    Abstract Extensive flooding caused by Hurricane María in Puerto Rico (PR) created favorable conditions for indoor growth of filamentous fungi. These conditions represent a public health concern as contamination by environmental fungi is associated with a higher prevalence of inflammatory respiratory conditions. This work compares culturable fungal spore communities present in homes that sustained water damage after Hurricane María to those present in dry, non-flooded homes. We collected air samples from 50 houses in a neighborhood in San Juan, PR, 12 and 22 months after Hurricane María. Self-reported data was used to classify the homes as flooded, water-damage or dry non-flooded. Fungi abundances, composition and diversity were analyzed by culturing on two media.     Abstract Extensive flooding caused by Hurricane María in Puerto Rico (PR) created favorable conditions for indoor growth of filamentous fungi. These conditions represent a public health concern as contamination by environmental fungi is associated with a higher prevalence of inflammatory respiratory conditions. This work compares culturable fungal spore communities present in homes that sustained water damage after Hurricane María to those present in dry, non-flooded homes. We collected air samples from 50 houses in a neighborhood in San Juan, PR, 12 and 22 months after Hurricane María. Self-reported data was used to classify the homes as flooded, water-damage or dry non-flooded. Fungi abundances, composition and diversity were analyzed by culturing on two media. Our results showed no significant differences in indoor fungal concentrations (CFU/m3) one year after the Hurricane in both culture media studied (MEA and G25N). During the second sampling period fungal levels were 2.7 times higher in previously flooded homes (Median = 758) when compared to dry homes (Median = 283), (p-value < 0.005). Fungal profiles showed enrichment of Aspergillus species inside flooded homes compared to outdoor samples during the first sampling period (FDR-adjusted p-value = 0.05). In contrast, 22 months after the storm, indoor fungal composition consisted primarily of non-sporulated fungi, most likely basidiospores, which are characteristic of the outdoor air in PR. Together, this data highlights that homes that suffered water damage not only have higher indoor proliferation of filamentous fungi, but their indoor fungal populations change over time following the Hurricane. Ultimately, after nearly two years, indoor and outdoor fungal communities converged in this sample of naturally ventilated homes.

    Abstract Objective: This study aimed to determine if there was a reduction in the amount of non-diagnostic cytopathology results of thyroid fine-needle aspiration (FNA) biopsies performed at San Juan City Hospital (SJCH) endocrinology clinics since the inclusion of a cytopathologist at the clinics.
    Methods: This research consisted of a retrospective analysis of thyroid nodule FNA biopsy results performed at SJCH endocrinology clinics. The biopsies analyzed were performed during academic years from July 2017-June 2018 and July 2018-June 2019, a period that reflects one academic year prior and a year after the inclusion of a cytopathologist to the clinics. The patients were classified into "pre group" and "post -group." Descriptive analysis was conducted, taking into consideration variables including sex, age, period, location, size of the nodule, and cytology results. A Chi-square test and Confidence Interval were used to assess the association and estimates between predictors and outcomes.
    Results: From the 145 thyroid nodules biopsied, a total of 121 nodules (83.4%) resulted in diagnostic cytologic results, while 24 nodules (16.6%) were non-diagnostic. From the "pre group," 57 nodules (78.1%) had a diagnosis, while the other 16 (21.9%) were reported as non-diagnostic. From the "post group", 64 nodules (88.9%) had a diagnosis, while the other 8 (11.1%) resulted in non-diagnostic findings (p-value: = 0.08). Even though results were statistically non-significant, a clear trend towards a decrease in non-diagnostic samples was evident.
    Conclusion: In our study, there was a decrease in the number of non-diagnostic thyroid nodule FNA results after on-site adequacy determination guided by a cytopathologist.

    Abstract Seed exchange networks among farmers favor circulation of crop varieties and have been discussed as an effective means of crop diversity conservation. This study aims to document the processes and structure of seed exchange networks of native maize, beans, and squash among farmers and other participating sectors (local market or seed banks), analyzing their influence on agrobiodiversity conservation in the municipalities of Ixtenco and Huamantla, in the Mexican state of Tlaxcala. Through interviews, questionnaires, and social network analysis, nodal farmers were identified. In the maize network, five nodal farmers were detected, the blanco maize being the most commonly exchanged seed. In the bean network, three nodal farmers were identified, with amarillo beans as the most exchanged seed. In the squash network, no nodal farmer was identified. For maize and beans, the greater the number of exchanges, the greater the varieties exchanged. The local market of Huamantla and the Vicente Guerrero seed bank are relevant seed sources. The nodal farmers propitiate circulation of a large number of seed varieties in the exchange networks and contribute to maintenance and conservation of agrobiodiversity.

