    Abstract A 7-year-old boy with a history of recurrent acute lymphoblastic leukemia (ALL), in remission, presented to primary care clinic after 2 days of progressive right hip pain with weight-bearing activities. He was otherwise asymptomatic at the time of presentation. Blood cultures revealed Gram-negative diplococci, which prompted an MRI that was significant for a hip joint effusion and femoral head bone marrow edema. The patient had no sick contacts and no significant past medical history other than ALL. The patient had been given all recommended childhood vaccinations. Arthrocentesis and needle biopsy of the femoral neck were not diagnostic for malignancy and revealed only mild hip joint inflammation, leading to a diagnosis of osteomyelitis. The organism in the original blood culture was identified as Haemophilus influenzae type b, β-lactamase negative. Review of the patient's medical records showed a history of complete immunization to Haemophilus influenzae type b. An immunologic evaluation was made to determine if the patient retained immunity from his other vaccinations. Pathogen-specific antibody testing revealed detectable antibodies to polio but not measles, mumps, rubella, varicella-zoster virus, tetanus, diphtheria, pertussis, or hepatitis B. This loss of immunologic memory appears to be a rarely described side effect of ALL chemotherapy. There is currently no protocol to evaluate the immunologic memory of patients who underwent chemotherapy for ALL or to revaccinate them after their treatment. It is unclear whether the loss of immunologic memory is genuinely rare or is underdiagnosed because affected patients are protected by herd immunity.

A CC&C Platform that can receive data from multiple hospital systems, analyze the data, arbitrate any resulting actions and determine the relative priorities to distribute work to the right person or teams-can lead to improved operational efficiencies and better patient outcomes.

    Abstract Aortic valve atresia with interruption of the aortic arch is an extremely rare anomaly; only eleven cases of this anomaly have been reported to date. In the absence of additional sources of blood flow to the ascending aorta, aortic valve atresia with interruption of the aortic arch is fatal. We present, to the best of our knowledge, the first case of a live birth with aortic valve atresia and interrupted left aortic arch (type B) without evidence of an aorticopulmonary communication or ductal supply to the native ascending aorta. Instead, blood flow to the native aortic root was derived from a persistent right embryonic dorsal aorta.

    Abstract The idiotype (Id)-granulocyte-macrophage colony-stimulating factor (GM-CSF) fusion proteins are potential vaccines for immunotherapy of B-cell lymphoma. In this study, four vaccine candidates were constructed by fusing murine GM-CSF to the amino- or carboxy-terminus of the 38C13 murine B-lymphocyte Id scFv with two different arrangements of the variable regions of the heavy chain and light chain (VL-VH and VH-VL). scFv (VH-VL) and GM-CSF/scFv fusion proteins were expressed in an Escherichia coli cell-free protein synthesis system. In order to promote disulfide bond formation during cell-free expression, cell extract was pretreated with iodoacetamide (IAM), and a sulfhydryl redox buffer composed of oxidized and reduced glutathione was added. The E. coli periplasmic disulfide isomerase, DsbC, was also added to rearrange incorrectly formed disulfide linkages. The 38C13 B-lymphocyte Id scFv was expressed with 30% of its soluble yield in active form (43 microg/ml) when tested with an anti-idiotypic mAb, S1C5, as the capture antibody in radioimmunoassay. It was found that the amino-terminal GM-CSF fusion proteins, GM-VL-VH and GM-VH-VL, showed much higher activity than the carboxy-terminal GM-CSF fusion proteins, VL-VH-GM and VH-VL-GM, in stimulating the cell proliferation of a GM-CSF-dependent cell line, NFS-60. Between the two amino-terminal GM-CSF fusion proteins, GM-VL-VH showed a higher total and soluble yield than GM-VH-VL.

