    Abstract Background: The chorda tympani is frequently harmed during middle ear surgery. Surgery-related chorda tympani injury is rarely associated with impaired taste perception in patients. The use of microscopes marked the beginning of modern tympanoplasty techniques, and the endoscopic tympanoplasty approach is now described in terms of minimally invasive surgery.Aim of the Study: The aim of the current study is to compare chorda tympani nerve [CTN] injuries occurring because of tympanoplasty performed using microscopic and endoscopic methods.Patients and Methods: This prospective study was carried out on 50 patients from Al-Azhar University hospitals who suffered from chronic suppurative otitis media [safe type]. Patients were randomized equally using opaque sealed envelopes into two groups. Group A: 25 patients underwent transcanal endoscopic tympanoplasty with elevation of the tympamomeatal flap. Group B: 25 patients underwent microscopic tympanoplasty by postauricular incision.Results: Chorda tympani nerve was with no injury in 24 [96%] cases in group A and 14 [56%] cases in group B, while 1 [4%] case in group A and 8 [32%] cases in group B had stretched chorda tympani nerve and 3 [12%] cases in group B had transected chorda tympani nerve. The Chorda tympani nerve was significantly less injury, less stretched in group A than group B, and more transected in group B than group A [P value=0.012].Conclusion: In patients undergoing tympanoplasty, Endoscopic ear surgery provides superior visualization, reduces the necessity for extensive manipulation of the chorda tympani, and subsequently lowers the occurrence of CTN injury compared to microscopic ear surgery.

    Abstract The absence of teeth, known as edentulism, poses considerable obstacles in prosthodontic care and greatly affects a person's well-being. Conventional complete dentures frequently lead to problems like instability and insufficient retention, especially in the lower jaw. Fortunately, the introduction of dental implants has transformed the way we approach edentulous patients, as they now offer support and enhanced retention for removable prostheses, thus revolutionizing their treatment. While a consensus exists on using two implants for retaining mandibular overdentures, the associated cost may be prohibitive for economically disadvantaged individuals. As a solution, the concept of single implant-retained mandibular overdentures has emerged, catering to individuals with limited financial resources and complete tooth loss. This review explores the efficacy and suitability of the single implant overdenture approach, along with an overview of treatment options for edentulous patients, including traditional dentures, tooth-supported overdentures, and implant-supported overdentures. The preservation of bone, improvements in functional abilities, and psychological benefits associated with overdentures are discussed. Moreover, various classifications and prosthetic options for implant overdentures, specifically for mandibular cases, are presented. This literature review aims to provide a comprehensive understanding of possible treatment options and focus on the single implant-retained mandibular overdenture approach and its implications in prosthodontic rehabilitation for edentulous patients.

    Abstract Image reconstruction algorithm and its controller constitute the main modules of the electrical capacitance tomography (ECT) system; in order to achieve the trade-off between the attainable performance and the flexibility of the image reconstruction and control design of the ECT system, hardware-software codesign of a digital processing unit (DPU) targeting FPGA system-on-chip (SoC) is presented. Design and implementation of software and hardware components of the ECT-DPU and their integration and verification based on the model-based design (MBD) paradigm are proposed. The inner-product of large vectors constitutes the core of the majority of these ECT image reconstruction algorithms. Full parallel implementation of large vector multiplication on FPGA consumes a huge number of resources and incurs long combinational path delay. The proposed MBD of the ECT-DPU tackles this problem by crafting a parametric segmented parallel inner-product architecture so as to work as the shared hardware core unit for the parallel matrix multiplication in the image reconstruction and control of the ECT system. This allowed the parameterized core unit to be configured at system-level to tackle large matrices with the segment length working as a design degree of freedom. It allows the trade-off between performance and resource usage and determines the level of computation parallelism. Using MBD with the proposed segmented architecture, the system design can be flexibly tailored to the designer specifications to fulfill the required performance while meeting the resources constraint. In the linear-back projection image reconstruction algorithm, the segmentation scheme has exhibited high resource saving of 43% and 71% for a small degradation in a frame rate of 3% and 14%, respectively.

    Abstract Background and objective: Isocitrate dehydrogenase genes (IDH1 and IDH2) encode important enzymes that play pivotal role in cellular metabolism. Mutations in TET2 have been demonstrated to contribute to DNA hypermethylation, either expression of mutant IDH1/2 or TET2 resulted in poor cell differentiation and epigenetic alterations in hematopoietic cells, suggesting a sharing of the oncogenetic impact. In this study, we investigated the frequency of genetic alterations in IDH1/2 and TET2 genes in Egyptian cohort of adult patients with de novo AML, and the association of IDH1/2 and TET2 genetic Polymorphism with AML prognostic criteria and explore prognostic molecular markers with clinical outcome.
    Methods: The SNP assay for IDH1, IDH2 and TET2 genes polymorphism tested with RT-PCR included three polymorphisms that are rs121913500, rs121913503, and rs2454206 respectively, were tested on 141 adult Egyptian patients fulfilling the AML diagnostic criteria.
    Result: The incidence of IDH mutations is 11/141 (7.8%); 5/141 (3.5%) IDH1 mutant and 6/141 (4.3%) IDH2 mutant. And the incidence of TET2 mutations is 72/141 (51.1%); 15/141 (10.7%) homozygous mutation and 57/141 (40.4%) heterozygous mutations. IDH1, IDH2 and TET2 genes mutations with DFS and OS in AML patients were not significantly correlated.
    Conclusions: TET2 SNP is common in Egyptian AML patients. Further research on IDH, TET2 and their relationships to other hematological malignancies and leukemogenesis transformation is advised and a study of a larger number of cases is needed for potential statistical significance.