    Title translation Post mortem examination in tuberculosis: A 10 year observational study in the San Juan de Dios General Hospital, Guatemala.
    Abstract Introduction: Tuberculosis is a very important health problem worldwide. Early detection and adequate treatment of this disease is also a problem in low income countries. Post mortem examination has enabled tuberculosis to be diagnosed, even in cases without a clinical diagnosis of the disease.
    Objective: To determine the number of tuberculosis cases diagnosed on autopsy during a period of 10 years, at the San Juan de Dios General Hospital, Guatemala (SJDGH).
    Material and methods: The final autopsy reports at the between April 2006 and March 2016 were reviewed. H&E and special stains in cases with a diagnosis of tuberculosis were revised for confirmation.
    Results: During the 10 years reviewed, 859 autopsies had been carried out, 21 of which had the diagnosis of tuberculosis; 18 were adults and 3 were children and the majority (80.95%) were cases of pulmonary tuberculosis. However, in only 5 cases had there been a clinical suspicion, but without further investigation and none had a confirmed clinical diagnosis of tuberculosis. No patients had documented HIV/AIDS.
    Conclusion: Autopsy continues to be important in the study of disease. In this retrospective study, 21 cases of tuberculosis in 21 cases were found in patients with no clinical diagnosis of the disease.

    Abstract Introducción: el bajo peso al nacer, ha sido definido por la Organización Mundial de la Salud como el peso al nacer menor de 2 500 gramos, independiente de la edad gestacional y cualquiera que sea la causa, y es el índice predictivo más importante de la mortalidad infantil, sobre todo la neonatal. Objetivo: caracterizar los factores de riesgo de bajo peso al nacer en el municipio San Juan y Martínez durante el período octubre 2015 a abril 2016. Métodos: se realizó un estudio observacional, descriptiva y transversal. El universo estuvo constituido por las 242 gestantes que aportaron los nacimientos del período estudiado. La muestra que integrada por las 23 gestantes que aportaron el mismo número de recién nacidos bajo peso, que cumplieron con los criterios de inclusión establecidos en la investigación. Resultados: según grupo de edad, un 39,1 % de mujeres se encuentran entre 23 a 27 años, el 60,8 % que aportaron bajo peso eran fumadoras, seguidas de las que ingieren café con un 56,5 %, de ellas el 43,5 % tuvieron una edad gestacional de pretérmino, las afecciones asociadas a la gestación resultaron ser en un 34,8 % la anemia, seguida de un 30,4 % con infección vaginal y se evidenció que presentaban una valoración nutricional de desnutridas un 39,1 %, manteniendo una ganancia de peso regular durante la gestación. Conclusiones: el mejor conocimiento del comportamiento de los factores de riesgos asociados al bajo peso al nacer en las gestantes, contribuye a mejorar la atención que reciben.

    Abstract Objective: We performed a descriptive study of patients who have failed to DDAs in our Veteran population. The primary outcome of this study is to describe the clinical profile of these patients and to evaluate their respective resistance mutation panel.
    Methods: This investigation is a descriptive retrospective study of patients with chronic hepatitis C between the ages of 21 to 89 years from the Veteran Affairs Caribbean Healthcare System in P.R. Eligible cases were Veterans treated for hepatitis C with second generation of DAAs from January 1, 2015 to December 31, 2016 who failed to therapy. Patient records were reviewed and those who met inclusion criteria were included.
    Results: Among Hispanic Veterans treated with DAA for genotype 1 HCV infection, 3.9% had failure to treatment with the second generation DAAs. 90% were genotype 1a; while 10% were 1b. 80% of these were identified as cirrhotic and the other 20% were non cirrhotic. 90% had resistant variants for Ns5a. Eight patients had Ns3 RASs testing requested of which 50% had presence of resistant variants. Five patients had Ns5b RASs testing performed of which 40% had positivity for resistant variants to Ns5b.
    Conclusion: Despite DAA effectiveness, phase III clinical trials with new IFN-free DAA-based therapies have a 5-7% treatment failure rates. Real-life data has showed that <15% of patients fail to achieve SVR in the most difficult to cure groups such as those with cirrhosis or subtype 1a. These findings are comparable with our current study.

    Abstract We determine the prevalence and trends of open neural tube defects (ONTDs) during 1991 to 2019 at the "Dr. Juan I. Menchaca" Civil Hospital of Guadalajara (Mexico). Also, details of potential risks were obtained in 662 newborns, including those 143 patients with anencephaly and open spina bifida (OSB) classified as isolated (cases) and 519 controls. Data were analyzed using multivariable logistic regression. Among 267 201 live births during the study period, 336 were born with ONTDs, yielding an overall prevalence of 12.6 per 10 000. After folic acid (FA)-related programs began in Mexico (2003-2019), only OSB showed a decline of 20.6%.     Abstract Global breast cancer incidence varies considerably, particularly in comparisons of low- and high-income countries; rates may vary even within regions. Breast cancer rates for Caribbean countries are generally lower than for North America and Europe. Rates in Puerto Rico are in the middle of the range between the highest and the lowest Caribbean countries. Populations in transition, with greater variability in risk factor exposures, provide an important opportunity to better understand breast cancer etiology and as potential sources of variation in rates. Understanding of exposures across the life span can potentially contribute to understanding regional differences in rates. We describe here the design and implementation of a population-based, case-control study in the San Juan Metropolitan Area (SJMA) of Puerto Rico, the Atabey Epidemiology of Breast Cancer Study. We describe steps taken to ensure that the study was culturally appropriate, leveraging the Atabey researchers' understanding of the culture, local health system, and other required resources to effectively recruit participants. A standardized, in-person interview was developed, with attention to life course events customized to the study population. In order to understand variation in global breast cancer rates, studies customized to the populations outside of North America and Europe are required.
