    Abstract Background: Ureteral injuries are not very common and can occur after many surgical procedures. Kidney salvage is desirable. Renal autotransplantation is a final option for some cases. In this case, we report an autotransplantation of the kidney after an iatrogenic injury of the ureter with totally extraperitoneal approach.
    Case report: A 41 years old female underwent left endoscopic ureterolithotomy with holmium laser for ureteral calculi. An iatrogenic ureteral injury, probably ureteral avulsion, occurred. After multiple interventions, she referred to us with a nephrostomy tube. Imaging was performed and left renal autotransplantation was chosen as surgical management. The approach was totally extraperitoneal. No alteration of renal function or of urine outflow was observed during the follow up.
    Conclusions: The report supports the safety and efficacy of renal autotransplantation.

    Abstract Background and purpose: The treatment of caliceal diverticulum and deeply secluded lower-pole stones is challenging, and the indications are controversial. This study assessed the changes in the quality of life (QoL) of patients undergoing percutaneous nephrolithotomy (PCNL) for symptomatic caliceal diverticula and deep lower-pole calculi to determine whether there is a change in the "bother factor" in order to improve treatment strategies for this group of patients.
    Patients and methods: Twenty-two patients undergoing PCNL were evaluated prospectively using the SF36 QoL questionnaire before and 6 weeks after PCNL. The results before and after PCNL were compared individually and by group and analyzed statistically. A P value <or=0.05 was considered significant.<br />Results: Most (87%) of the patients were completely stone free after the procedure. Significant QoL findings were a reduction in symptoms interfering with performance at work (40%) and improved general health (33%). Although there was an overall improvement in QoL across the entire group, this reached statistical significance for only two of the seven domains of the questionnaire.
    Conclusions: In spite of a high stone-free rate, fewer than half of patients benefited subjectively from the procedure in terms of improvement of QoL. Therefore, patients should be made aware of the limited impact of surgical treatment on QoL, and asymptomatic patients can be given the option of further watchful waiting.

    Abstract Purpose: Ureteral stenting plays a vital role in urology but still entails complications such as migration, encrustation, and blockage. We present our experience with a novel ureteral stent (Zebrastent) that had the complication of breakage which we explored by fatigue testing.
    Materials and methods: We developed a device that can reproduce the movements of the Zebrastent in vitro (bending and cranio-caudal movements). We estimated the number of movements of the stent during 1 day and we thus calculated how much stress would result if it were in place for 9 months.
    Results: We estimate that the stent will not break when in place up to 9 months in a kidney with average curvature. However, the stent broke when it was used in a kidney with a sharper-than-normal curvature.
    Conclusions: The Zebrastent may be a valuable addition to the armamentarium of the endourologist, provided that the appropriate size is selected.

    Abstract Objective: It was the aim of this study to assess the efficacy and safety of combined forced hydration and diuresis with limited inversion during shock wave lithotripsy (SWL) by comparing this treatment modality with conventional SWL for lower calyceal nephrolithiasis.
    Patients and methods: In this prospective, non-randomized study, we included 100 patients with lower calyceal calculi <or=2 cm. Fifty of them received conventional SWL and the other 50 underwent SWL combined with oral hydration, diuresis and 12 degrees inversion position during SWL. Intravenous urography was performed for all patients prior to their treatment. Patients in both groups were treated on Dornier MPL 9000. Blood pressure monitoring was applied during the SWL session. Follow-up was performed the first and the third month after treatment with plain kidney-ureter-bladder X-ray and kidney-ureter-bladder ultrasound.<br />Results: Clinical outcomes were available in 90 patients. Follow-up at 3 months showed that 83.3% of the patients belonging to the study group were rendered stone free, whereas 71.5% were stone free in the control (p > 0.05). Complications were minimal and not statistically significant.
    Conclusions: Forced diuresis and inversion therapy is very well tolerated; however, the stone-free rate was not significantly improved.
