    Abstract Recentemente, o tema das desigualdades de renda ganhou grande visibilidade, porém, na maioria das vezes, os fatores explicativos estão no desempenho econômico e nas políticas sociais. Neste trabalho, incorporamos a dinâmica das classes ao debate e oferecemos uma interpretação complementar. As classes ocupacionais operam mecanismos de fechamento social e distribuição de poder que condicionam a distribuição de renda. Tomamos a Região Metropolitana de São Paulo, entre 1981 e 2011, como estudo de caso. Usamos técnicas de decomposição da desigualdade e construímos cenários contrafactuais. As análises mostram que uma polarização “skillbiased” teve lugar durante os anos 1990, mas é revertida durante a década de 2000. Encontramos também polarização associada a outros fatores, que não a qualificação. As dinâmicas das classes ocupacionais foram justamente os componentes que mais tiveram importância na redução das desigualdades, seguidas de perto pelo processo de formalização da mão de obra. O principal vetor de queda das desigualdades foi a homogeneização interna às classes, sugerindo que o regime de classes tenha se tornado mais coeso. Dessa forma, resta principalmente a desigualdade entre ocupações – fazendo com que esse componente assuma maior importância explicativa sobre o estoque de desigualdade atual.

    Abstract Background: Leishmania skin test (LST) evaluates the delayed type hypersensitivity to Leishmania antigens (LA) and has been used for diagnosis of cutaneous leishmaniasis (CL). In CL patients LST is usually positive but a small percentage have negative LST. The aim of this study was to determine the clinical and immunologic features and response to antimony therapy in LST-negative CL patients.
    Methods: We compare the clinical presentation, response to therapy, and immune response of CL patients with negative vs positive LST.
    Results: The clinical presentation was similar in both groups but LST-negative patients had a lower cure rate. In the lesions, LST-negative patients displayed less inflammation and necrosis, and higher frequency of CD8+ T cells. Mononuclear cells from LST-negative patients had a poor T helper 1 cell (Th1) response but levels of interleukin-1β (IL-1β), IL-6, IL-17, granzyme B, and metalloproteinase-9 (MMP-9) were similar to the LST-positive group upon stimulation with LA. Leishmania internalization and killing by macrophages were similar in both groups. Cure of disease was associated with restoration of Th1 response.
    Conclusions: In LST-negative patients, impaired Th1 response is associated with therapeutic failure. Increased frequency of CD8+ T cells and high production of inflammatory cytokines, granzyme B, and MMP-9 contributes to immunopathology.

    Abstract A size exclusion high performance liquid chromatography (SEC-HPLC) method has been developed which is capable of separation and quantitation of bovine serum albumin (BSA) and bovine serum globulin (BSG) components of serum-based lubricant (SBL) solutions. This allowed characterization of the stability profiles of these proteins when acting as lubricants during hip wear simulation, and identification of wear-specific mechanisms of degradation. Using cobalt-chromium metal-on-metal (MOM) hip joints, it was observed that BSA remained stable for up to 3 days (215K cycles) of wear testing after which the protein degraded in a fairly linear fashion. BSG on the other hand, began to degrade immediately and in a linear fashion with a rate constant of 5% per day. Loss of both proteins occurred via the formation of high molecular weight aggregates which precipitated out of solution. No fragmentation of the polypeptide backbone of either protein was observed. Data obtained suggest that protein degradation was not due to microbial contamination, denaturation at the air-water interface, or frictional heating of articulating joint surfaces in these studies. We conclude that the primary source of protein degradation during MOM simulation testing occurs via high shear rates experienced by SBL solutions at articulating surfaces, possibly coupled with metal-protein interactions occurring as new and reactive metal surfaces are generated during wear testing. The development of this analytical methodology will allow new studies to clarify the role of SBL solutions in wear simulation studies and the interactions and lubricating properties of serum proteins with prosthetic surfaces other than MOM.
