    Abstract Introduction Diabetes is one of the most common endocrine diseases worldwide. Type 2 diabetes and depression are commonly comorbid, high-prevalence, chronic disorders. Individuals with diabetes mellitus may have concurrent mental health disorders and are shown to have poorer disease outcomes. However, the evidence for clinical correlation is still unclear. Objectives To find the prevalence of depression among patients with type 2 diabetes at King Abdullah Medical City (KAMC). Materials and methods A cross-sectional observational study was conducted at KAMC in Makkah, the Kingdom of Saudi Arabia. Study participants were 267 people with type 2 diabetes (aged between 18-70 years). Those who had preexisting depression or drank alcohol, pregnant women, and postpartum women were excluded from the study. Participants' clinical and demographic data and depression assessment were obtained on phone through an interview and using the Arabic version of the Patient Health Questionnaire-9 (PHQ-9), respectively. Glycated hemoglobin (HbA1c) test results of participants were collected electronically. Results The participants' mean age was 57.88 ± 8.71 years, and 64.4% were males. Only 15.4% were current smokers and 16.9% were ex-smokers. The most common medical condition was hypertension (65.2%) and the majority (92.1%) had uncontrolled HbA1c with a mean value of 8.37 ± 1.92. Among the studied patients, 73% suffered from different degrees of depression; 36%, 19.9%, 8.6%, 5.2%, and 3.4% of the participants were suffering from minimal, mild, moderate, moderately severe, and severe depression, respectively. Only neuropathy was a significant risk factor of depression (odds ratio=2.87, 95% confidence interval=1.18-6.97, p=0.02). Conclusion Depressive symptoms are common in patients with uncontrolled type 2 diabetes who also suffer from neuropathy and retinopathy. Hypertension, cardiovascular diseases, and unhealthy diet had a significant correlation with depression.

    Abstract Background Coronavirus disease 2019 (COVID-19) patients admitted to the intensive care unit (ICU) are at a higher risk of developing delirium. In this study, we estimated the incidence of delirium and its risk factors in ICU patients with COVID-19 at King Abdullah Medical City (KAMC), Makkah, Saudi Arabia. Methodology We conducted a retrospective, analytical, cohort study of adult COVID-19 patients admitted to the ICU of KAMC between May 2020 and July 2021. Data were collected from electronic medical records. Results Of the 406 examined patients with COVID-19 aged >18 years, 55 developed delirium in the ICU setting. The incidence rate was 0.59% per 100 ICU days in these 55 patients; the mean age was 62.36 ± 17.9 years, and 65.5% were men. Binary logistic regression showed that age (p = 0.027), nationality (p = 0.045), presence of infectious diseases other than COVID-19 (p = 0.047), and ICU outcome (p = 0.013) were significant risk factors for developing delirium. The clinical presentation and prognosis of patients who developed delirium were assessed using the Acute Physiology and Chronic Health Evaluation II and Sequential Organ Failure Assessment scores, and the mean scores were 16.13 ± 7.96 and 5.25 ± 3.48, respectively. The mean length of ICU stay was 22.2 ± 33.3 days; 39 (70.9%) patients were discharged and 16 (29.1%) died. Conclusions Older age, nationality, infections, and ICU outcomes were risk factors for developing delirium in hospitalized COVID-19 patients at KAMC. Early detection of cognitive comorbidities and delirium in these patients is important.

    Abstract Introduction: The aim of this study was to present the clinical presentation and short-term outcomes of uncomplicated rhegmatogenous retinal detachment managed by pneumatic retinopexy at a tertiary eye hospital in western Saudi Arabia.
    Materials and methods: This one-armed retrospective cohort study evaluated selected cases of rhegmatogenous retinal detachment managed by pneumatic retinopexy between 2017 and 2018. Data were collected on patient demographics, preoperative ophthalmic assessment, surgical details, follow up at six months postoperatively, complications and the need for additional surgery. Anatomic success was defined as retinal attachment at 6 months and functional success was defined as vision >20/200 at six months postoperatively. The association of lens status and concomitant laser treatment to anatomical success rates were evaluated.
    Results: The study sample consisted of 15 eyes with rhegmatogenous retinal detachment in the upper quadrant. In 14 cases, C3F8 gas was used. Anatomic and functional success was noted in 73.3% [95% confidence interval (CI) 51.0: 95.7] and 86.7% (95% CI 69.2; 100) of eyes, respectively. The association between anatomical success and lens status was not significant [RR = 4.5 (95% CI 0.6

    37.5), P=0.1]. There was no significant association between anatomical success and concomitant laser treatment. [RR = RR = 1.7 (95% CI 0.7

    4.0), P=0.2]. A 2nd retinal surgery was not required in 33.3% of eyes (95% CI 9.5

    57.2).
    Conclusion: If stringent selection criteria are used for managing rhegmatogenous retinal detachment with pneumatic retinopexy, anatomical and functional success by six months postoperatively is achieved in the majority of cases. Additional surgeries could further improve outcomes.

