    Abstract An intratendinous ganglion cyst is a very rare benign lesion with an unknown etiology. The clinical diagnosis can be difficult as patients may have mild symptoms or impaired hand functionality. Ultrasound and magnetic resonance imaging can differentiate a ganglion cyst from other soft-tissue tumors and tumor-like lesions and provide excellent information on the location of an intratendinous lesion to schedule surgical treatment. We present a case report of a 50-year-old female diagnosed with an intratendinous ganglion cyst of the extensor indicis. She complained of right-hand swelling for three months, which was associated with pain. The US revealed an oval hypoechoic mass with cystic formation at the extensor indicis, measuring 9 x 4 mm, compatible with an intratendinous ganglion cyst. The cyst was excised by enucleation. After surgery, the patient was referred to the Department of Physical and Rehabilitation Medicine for evaluation. She started a rehabilitation programme. The patient presented a favourable clinical evolution with a return to her previous professional activity. However, six months after surgery, the cyst recurred, but with a smaller size and no associated pain.

    Abstract Horses are considered as reservoirs of multidrug resistant bacteria that can be spread through the environment and possibly to humans. The aim of this study was to characterize the oral Gram-negative microbiota of healthy horses and evaluate their antimicrobial susceptibility profile in a One Health approach. For this purpose, samples were collected from the gingival margin of healthy horses, free of antimicrobial therapy, cultured in selective mediums, identified, and tested for antimicrobial susceptibility. Fifty-five Gram-negative isolates were identified, with 89.5% being zoonotic and 62% affecting humans, which were also found commonly in the environment. Forty-eight isolates (96%) were MDR. The phenotypic resistance presented as higher to macrolides (81.8%), β-lactams (55.4%), and quinolones (50%), and lower to sulfonamides (27.3%), tetracyclines, and amphenicols (both with 30.9%). In total, 51.5% of the isolates presented resistance to carbapenems. In addition to being the first report on the commensal oral microbiota of horses and respective susceptibility profile, this study highlights the horse as a valuable sentinel that can control the evolution and transmission of multidrug-resistant bacteria between the "One Health triad" since it is in contact with humans, other animals, and the environment, in different geographic locations.

    Abstract Hyponatraemia is the most prevalent electrolyte disorder in the neurocritical care setting and is associated with a significant morbimortality. Cerebral salt wasting and inappropriate antidiuretic hormone secretion syndrome have been classically described as the two most frequent entities responsible for hyponatraemia in neurocritical care patients. An accurate aetiological diagnosis of hypotonic hyponatraemia requires a proper volume status assessment. Nevertheless, determination of volume status based on physical examination, laboratory findings and imaging modalities have several limitations and can lead to improperly diagnosis and hyponatraemia mismanagement. Point-of-care ultrasound (POCUS), specifically Venous Excess UltraSound (VExUS) score, is a fast and valuable tool to evaluate venous congestion at the bedside and identify hypervolaemia, helping the physicians in therapeutic decision making in a patient with hyponatraemia. We report a case where the use of POCUS, and more specifically VExUS, can be helpful in volume status assessment, complementing the complex management of multifactorial hyponatraemia in a neurocritical patient.

    Abstract An 18-year-old, otherwise healthy woman, presented to the emergency department with a three-day history of acute-onset diffuse rash. She denied recent introduction of new drugs or dermocosmetic products and previously adverse drug reactions or allergies. She hadn´t had any known contact with anyone with confirmed SARS-CoV-2 infection. On examination she had a temperature of 38.6°C. Physical examination showed a disseminated, asymptomatic and not evanescent annular erythematous urticarial papules and plaques, some with targetoid appearance, mostly located at the back (A), chest (B), inguinal zone (C), distal portions of upper (D) and lower limbs and forehead. The palmoplantar skin and mucous membranes were spared. Laboratory tests showed d-dimer elevation (0.43 μg/ml, normal range 0.0-0.25 μg/ml). Chest radiography and computed tomographic pulmonary angiography was normal. A reverse-transcriptase–polymerase-chain-reaction test for severe acute respiratory syndrome coronavirus 2, performed on a nasopharyngeal swab, was positive. A diagnosis of COVID-19 infection was made. On the sixth day of follow-up, fevers and rash were disappeared. The particular interest of this case is the inaugural appearance of a cutaneous manifestation, before any other symptom. It is essential to take into account and promote the recognition among clinicians of this possible skin manifestation of COVID-19, because an early diagnose, could contribute to break the chain of transmission immediately.
