitalizations during the first half of pregnancy. The etiology of HG has not yet been elucidated, and the treatment is mainly symptomatic. Untreated severe HG can lead to catastrophic maternal complications such as cardiac arrhythmia and death. In contrast, the impact of untreated severe HG on the fetuses remains contradictory. Evidence suggested that HG may increase the risk of a small for gestational age (GA) fetus. We here report a case of 32-year-old nulliparous woman, GA of 14 weeks, who presented with worsening HG and later had a diagnosis of missed abortion. More research is needed to clarify the possibility of HG as a contributory cause of abortion.