    Abstract Background: Coronary artery bypass grafting is known to be associated with better outcome in ischemic heart disease patients with low ejection fraction. We aim to demonstrate the effect of coronary artery bypass grafting (CABG) on left ventricle (LV) systolic function and to identify the predictors that adversely lead to postoperative poor outcome.
    Result: This is a cross-sectional prospective study; we included 110 patients with left ventricular ejection fraction (LVEF) < 50% who underwent CABG with a mean age of 56.1 ± 12.2 years old. Those patients were classified into two groups: group I, 76 (69%) patients with LVEF > 35%, and group II, 34 (31%) patients with LVEF < 35%. Our results as regards demographic and clinical data revealed that group II patients had a significantly higher prevalence of diabetes mellitus (DM) and Euro SCORE II compared to group I patients (p = 0.05 and < 0.001 respectively); otherwise, all other clinical predictors did not differ between the two studied groups. There was a significant improvement in LVEF post-surgery (p = 0.05) in both groups with observed no significant difference recorded for in-hospital mortality rate among patients with different groups. DM, significant diastolic dysfunction, and insertion of IABP are predictors of in-hospital mortality of the patients (p = 0.001, 0.03 and < 0.001, respectively) CONCLUSION: We concluded that there is a significant improvement of LV systolic function after CABG and hence better survival rate. DM, significant diastolic dysfunction, and perioperative insertion of IABP are predictors of mortality after cardiac surgery. Special care should be provided to such patients to improve their outcome.

    Abstract Background. Coronary artery disease (CAD) is considered as the leading cause of the cardiovascular fatalities worldwide. CAD is diagnosed by many modalities of imaging such as myocardial perfusion imaging (MPI) and coronary angiography (CAG). Methods. A retrospective cross-sectional study was conducted that included all patients referred to the KAMC (King Abdullah Medical City) nuclear cardiology lab from its opening until the end of May 2014 (a period of 17 months). A total of 228 patient reports with a history of conducting either CAG or MPI or both were used in this study and statistically analyzed. Results. An analysis of the MPI results revealed that 78.5% of the samples were abnormal. On the other hand, 26.75% of the samples revealed that they were subjected to CAG and MPI. There was a significant and fair agreement between MPI and CAG by using all the agreement coefficients (kappa = 0.237, phi = 0.310, and P value = 0.043). The sensitivity, specificity, and accuracy of MPI with reference to CAG were 97.8%, 20%, and 78.69%, respectively. In addition, positive predictive and negative predictive values were 78.95% and 75%, respectively. Conclusion. In a tertiary referral center, there was a significant agreement between MPI and CAG and a high accuracy of MPI. MPI was a noninvasive diagnostic test that could be used as a gatekeeper for CAG.

    Abstract Background: The region of Al-Baha area of interest is associated with risk factors; yet no research in the literature has explored oral cancer (OC) and its associated social determinants and risk factors in this region. The aim of this study was to access the prevalence of OC in the Al-Baha region and its association with the social determinants and risk factors in this population.
    Methods: A retrospective cohort hospital-based study was conducted in Al-Baha Region between January 2012 and December 2019. Data collected from all the patients diagnosed with OC were extracted. OC was assessed based on the hospital records, histopathological data, and Tumor, Nodes, Metastasis (TNM) staging aided in cancer assessment. Detailed demographic data associated with OC was collected using semistructured telephonic interviews. Risk assessment was reviewed based on population, exposure of interest, and outcome.
    Results: A total of 19°C patients out of 421 cancer patients were diagnosed. The mean age for OC in males was 59.72 ± 15.83 years and 69.14 ± 5.86 years in females. Oral squamous cell carcinoma of the tongue was the most with one case of metastasis. The average estimate per day smoking, E- cigarettes, water pipe smoking was 14.55 ± 7.15, 11.15 ± 3.55, and 0.52 ± 4.7, respectively, with males' habits more than females. These contributes to the risk factors associated with socioeconomic factors.
    Conclusions: Al Baha region experiences different socioeconomic determinants with a lower prevalence of OC. The risk of OC is associated with various adverse oral habits, which can be prevented.

    Abstract The study of rotating-disk heat-flow problems is relevant to computer storage devices, rotating machineries, heat-storage devices, MHD rotators, lubrication, and food-processing devices. Therefore, this study investigated the effects of a Hall current and motile microorganisms on nanofluid flow generated by the spinning of a disk under multiple slip and thermal radiation conditions. The Buongiorno model of a nonhomogeneous nanofluid under Brownian diffusion and thermophoresis was applied. Using the Taylor series, the effect of Resseland radiation was linearized and included in the energy equation. By implementing the appropriate transformations, the governing partial differential equations (PDEs) were simplified into a two-point ordinary boundary value problem. The classical Runge-Kutta dependent shooting method was used to find the numerical solutions, which were validated using the data available in the literature. The velocity, motile microorganism distribution, temperature, and concentration of nanoparticles were plotted and comprehensively analyzed. Moreover, the density number, Sherwood number, shear stresses, and Nusselt number were calculated. The radial and tangential velocity declined with varying values of magnetic numbers, while the concentration of nanoparticles, motile microorganism distribution, and temperature increased. There was a significant reduction in heat transfer, velocities, and motile microorganism distribution under the multiple slip conditions. The Hall current magnified the velocities and reduced the heat transfer. Thermal radiation improved the Nusselt number, while the thermal slip conditions reduced the Nusselt number.