    Abstract This paper presents a printed multiple-input multiple-output (MIMO) antenna with the advantages of compact size, good MIMO diversity performance and simple geometry for fifth-generation (5G) millimeter-wave (mm-Wave) applications. The antenna offers a novel Ultra-Wide Band (UWB) operation from 25 to 50 GHz, using a Defective Ground Structure (DGS) technology. Firstly, its compact size makes it suitable for integrating different telecommunication devices for various applications, with a prototype fabricated having a total size of 33 mm × 33 mm × 0.233 mm. Second, the mutual coupling between the individual elements severely impacts the diversity properties of the MIMO antenna system. An effective technique of orthogonally positioning the antenna elements to each other increased their isolation; thus, the MIMO system provides the best diversity performance. The performance of the proposed MIMO antenna was investigated in terms of S-parameters and MIMO diversity parameters to ensure its suitability for future 5G mm-Wave applications. Finally, the proposed work was verified by measurements and exhibited a good match between simulated and measured results. It achieves UWB, high isolation, low mutual coupling, and good MIMO diversity performance, making it a good candidate and seamlessly housed in 5G mm-Wave applications.

    Abstract Introduction: The durability of hemodialysis vascular access remains a troublesome issue for the patients as well as vascular surgeons that requires frequent reinterventions to maintain the access function.
    Aim: This study aimed to evaluate straight thigh polytetrafluethylene (PTFE) arteriovenous graft (AVG) in patients with exhausted upper extremities dialysis access.
    Method: Our study was a retrospective analysis of prospectively collected data of 30 patients were operated upon for straight pattern lateral thigh PTFE AVG between 2016 and 2018. The primary outcome was efficacy and patency of AVG, and secondary outcome was procedure safety, including infection, thrombosis, ligation, lower limb functional status, and mortality.

    Conclusion: Lateral straight thigh PTFE AVG is a reliable and durable alternative modality with adequate dialysis efficacy in patients with exhausted upper extremities dialysis accesses. Secondary procedures due to complications were frequent but no significant major bleeding as regard patients with ruptured graft were non-significant and no distal threatening ischemia or related deaths were observed.

    Abstract Background: Gastric cancer has long been assumed to be more advanced and has a worse prognosis in younger people than in older ones. This assumption, however, has never been validated, most likely attributable to the lack of patient numbers for a stage-by-stage survival comparison. Objectives: To present NCI, Cairo University experience in managing young patients (≤ 40 years) with gastric adenocarcinoma Patients and methods: Retrospective cohort study included all cases diagnosed in NCI with gastric adenocarcinoma at or below the age of 40 years in the period from 2010 to 2021. The data included demographic attributes, clinical presentation, investigations and results, adopted treatment modalities, and prognosis of these cases. Results: During the period of the study,30 cases of gastric adenocarcinoma with ages of around 40 years or less have been diagnosed and managed in NCI. Most cases presented in an advanced stage as stage 4 represented 46.7% (14 cases). Seventeen cases (56.7%) underwent surgical exploration, where chemotherapy was taken for twenty-six cases (86.7%). The overall survival showed that patients presented with advanced stage, had omental deposits or ascites had a worse prognosis (P-value = 0.001, <0.001ز Conclusion: Gastric Cancer is aggressive in young age and presents mainly in advanced stage. In terms of overall survival, patients benefit significantly from combined treatment modalities (chemotherapy and surgical resection) compared to receiving only one treatment option.

    Abstract Abstract Background Hepatic steatosis has become a major worldwide health problem, so assessment of hepatic steatosis in potential living donors is crucial prior to liver transplantation. Until now liver biopsy (LB) is considered the gold standard for diagnosing steatosis before transplantation, however steatosis assessment using imaging modalities, such as computerized tomography (CT), would be better for the donor, due its non-invasiveness. This study aimed to assess the efficacy of CT as a semiquantitaive tool for liver steatosis assessment in liver donors in comparison to liver biopsy results. Methods This cross-sectional study was carried out on 53 potential liver graft donors. All patients were subjected to non-contrast CT of the abdomen, tru-cut liver biopsy, and histopathological evaluation. The CT liver attenuation (CTL), the hepatic/splenic CT attenuation ratio (CTL/S) and difference between hepatic attenuation value and splenic attenuation values (CTL–S) were determined as well as the correlations of these indices and the findings of Liver biopsy (LB) were compared. Results According to the hepatosteatosis grades in the pathology results, the patients were divided into two groups: group A: 38 patients with grade 0 hepatosteatosis and group B: 15 patients with grade 1 and 2 hepatosteatosis. CTL, CTL–S, CTL/S, ratio of mean right hepatic lobe and splenic attenuation (RT/S) and ratio of mean left hepatic lobe and splenic attenuation (LT/S) were found to be effective in the diagnosis of hepatosteatosis grades at cutoff values ≤ 55.4, ≤ 8.7, ≤ 1.17, ≤ 1.1548 and ≤ 1.2971 with 80%, 80%, 73.3%, 86.7% and 86.7% sensitivity and 71.1%, 56.8%, 73.7% 71.1% and 50.0% specificity respectively. Also, CTL/S was found to be very effective in the diagnosis of grade II hepatosteatosis at cutoff values ≤ 0.9 with 100% sensitivity and 100% specificity.     Abstract Congenital symmastia is a medial confluence of the breasts. It is a rare anomaly with few reports in the literature and no standard treatment. In this article, we present a case of congenital symmastia treated by 3 steps: liposuction, fixation of the skin to the chest wall in the area of the intermammary sulcus, and postoperative intermammary compression. A successful result was achieved with normal cleavage between the breasts. So, this is considered the ideal treatment for this condition.
