    Abstract Os Diagnósticos Sócioeconômicos e Ambientais fazem parte de um conjunto de projetos ambientais que visam contribuir para a utilização racional e sustentável dos recursos naturais, bem como avaliar as relações entre o homem e a ambiência dentro das unidades naturais, ou seja, as bacias hidrográficas. A utilização deste diagnóstico específico permite analisar as deteriorações sociais, econômicas, tecnológicas, sócio-econômicas e ambientais, com o intuito de fornecer subsídios para programas de Educação Ambiental Técnica. Visa também a disseminação de conhecimentos e conseqüentemente a busca de alternativas para a melhoria da qualidade e do nível de vida da população. O referido diagnóstico foi realizado na área rural da Sub-Bacia Hidrográfica do Arroio Inhamandá, no município de São Pedro do Sul (RS). Os resultados obtidos apresentam um elevado índice de deterioração nas cinco linhas abordadas pelo diagnóstico, demonstrando a real necessidade de programas que visem a ações para a melhor utilização dos recursos naturais, além da melhoria da qualidade de vida da comunidade rural.

    Abstract A 9-year-old neutered male cat, previously test-positive for feline immunodeficiency virus (FIV), was presented with an history of vomiting, hyporexia, and weight loss. Panleukopenia was identified on complete blood counts, and bone marrow evaluation revealed ineffective granulocytic hyperplasia and rare neutro-, erythro-, and rubriphagocytosis. Prednisolone was initiated with no response, and progression to pancytopenia occurred. On abdominal ultrasonographic examination, splenomegaly was present. PCR testing was positive for Candidatus Mycoplasma haemominutum and IgG antibodies against Toxoplasma gondii were detected (titer 1:2560). Treatment with antibiotics, feline recombinant interferon-ω, chlorambucil, mycophenolate, and raltegravir was implemented with no clinical improvement, and splenectomy was performed. Cytologic evaluation of splenic aspirates revealed exuberant neutro-, erythro-, and rubriphagocytosis. Histopathology of the spleen also showed many erythrophagocytic macrophages with no evidence of malignancy, and a diagnosis of hemophagocytic syndrome (HS) was made. The WBC count and hematocrit reached reference values 1 day and 3 months, respectively, after splenectomy. The cat was treated with cyclosporine and lomustine. Disease progression led to the development of septic hepatitis, and the cat was euthanized. To our knowledge, this is the first case of presumptive HS in cats that might have been associated with FIV, Toxoplasma gondii, and Candidatus Mycoplasma haemominutum co-infection.

    Abstract Theobromine is a caffeine metabolite most abundant in dark chocolate, of which consumption is linked with a lower risk of cognitive decline. However, the mechanisms through which theobromine affects neuronal function remain ill-defined. Using electrophysiological recordings in mouse hippocampal synapses, we now characterized the impact of a realistic concentration of theobromine on synaptic transmission and plasticity.     Abstract Objective: To investigate whether a specific nystagmus observed during the Epley maneuver can enable immediate reassessment of patients, eliminating the need for deferred diagnosis.
    Study design: Prospective cohort.
    Setting: Tertiary center.
    Patients: Patients with posterior semicircular canal (PSC) benign paroxysmal positional vertigo (BPPV) diagnosed by the Dix-Hallpike test between March 2022 and March 2023.
    Interventions: All patients were treated with the Epley maneuver. The presence and the characteristics of the nystagmus were recorded in each of the four intermediate positions of the maneuver. Fifteen minutes later, all patients were reevaluated with the Dix-Hallpike test to determine the therapeutic success.
    Main outcome measures: Outcome of the Epley maneuver and its relationship with nystagmus characteristics during the four positions of the maneuver.
    Results: Sixty-six patients (77.3% female) were included in the study, of which 66.7% exhibited right PSC BPPV. Therapeutic success of the first Epley maneuver was 74.2%. Almost one third (30.3%) of patients presented with nystagmus in the second and third positions of the Epley maneuver. All patients with down-beating and torsional nystagmus toward the opposite direction of the diagnostic nystagmus in the fourth position of the Epley maneuver needed a second rehabilitative maneuver. None of the patients without nystagmus in this fourth position remained with active BPPV after the first Epley maneuver.
    Conclusions: The presence of nystagmus in the fourth position of the Epley maneuver appears to have a high predictive value, reducing waiting time for a second diagnostic maneuver. Its absence may exempt subsequent reevaluation.