    Abstract In an attempt to find a new class of antimicrobial agents, a series 2-pyridinone and 2-iminochromene derivatives containing Lidocaine analogue were designed and synthesized. The 2-pyridinone derivatives (3), (4), and (6) were obtained through the cyclocondensation of 2-cyano-N-(2,6-dimethylphenyl)-acetamide (2) with 1,3-dicarbonyl compounds and/or ternary condensation of (2), aromatic aldehyde, and malononitrile. Also, a series of 2-iminochromene derivatives (7–9) were synthesized through the condensation reaction of cyanoacetamide derivative (2) with salicylaldehyde derivatives. The structure of the new compounds were confirmed based on elemental analysis and spectral data. These compounds were screened for their antibacterial and antifungal activity The minimal inhibitory concentration (MIC) (µg/mL) of the most active (4), (5b), and (8) derivatives were determined. The MIC values between 7.81 and 31.26 µg/mL against bacterial species for (8) derivative, and upon comparison to tetracycline exhibited a positive control MIC (31.26–62.6 µg/mL). Besides, the activity against C. albicans (ATCC 1023) showed a MIC value of 15.63 µg/mL, which is similar to that of Amphotericin B.

    Abstract The study synthesized Pluronic F-127 nanoparticles that encapsulate Fe2O3 (PF127Fe2O3NPs), nanoparticles, characterized their formation, and evaluated their cytotoxicity and anticancer activity using Berberis vulgaris leaf extract, using various analytical methods such as FTIR, Ultraviolet-visible, photoluminescence, dynamic light scattering, X-ray diffraction, and morphology analysis. We assessed the antioxidant properties of PF127Fe2O3NPs, cytotoxicity, and apoptosis through 3-[4,5-dimethylthiazol-2-yl]-2,5 diphenyl tetrazolium bromide (MTT) assay and acridine orange/ethidium bromide staining in breast cancer cells, such as MCF7, and MDA-MB-231. The characterization results demonstrated that PF-127 was coated with Fe2O3 nanoparticles. MTT assay data revealed that PF127Fe2O3NPs effectively prevent cancer cells from proliferating and act as an anticancer drug. The antimicrobial results revealed that the fabricated nanoparticles are effective against gram-negative (Klebsiella pneumoniae, Escherichia coli, and Shigella dysenteriae) and gram-positive (Streptococcus pneumoniae, Staphylococcus aureus, and Bacillus subtilis) bacteria. Treatment of PF127Fe2O3NPs in a dose-dependent manner on MCF7, and MDA-MB-231, exhibited increased antioxidant activity, nuclear damage, and apoptotic activity. These results confirm the apoptotic activity of PF127Fe2O3NPs. The study concludes that MCF7 appears to be more sensitive to PF127Fe2O3NPs than MDA-MB-231. In conclusion, we have found that it can be used as an effective antioxidant and anticancer agent in therapeutics.

    Abstract Background: The aim of this study was to evaluate the accuracy of the degree of fracture reduction after open reduction and internal fixation of Zygomaticomaxillary Complex (ZMC) fractures in the Saudi population of the Al-Baha region, using Gillies approach. Further comparison with preoperative and postoperative standardized computed tomography (CT) views with the calculation of residual deformity percentage, which remained after the ZMC rehabilitation.
    Methods: A 5-year retrospective CT-based study on preoperative and postoperative axial CT scans of 46 male patients with ZMC fractures. The CT measurements were made (in millimeters) at the fracture site of maximum displacement through the anterior orbital rim and orbital floor, posterolateral wall of the maxillary sinus, zygomatic arch, and zygomaticofrontal suture. For the zygomatic arch, measurements were made (in mm) by drawing a tangent to the fractured arch segments and dropping a perpendicular to the inward displaced fractured arch. The total difference in all measured parts between preoperative and postoperative displacement was calculated in percentages.
    Results: Upon comparison of preoperative and postoperative zygomaticomaxillary complex CTs, three-point fixation at the regions of infraorbital rim, frontozygomatic suture, and posterolateral wall of the maxilla results in a reduction of the fracture sites in the range of 72.85% to 85%. Maximum reduction was noted at the zygomatic arch, that is, 85%, and minimum at the infraorbital rim, that is, 72.85%. The reduction obtained at all four sites was statistically significant, with
    Conclusion: Gillies temporal approach and three-point fixation at the regions of the infraorbital rim, frontozygomatic suture, and posterolateral wall of the maxilla results in satisfactory treatment of ZMC fractures and improves patients oral health and quality of life.
