    Author's details Wesley E. Sowers, MD is Clinical Professor of Psychiatry at the University of Pittsburgh Medical Center and is the Director of the Center for Public Service Psychiatry and its associated Fellowship Program at Western Psychiatric Hospital. He is board certified in Adult Psychiatry with subspecialty certifications in Addiction, Administrative and Community Psychiatry. He is a Past President of the American Association for Community Psychiatry and has served on the Board of Directors of that organization since 1988. He has also been on the Board of the American Association of Psychiatric Administrators since 1999. He has served as Co-Chair of the Mental Health Services Committee of the Group for Advancement of Psychiatry since 2009. From 2008 to 2016 he was co-director of the SAMHSA sponsored Recovery to Practice curriculum development project for psychiatry.     Abstract We describe the first use of an articulating Total Control Introducer (TCI) and video laryngoscope (VL) to guide intubation of a patient with known difficult airway. The patient was an 84-year-old woman with a Mallampati IV airway assessment, small mouth opening, limited neck extension, and micrognathia. We utilized a VL to visualize the glottis and an articulating TCI for endotracheal tube placement. Anteflexion and retroflexion of the introducer tip allowed dynamic navigation through the upper airway and into the trachea. Intubation was performed on first attempt without complications.

    Abstract There is evidence that burn injury stimulates ubiquitin-proteasome-dependent protein breakdown in skeletal muscle. In this proteolytic pathway, protein substrates are conjugated to multiple molecules of ubiquitin, whereafter they are recognized, unfolded and degraded by the multicatalytic 26 S protease complex. The 20 S proteasome is the catalytic core of the 26 S protease complex. The influence of burn injury on the expression and activity of the 20 S proteasome has not been reported. We tested the hypothesis that burn injury increases 20 S proteasome activity and the expression of mRNA for the 20 S proteasome subunits RC3 and RC7. Proteolytic activity of isolated 20 S proteasomes, assessed as activity against fluorogenic peptide substrates, was increased in extensor digitorum longus muscles from burned rats. Northern-blot analysis revealed that the expression of mRNA for RC3 and RC7 was increased by 100% and 80% respectively following burn injury. Increased activity and expression of the 20 S proteasome in muscles from burned rats support the concept that burn-induced muscle cachexia is at least, in part, regulated by the ubiquitin-proteasome proteolytic pathway.

    Abstract We present a systematic methodology, built within the Open Knowledgebase of Interatomic Models (OpenKIM) framework (https://openkim.org), for quantifying properties of grain boundaries (GBs) for arbitrary interatomic potentials (IPs), GB character, and lattice structure and species. The framework currently generates results for symmetric tilt GBs in cubic materials, but can be readily extended to other types of boundaries. In this paper, GB energy data are presented that were generated automatically for Al, Ni, Cu, Fe, and Mo with 225 IPs; the system is installed on openkim.org and will continue to generate results for all new IPs uploaded to OpenKIM. The results from the atomistic calculations are compared to the lattice matching model, which is a semi-analytic geometric model for approximating GB energy. It is determined that the energy predicted by all IPs (that are stable for the given boundary type) correlate closely with the energy from the model, up to a multiplicative factor. It thus is concluded that the qualitative form of the GB energy versus tilt angle is dominated more by geometry than the choice of IP, but that the IP can strongly affect the energy level. The spread in GB energy predictions across the ensemble of IPs in OpenKIM provides a measure of uncertainty for GB energy predictions by classical IPs.

    Abstract Objectives: To revise a sex and gender appraisal tool for systematic reviews (SGAT-SR) and apply it to Cochrane sepsis reviews.
    Study design and setting: The revision process was informed by existing literature on sex, gender, intersectionality, and feedback from an expert advisory board. We revised the items to consider additional factors associated with health inequities and appraised sex and gender considerations using the SGAT-SR-2 and female Participation-to-Prevalence Ratio (PPR) in Cochrane sepsis reviews.
    Results: SGAT-SR-2 consists of 19 questions appraising the review's sections and use of the terms sex and gender. amongst 71 SRs assessed, 50.7% included at least one tool item, the most frequent being the number of participants by sex or gender at included study-level (24/71 reviews). Only four reviews provided disaggregated data for the full set of included trials, while two considered other equity-related factors. Reviews rarely appraised possible similarities and differences across sex and gender. In half of a subset of reviews, female participants were under-represented relative to their share of the sepsis population (PPR<0.8).
    Conclusion: The SGAT-SR-2 tool and the PPR can support the design and appraisal of systematic reviews to assess sex and gender considerations, address to whom evidence applies, and determine future research needs.
