    Abstract Background and aims: The autonomic nervous system (ANS) is cardinal for systemic homeostasis. Autonomic dysfunction is prevalent in as high as 65% of patients presenting for cardiac surgery in the Indian scenario. Pre-existing cardiac autonomic dysfunction (CAD) in surgical patients can accentuate perioperative haemodynamic fluctuations during stressful intraoperative events, predispose to adverse cardiac events, and contribute to morbidity and mortality. The prevalence and predictors of CAD in the elective neurosurgical population are unknown in the Indian scenario. The current study was conducted to bridge this knowledge gap.
    Methods: In this single-centre prospective observational study conducted at a tertiary care neurosciences centre, among 400 consenting adult patients of either gender, between 18 and 80 years of age, undergoing elective neurosurgery, the preoperative ANS function at the bedside was assessed as the primary outcome measure. The ANS status was evaluated using ANSiscope™-derived indices of heart rate variability. The diagnosis of CAD was made when the ANS index exceeded a threshold of 13.5.     Results: The prevalence of preoperative CAD in our study population was 79.7% (319/400 patients). None of the demographic and baseline clinical characteristics we studied predicted CAD in our study.
    Conclusion: We observed a significant prevalence of preoperative CAD among elective neurosurgical patients. None of the parameters we evaluated predicted CAD in our study.

    Abstract Background: We compared low-risk cesarean birth rates for Black and White women across hospitals serving increasing proportions of Black women and identified hospitals where Black women had low-risk cesarean rates less than or equal to White women.
    Methods: In this cross-sectional analysis of secondary data from four states, we categorized hospitals by their proportion of Black women giving birth from "low" to "high". We analyzed the odds of low-risk cesarean for Black and White women across hospital categories.
    Results: Our sample comprised 493 hospitals and the 65,524 Black and 251,426 White women at low risk for cesarean who birthed in them. The mean low-risk cesarean rate was significantly higher for Black, compared with White, women in the low (20.1% vs. 15.9%) and medium (18.1% vs. 16.9%) hospital categories. In regression models, no hospital structural characteristics were significantly associated with the odds of a Black woman having a low-risk cesarean. For White women, birthing in a hospital serving the highest proportion of Black women was associated with a 21% (95% CI: 1.01-1.44) increase in the odds of having a low-risk cesarean.
    Discussion: Black women had higher odds of a low-risk cesarean than White women and were more likely to access care in hospitals with higher low-risk cesarean rates. The existence of hospitals where low-risk cesarean rates for Black women were less than or equal to those of White women was notable, given a predominant focus on hospitals where Black women have poorer outcomes. Efforts to decrease the low-risk cesarean rate should focus on (1) improving intrapartum care for Black women and (2) identifying differentiating organizational factors in hospitals where cesarean birth rates are optimally low and equivalent among racial groups as a basis for system-level policy efforts to improve equity and reduce cesarean birth rates.

    Abstract Utilizing the expertise and knowledge resources of the diaspora, particularly the scientific diaspora, has been part of the strategies of many countries. In the recent years, realizing the importance of the potential of the diaspora to contribute to national development and Science, Technology, and Innovation ecosystem, countries have used Science Diplomacy also to engage with the scientific diaspora. Science Diplomacy is hailed as an enabler and facilitator and is often seen in the context of international S&T collaboration or big science projects. But the use of Science Diplomacy for diaspora engagement calls for specific strategies and meaningful initiatives. India is one of the major developing countries that has given a major thrust to engaging with the scientific diaspora. India is also a leading player in the global Science Diplomacy arena. This article critically examines India's initiatives and strategies for engagement with the scientific diaspora. It points out that the Science Diplomacy dimension is missing in this. Using examples from other countries, recent thinking, and developments in Science Diplomacy, this study outlines an approach with some examples of strategies and initiatives for harnessing Science Diplomacy to enhance engagement with the scientific diaspora and create a win-win milieu for India and the diaspora. The approach takes into account the proposed and ongoing initiatives in emerging technologies in India, including quantum technologies and Artificial Intelligence. Such a framework will create a synergy among various programs and initiatives by using Science Diplomacy as a facilitator and catalyst. Under this framework, Diaspora is involved not only as experts and contributors to scientific advancements but also as stakeholders. This dual role of the STI Diaspora can bring a paradigm shift in traditional understanding and use of science diplomacy, particularly to engage and harness the potential of the STI Diaspora for Sustainable Development.