    Abstract Bupropion is a structurally and biochemically unique antidepressant that inhibits the neuronal uptake of dopamine and norepinephrine. Often prescribed for children and adolescents, bupropion displays both neurologic and cardiac toxicities in overdose more serious than toxicities resulting from poisonings by tricyclic antidepressants and selective serotonin reuptake inhibitors. Bupropion was briefly removed from the market in the 1980s. The incidence of bupropion poisonings in the United States, and resultant morbidity and mortality in children and adolescents, has been steadily increasing since 2012. Antidepressants less toxic than bupropion in overdose should be considered in the vulnerable 6- to 19-year-old patient population.

    Abstract Lower income couples tend to report more difficulty sustaining high-quality intimate relationships. As a result, policy initiatives have been enacted to fund relationship education (RE) programs that aim to increase lower income couples' relationship satisfaction. Generally, these programs demonstrate small, albeit statistically significant improvements in mean levels of relationship functioning. It is critical, however, to understand if RE programming influences the developmental course of intimate relationships, and if this influence depends on couples' initial levels of concerns about their relationships. Using dyadic group-based modeling and three waves of data from 6034 couples in the Supporting Healthy Marriages project, a randomized control trial of RE, we categorized couples into four relationship concern groups (No Relational Concerns, Both Relationally Concerned, Men's Relational Concerns, and Women's Relational Concerns) and explored how these groups moderated the long-term efficacy of RE programming. Results indicated that RE was associated with different developmental trajectories of satisfaction, but RE effects differed for men and women. Specifically, random assignment into RE was associated with men maintaining high levels of relationship satisfaction, whereas women's satisfaction decreased over time. These effects were not moderated by initial relationship concerns. The association between RE and relationship satisfaction trajectories was small in magnitude, suggesting that more comprehensive services are needed to strengthen lower income couples' intimate relationships.

    Abstract The identification of fraudulent and questionable research conduct is not something new. However, in the last 12 years the aim has been to identify specific problems and concrete solutions applicable to each area of knowledge. For example, previous work has focused on questionable and responsible research conducts associated with clinical assessment, measurement practices in psychology and related sciences; or applicable to specific areas of study, such as suicidology. One area of study that merits further study of questionable and responsible research behaviors is psychometrics. Focusing on psychometric research is important and necessary, as without adequate evidence of construct validity the overall validity of the research is at least debatable. Our interest here is to (a) identifying questionable research conduct specifically linked to psychometric studies; and (b) promoting greater awareness and widespread application of responsible research conduct in psychometrics research. We believe that the identification and recognition of these conducts is important and will help us to improve our daily work as psychometricians.

    Abstract Background: Brachioradial pruritus (BRP) is a rare form of chronic dysesthesia characterized by persistent itching, burning, or tingling commonly involving the dorsolateral upper extremities. Although the exact pathophysiology remains unclear, the condition may be a manifestation of atypical cervical radiculopathy.
    Observations: The authors describe two patients with BRP, a 44-year-old female and a 51-year-old male, both of whom benefited from surgical intervention for atypical cervical radiculopathy, while also highlighting their presentation, diagnostic testing, treatment, and outcomes.
    Lessons: The cases demonstrate the potential relationship between cervical spondylosis and BRP while examining the role of surgical intervention as a treatment option.

    Abstract Recent research has identified significant differences in how satisfaction in newlywed relationships progress, with the majority of spouses reporting relatively high marital satisfaction. However, most studies have focused on white, middle class, childless couples, and it is not clear whether these findings hold for socioeconomically disadvantaged couples (those with low educational attainment or income). Further, previous work has largely ignored differences between spouses within the same marriage and the circumstances under which interpersonal processes between spouses are influential to prospective marital satisfaction. Using dyadic growth-mixture modeling and three waves of data from 530 newlywed couples in the Supporting Healthy Marriages study, we simultaneously modeled husbands' and wives' marital satisfaction and identified two classes of couples, labeled the Relatively satisfied class and Husbands' moderate intercept, wives' low, and increasing satisfaction class. Husbands consistently reported better marital satisfaction than their wives regardless of class membership. Wives' perceptions of interpersonal processes during experiences of financial stress predicted class membership. Dissolution rates also differed between classes. We conclude by providing practical implications for working with socioeconomically disadvantaged families.
