    Abstract This article re-examines from a new perspective the efforts of James Smith (1771-1841), a Maryland doctor, to eradicate smallpox in the United States. As one of the few successful cowpox inoculators at the turn of the nineteenth century, Smith recognized the necessity for a public vaccine institution that could ensure the safe production and continuous preservation and circulation of vaccine matter. Thus, he devoted himself to creating statewide and national vaccine institutions funded by the state and federal governments. He established the National Vaccine Institution (NVI), but despite his efforts, the NVI existed only a short time from 1813 to 1822. Previous studies on Smith have focused on the 1813 Vaccination Act (An Act to Encourage Vaccination) and the NVI, and have evaluated them as failed projects or historically missed opportunities. However, this kind of approach does not justly place the act and institutions within Smith's larger plan and do not fully discuss the role of the NVI in his system of promoting vaccination in the United States. This article analyzes how he responded to the problems hindering cowpox vaccination, including spurious vaccine, failed vaccination, and low public acceptance of cowpox vaccine. In doing so, this study shows that Smith attempted to establish a universal and systematic vaccination system connecting citizens, government, and medical personnel through the NVI, as well as ensuring a safe and regular supply of vaccine.

    Abstract Aims: This paper focuses on the benefits of inclusive leadership when undertaking a priority setting partnership in community nursing, through providing a collaborative and committed nurse-led forum for initiating impactful changes, identifying evidence uncertainties and driving research capacity-building initiatives.
    Design: This is a Discussion paper. The project was undertaken between 2020 and 2021.
    Data sources: This paper is based on shared reflections as 70@70 Senior Nurse Research Leaders and is supported by literature and theory. It draws on issues relating to collective leadership, stakeholder engagement, diversity, inclusivity and COVID-19.
    Implications for nursing: The James Lind Alliance Priority Setting Partnership catalysed the development of a rigorous evidence-base in community nursing. The collaborative opportunities, networks and connections developed with patients, carers, nursing leaders, policy makers and healthcare colleagues raised the profile of community nursing research. This will benefit nursing research, practice, education and patients in receipt of community nursing care. Collective buy in from national leaders in policy, education, funding and commissioning has secured a commitment that the evidence uncertainties will be funded.
    Conclusion: Four key learnings emerged: collective leadership can ensure learning is embedded and sustained; developing an engaged stakeholder community to promote community nursing research is essential; a diverse membership ensures inclusivity and representation; and insights into the impact of COVID-19 aid progress. The process increased research engagement and created capacity and capability-building initiatives. This will help community nurses feel empowered to lead changes to practice. Sustained engagement and commitment are required to integrate research priorities into community nursing research, education and practice and to drive forward changes to commissioning and service delivery.
    Impact: The study promoted research capacity building through inclusive leadership. This can increase community nurses' research engagement and career development and patient care quality and safety; this can incentivize funders and policy makers to prioritize community nursing research.

    Abstract "Promising careers? A critical analysis of a randomised control trial in community health worker recruitment in Zambia" (Wintrup, 2021) raises important questions about the uses of randomized controlled trials (RCTs) and uses our RCT embedded in Zambia's National Community Health Assistant (CHA) Program (Ashraf et al., 2020a) as a case study to illustrate the pitfalls of the RCT methodology and especially its potential to do harm.     Abstract The frequency and magnitude of storm events is expected to increase with climate warming. Tidal freshwaters may be particularly sensitive to these events owing to their proximity to riverine inputs. Continuous and synoptic water quality data from riverine and tidal freshwater segments of the James, Mattaponi, and Pamunkey Rivers (Virginia, USA) were analyzed to characterize their response to high discharge events. Water quality attributes subject to dilution effects (conductivity, pH) showed negative responses to increasing discharge, which were similar in magnitude among riverine and estuarine segments. Variables associated with sediment transport (turbidity, TSS, E. coli, TN, and TP) showed positive responses to discharge, stronger responses in riverine vs. estuarine segments, and relatively linear responses over the full range of discharge. Variables affected by internal biological processes (CHLa, DO) showed negative responses to increasing discharge, greater sensitivity in the low range of discharge, and stronger responses in estuarine segments. High discharge was associated with reduced chlorophyll-a (CHLa), lower dissolved oxygen, and more heterotrophic conditions (lower production relative to respiration). During unusually high discharge in 2018, annual GPP was reduced to 38% (Pamunkey) and 78% (James) of long-term average values. Inter-estuarine differences in their sensitivity to high discharge reflect differences in forms of primary production, as the phytoplankton-dominated James exhibited rapid recovery in CHLa and GPP between storm events.
