    Abstract Leptospirosis is an important sporadic zoonotic disease. This disease is caused by the spirochete leptospira icterohemorrhagica. In India, leptospirosis has always remained a major health concern, particularly during monsoons. This disease mainly affects vital organs such as the lung, kidney, and heart. Acute respiratory distress syndrome and acute renal failure mainly contribute to the mortality in patients, but cardiac involvement leading to death still remains an underestimated critical clinical factor. We report here two autopsy cases and highlight histopathological changes in heart which helped us in diagnosing in correlation with other organ findings the final cause of death as leptospirosis.

    Abstract Introduction: Coronavirus Disease 2019 (COVID-19) pandemic has compelled medical education institutes to impart knowledge to the undergraduate MBBS students through online medium. In order to adopt the new online pedagogy it is important to understand perceptions of students and their educators. Further, it is imperative for us to know the shortcomings of online teaching methodologies to develop and inculcate newer teaching methods to maintain quality education. Aim: To understand the perception and challenges faced by the first year MBBS students, faculty and their parents during the compelling online classes as teaching methodology compared to Offline. Materials and Methods: The present study was questionnaire- based, cross-sectional conducted in the Department of Biochemistry, Employees’ State Insurance Corporation (ESIC) Medical College and Hospital, Faridabad, Haryana, India, between August 2021 and October 2021. The present study included students of MBBS first year (n=123), faculty (n=27) from ESIC Medical College and Hospital, Faridabad, India, and parents/ guardians of students (n=94). A five-point Likert scale was used to determine the perception of students/faculty/parents. Mean value calculated greater than 3 was considered disagreement and less than 3 as agreement. Online mode (Edumarshal software, Google forms) was used. Statistical analysis was done calculating mean, Standard Deviation (SD) and Paired t-test was used to study significance of study parameters on continuous scale. Results: Student’s perception was that offline teaching was significantly better than online classes in term of enthusiasm, interaction, clarity, periodic assessments (p-value<0.001). Faculty perceived that interference of the external environment was significantly more during online class (Mean±SD=1.07±0.99), periodical assessments were better conducted offline (Mean±SD=1.04±1.05) compared to online.     Abstract Background: Rapid nutrition screening (NS) is vital for apt management in patients with alcoholic liver cirrhosis (ALC).
    Aim: To identify a quick method of NS having high reliability and prognostic significance.
    Methods: NS of patients with ALC was assessed using mid-upper arm circumference (MUAC), handgrip strength (HGS), fat-free mass index (FFMI), and the Royal Free Hospital-Global Assessment (RFH-GA). Baseline clinical and biochemical information were recorded along with 90-day survival data. The classification and regression tree method was used to classify HGS, MUAC, and FFMI values as well nourished (WN), moderately malnourished (MM), and severely malnourished (SM), and their concordance with RFH-GA categories was assessed using Kendall tau-b coefficient. The prognostic proficiency of each method was tested by Cox regression analysis.
    Results: According to the RFH-GA, of 140 male patients with ALC, 13 of 140 (9.3%) were WN, 93 of 140 (66.4%) were MM, and 34 of 140 (26.8%) were SM. HGS has the strongest association with the RFH-GA (Kendall tau-b = 0.772; diagnostic accuracy -81.4%). HGS was found to be the independent predictor of 90-day mortality (26 of 140 [18.6%]; hazard ratio, 0.93; 95% CI, 0.88-0.98; P = 0.002) after adjusting for age, body mass index, and disease severity. The hazard of mortality was 8.5-times higher in patients with ALC with HGS < 22 kg as compared with those with HGS > 29.
    Conclusion: HGS is a reliable tool for rapid NS. HGS < 22 kg suggests a high risk for severe malnutrition and is strongly associated with short-term mortality in male patients with ALC.

    Abstract Background: Suicide results from complex interactions of various risk factors-reasons for dying (RFD)-and protective factors-reasons for living (RFL). Suicide is not necessarily a wish to die but may be an appeal for help. We analyzed RFD and RFL in persons who had attempted suicide, through their clinical records at a Crisis Intervention Clinic (CIC).
    Methods: We retrospectively analyzed demographic and clinical data, and classified RFD and RFL, among patients with either ideas or attempt of suicide registered at our CIC (
    Results: In the total sample, males and females were equally represented and educated, but males were significantly older. Most common diagnosis was nonpsychotic mood disorder. Commonest mode of suicide attempt was hanging. Family conflict vs. family responsibility, hope vs. hopelessness, stressful life events, and negative cognitions about the self and the world were important RFD. RFL included feeling responsible, love for family and for self, hope, career success, and religious beliefs.
    Conclusion: RFD and RFL could both be grouped in similar categories related to family, career, hope, etc.

    Abstract Metanephric adenoma (MA) is an unusual renal neoplasm with benign behaviour most of the times. Clinically MA mimics malignant renal neoplasms due to nonspecific signs and symptoms such as polycythaemia, haematuria and abdominal pain. MA usually presents as a mass lesion on radiographic studies and can be found incidentally. The treatment of choice is surgical excision. Though MA is usually a benign tumour, increase in the knowledge of MA pathology may lead to less invasive treatments in the future.     Abstract Background: Human immunodeficiency virus (HIV) infection in India has now been prevalent over three decades, and an increasing number of children are being affected with HIV. The spectrum of pathologic lesions in children with acquired immunodeficiency syndrome (AIDS) in India has not been well described.
    Materials and methods: A review of systematically conducted autopsies of 11 (10 boys and 1 girl) children with AIDS is presented.
    Results: The mode of HIV transmission in 6 children was vertical; in one it was blood transfusion and in 4 children route was presumably vertical as these were children of orphanage. The clinical manifestations were failure to thrive; 9 children, persistent gastroenteritis; 8, recurrent fever; 5, bacterial infections; 5, hepatosplenomegaly; 5, candidiasis; 1, scabies; 1, skin rash; 2, tuberculous (TB) meningitis; 1 and paraplegia; in 1 child. The spectrum of pathologic lesions observed were precocious involution in thymus in 3 and dysinvolution in 2 cases. Infectious diseases comprised of TB; 4 cases, cytomegalovirus infection (CMV) 4; bacterial pneumonia and meningitis; 7, and esophageal candidiasis in 2 cases. Dual or multiple infections were observed in 9 (82%) cases; these comprised of two lesions in 2, three lesions in 2, four lesions in 4, and five lesions in 1 case. TB, bacterial pneumonia, meningitis, and CMV infection are the most frequent causes of death in children with AIDS. Vascular lesions showing features of arteriopathy were observed in 5 cases and brain in one case showed non-Hodgkin's lymphoma.
    Conclusions: This study provides a better insight into the spectrum of pathologic lesions in children with AIDS in India. TB and CMV infection has been found to be the most prevalent infection in our children.
