    Abstract Patients with irreversible malignant and non-malignant diseases have comparable mortality rates, symptom burdens, and quality of life issues; however, non-cancer patients seldom receive palliative care (PC) or receive it late in their disease trajectory. To explore the characteristics of non-cancer patients receiving PC in northern Italy, as well as the features and outcomes of their care, we retrospectively analyzed the charts of all non-cancer patients initiating PC regimens during 2019 in three publicly funded PC departments in Italy’s populous Lombardy region. We recorded the baseline variables (including data collected with the NECPAL CCOMS-ICO-derived questionnaire used since 2018 to evaluate all admissions to the region’s PC network), as well as treatment features (setting and duration) and outcomes (including time and setting of death). Of the 2043 patients admitted in 2019, only 12% (243 patients—131 females; mean age 83.5 years) had non-oncological primary diagnoses (mainly dementia [n = 78], heart disease [n = 55], and lung disease [n = 30]). All 243 had Karnofsky performance statuses ≤ 40% (10−20% in 64%); most (82%) were malnourished, 92% had ≥2 comorbidities, and 61% reported 2−3 severe symptoms (pain, dyspnea, and fatigue). Fifteen withdrew or were discharged from the study PCN; the other 228 remained in the PCN and died in hospice (n = 133), at home (n = 9), or after family-requested transfer to an emergency department (n = 1). Most deaths (172/228, 75%) occurred <3 weeks after PC initiation. These findings indicate that the PCN network we studied cares for few patients with life-limiting non-malignant diseases. Those admitted have advanced-stage illness, heavy symptom burdens, low performance statuses, and poor survival. Additional efforts are needed to improve PCN accessibility for non-cancer patients.

    Abstract The wastewater reuse is an important measure to face water shortage, thus improving the resilience of agricultural production chains. However, treated wastewater can contain residual organic micropollutants residues that may result in crop contamination. Among edible crops, olive is the most important agricultural product in the Mediterranean region. Methods to assess the contamination of organic micropollutants such as polycyclic aromatic hydrocarbons (PAHs) and polychlorinated biphenyls (PCBs) in olives are poorly investigated. Given the complexity of olives, this study focused on the development and validation of a method for the simultaneous extraction of PAHs and PCBs from olives, and subsequent analysis by gas chromatography coupled with mass spectrometry detection. Extraction was optimized through a QuEChERS protocol, studying the effect of the extraction solvent (CH 2 Cl 2 , cyclohexane, CH 3 CN) and of the dispersive-solid phase extraction (d-SPE) sorbent (octadecyl silica, Florisil, primary secondary amine, Z-Sep) on the recovery of micropollutants. The best recoveries (94–122%, relative standard deviations below 5%) were obtained using CH 3 CN/H 2 O and a double purification step with Z-Sep and Florisil. The method developed for PAHs and PCBs, which showed good intra-day (<2.7%) and inter-day (<2.9%) precision and low matrix effect (|ME| < 14%), was applied to the analysis of olives grown by irrigation with reclaimed wastewaters.

    Abstract The chemical contamination in fruit and vegetables represents a challenging analytical issue, with tomatoes deserving to be investigated as they are fundamental components of the Mediterranean diet. Polychlorinated biphenyls (PCBs), polycyclic aromatic hydrocarbons (PAHs) and nitro-PAHs contamination is of serious concern, due to particulate deposition and to uptake from contaminated soils and water. However, time-consuming, non-simultaneous and/or non-eco-friendly extraction procedures are typically used to investigate organic contamination in tomatoes, with nitro-PAHs that have not yet been studied. Based on these premises, this work reports the development of a QuEChERS-based approach, coupled with gas chromatography/mass spectrometry, for the simultaneous determination of 16 PAHs, 14 PCBs and 4 nitro-PAHs in three tomato cultivars. The effect of dichloromethane, cyclohexane and acetone, as well as of four clean-up phases were studied through the advanced combination of full factorial experimental design and multiple response optimization approaches. The final protocol, based on cyclohexane extraction followed by a double purification step with primary secondary amine and octadecyl silica and a sulfuric acid oxidation, led to 60–120% recoveries (RSD% < 15%). Good repeatability (inter-day precision <15%) and negligible matrix effect (<16%) were confirmed and the protocol was applied to the analysis of real tomato samples purchased in a local market.

    Abstract Transcatheter aortic valve implantation (TAVI) is a less invasive alternative to surgical implantation and its implementation is progressively increasing worldwide. We routinely perform pre-procedural aortic angiography CT to assess aortic dimensions and vascular anatomy. This study aims to evaluate the image quality of CTA for TAVI planning using dual-layer spectral CT, with virtual monoenergetic image reconstructions at 40 keV. Thirty-one patients underwent a CTA protocol with the injection of 20 mL of contrast media. Image quality was assessed by measuring the mean density in Hounsfield Units (HU), the signal-to-noise ratio, and the contrast-to-noise ratio in VMI reconstructions. Additionally, a blinded subjective analysis was conducted by two observers. The results showed significant enhancement at all sampled vascular levels with a gradual decrease in HU from proximal to distal regions. Favourable subjective ratings were given for all parameters, with greater variability in the evaluation of iliac axes. A significant negative correlation (

    Abstract Over the past decade, biochar (BC) has received significant attention in many environmental applications, including water purification, since it is available as a low-cost by-product of the energetic valorisation of biomass. Biochar has many intrinsic characteristics, including its porous structure, which is similar to that of activated carbon (AC), which is the most widely used sorbent in water treatment. The physicochemical and performance characteristics of BCs are usually non-homogenously investigated, with several studies only evaluating limited parameters, depending on the individual perspective of the author. Within this review, we have taken an innovative approach to critically survey the methodologies that are generally used to characterize BCs and ACs to propose a comprehensive and ready-to-use database of protocols.     Abstract In the analysis of contaminants in food products, sample preparation is performed by proper adsorbents, whose choice is crucial to eliminate matrix interference. In this work we modified SBA-15 adsorbents by functionalization with (3-aminopropyl)-triethoxysilane (SBA-15-APTES) and N-[3-(trimethoxysilyl)propyl]aniline (SBA-15-AN) aiming to use them for the first time in the clean-up step of a QuEChERS (quick, easy, cheap, effective, rugged and safe) extraction of micropollutants from strawberry, a sugar rich fruit. After physico-chemical characterization by nitrogen adsorption, infrared spectroscopy and thermogravimetric analysis, the adsorption capabilities of SBA-15 sorbents and possible interaction mechanisms were studied at different pH (2.1-8.5) for glucose, sucrose and fructose at concentrations characteristic of those found in strawberries. The performance of the two SBA-15 sorbents was compared with that of commercial PSA (primary secondary amine), usually proposed in QuEChERS protocols. Both SBA-15 materials exhibit up to 30% higher adsorption than PSA, suggesting their possible QuEChERS application. Synthesized SBA-15 adsorbents were hence used as innovative dispersive sorbents in the QuEChERS extractions of 13 PAHs and 14 PCBs from strawberry. For PCBs, SBA-15-AN provides better matrix removal than PSA and comparable extraction recoveries around 90%. For PAHs, the use of SBA-15-AN has the advantage of lower relative standard deviation (7%) than PSA (19%).