    Abstract Introduction: Process safety is becoming more difficult as requirements increase. When balancing quality, volume, and safety on a budget, major accidents are unavoidable. The inability of organizations to absorb unwelcome and unanticipated disruptions is a major contributor to complacency and a decline in safety attitudes. The inability of industries to interpret standards impedes self-regulation. Methods: A total of 50 emergency management element questionnaires were developed to evaluate the state of an industry. These questionnaires were categorized as first aid, personal protective equipment (PPE), communication systems used in plants, safety education and training, occupational hygiene and health, management systems on accident reporting, safety and health, analysis and investigation, and procedures and protocols. From 32 LPG industries in Tamil Nadu districts, information for all 50 questions was gathered and the statistical tool "Analysis ToolPak" in Excel 2010 was used to analyze the data. Results: This study conducted a comprehensive statistical analysis of various aspects of industrial on-site emergency planning. The questionnaire's reliability was determined using Cronbach's alpha test. Utilizing descriptive statistics, t-tests for means, one-way ANOVA, and histograms, statistical evaluation was conducted. Encouragingly, all the results were favorable, providing compelling evidence that the LPG-utilizing industries in various regions of Tamil Nadu, India, are exceeding safety requirements. Conclusion: In conclusion, the findings suggest that improved legal frameworks, laws, and implementation of EME in communication systems can greatly enhance the development of safety in plants, fire protection, and emergency health services. These measures can significantly reduce the occurrence of accidents and improve the response time to emergencies, ultimately saving lives and reducing property damage.

    Abstract Introduction: Transarterial radioembolization (TARE) is one of the most promising therapeutic options for hepatic masses. Radiomics features, which are quantitative numeric features extracted from medical images, are considered to have potential in predicting treatment response in TARE. This article aims to provide meta-analytic evidence and critically appraise the methodology of radiomics studies published in this regard.
    Methods: A systematic search was performed on PubMed, Scopus, Embase, and Web of Science. All relevant articles were retrieved, and the characteristics of the studies were extracted. The Radiomics Quality Score and Checklist for Evaluation of Radiomics Research were used to assess the methodologic quality of the studies. Pooled sensitivity, specificity, and area under the receiver operating characteristic curve in predicting objective response were determined.
    Results: The systematic review included 15 studies. The average Radiomics Quality Score of these studies was 11.4 ± 2.1, and the average Checklist for Evaluation of Radiomics Research score was 33± 6.7. There was a notable correlation (correlation coefficient = 0.73) between the two metrics. Adherence to quality measures differed considerably among the studies and even within different components of the same studies. The pooled sensitivity and specificity of the radiomics models in predicting complete or partial response were 83.5% (95% confidence interval 76%-88.9%) and 86.7% (95% confidence interval 78%-92%), respectively.
    Conclusion: Radiomics models show great potential in predicting treatment response in TARE of hepatic lesions. However, the heterogeneity seen between the methodologic quality of studies may limit the generalizability of the results. Future initiatives should aim to develop radiomics signatures using multiple external datasets and adhere to quality measures in radiomics methodology.

    Abstract Introduction: Black and underinsured women in the United States are more likely than their counterparts to develop uterine fibroids (UFs) and experience more severe symptoms. Uterine artery embolization (UAE), a uterine-sparing therapeutic procedure, is less invasive than the common alternative, open hysterectomy. To determine whether demographic disparities persist in UF treatment utilization, we reviewed patient characteristics associated with UAE versus hysterectomy for UF among studies of US clinical practices.
    Methods: A systematic literature review was conducted via PubMed, Embase, and CINAHL (PROSPERO CRD42023455051), yielding 1,350 articles (January 1, 1995, to July 15, 2023) that outlined demographic characteristics of UAE compared with hysterectomy. Two readers screened for inclusion criteria, yielding 13 full-text US-based comparative studies specifying at least one common demographic characteristic. Random effects meta-analysis was performed on the data (STATA v18.0). Egger's regression test was used to quantify publication bias.
    Results: Nine (138,960 patients), four (183,643 patients), and seven (312,270 patients) studies were analyzed for race, insurance status, and age as predictors of treatment modality, respectively. Black race (odds ratio = 3.35, P < .01) and young age (P < .05) were associated with UAE, whereas private insurance (relative to Medicare and/or Medicaid) was not (odds ratio = 1.06, P = .52).     Conclusions and implications: Knowledge of demographic characteristics of patients with UFs receiving UAE versus hysterectomy is sparse (n = 13 studies). Among these studies, which seem to be racially well distributed, Black and younger women are more likely to receive UAE than their counterparts.
