    Abstract Background: Multiple sclerosis (MS) accounts for 176 cases per 100,000 inhabitants (female/male ratio = 2:1) in Italy. For most of the patients (67%), the disease course is relapsing-remitting MS (RRMS).
    Objective: To compare the costs and quality-adjusted life years (QALYs) of teriflunomide in RRMS naïve patients vs. RRMS patients previously treated (experienced) with other disease-modifying therapies in Italy.
    Methods: A four health states Markov model-supported cost-utility analysis (CUA) covering a 7-year timespan through annual cycles was developed, following the healthcare sector and the societal viewpoints. Part of the parameters that populated the Markov model was obtained from a questionnaire administered to four primary Italian MS centres. Costs of healthcare and non-healthcare resources, expressed in euro (€) 2019, and QALYs were discounted at 3% real social discount rate. One-way, scenario and probabilistic sensitivity analyses tested the uncertainty of the baseline findings.
    Results: Baseline CUA shows that teriflunomide in RRMS naïve patients is strongly dominant vs. experienced patients (healthcare sector perspective: - €1042.68 and + 0.480 QALYs; societal perspective: - €6782.81 and + 0.480 QALYs). Sensitivity analyses confirmed the robustness of the baseline results.
    Conclusion: Teriflunomide in RRMS naïve vs. experienced patients is cost-effective and possibly strongly dominant from both the healthcare sector and the society viewpoints in Italy. Our findings need further confirmation from real-world studies.

    Abstract Purpose of review: In patients with muscle invasive bladder cancer (MIBC) or unresectable non-MIBC, radical cystectomy is routinely combined with bilateral pelvic lymph node dissection (LND) owing to the oncological benefits found in recurrence-free survival (RFS), overall survival (OS) and cancer-specific survival (CSS) compared with radical cystectomy alone. However, the optimal anatomic extent of LND is still unclear.
    Recent findings: Retrospective studies were consistent in reporting oncological benefits of extended LND over nonextended LND. A recent RCT (the LEA trial) failed to demonstrate any benefit in terms of RFS, CSS and OS of super-extended LND over standard LND. Several confounding factors hindered the interpretation of the results, leaving the question of the right extent for LND still open. Results of a similar study, the SWOG S1011 are, therefore, highly anticipated. This study differed from the LEA study in several aspects but might also turn out to be a negative study.
    Summary: There are still no firm data on the oncological benefit brought by more extended LND in patients with MIBC. Survival benefits seem limited, at least in the general population. Other factors could influence the impact of LND on survival, including the administration of adjuvant and neoadjuvant chemotherapies.

    Abstract Management strategy evaluation (MSE) has become a more common tool for engaging stakeholders in fisheries management, and stakeholder participation in MSE is increasingly recognized as a vital component of the process. The participation of stakeholders, specifically fishers, in MSE is of particular importance because they often possess intimate knowledge of the socio-ecological management system that MSE seeks to model. When the resources to conduct a “full” MSE with direct fisher involvement are unavailable, MSEs are sometimes conducted by desk-based analysts with no fisher engagement. We propose an intermediate framework in which information collected from semi-structured interviews is used to inform a “desk-based” MSE. We demonstrate that semi-structured interviews with commercial and recreational fishers can elicit some of the same kinds of information that fishers provide during direct participation in MSE. We conducted 30 semi-structured interviews with commercial and recreational fishers from the Southeast United States participating in either Atlantic cobia (Rachycentron canadum) or black sea bass (Centropristis striata) fisheries. We collected primarily qualitative and some quantitative information about preferred conceptual objectives and management measures, and how their fishing behavior has changed in response to past management action. Commercial fishers generally preferred conceptual objectives and management measures that align with traditional MSY-based fisheries management, while recreational fishers’ responses were substantially more heterogeneous, indicating a more diverse range of desired objectives and preferred management measures. We synthesized this information to develop a suite of management procedures that employ a range of fishing mortality-based constant-catch harvest control rules and size-based management measures for simulation testing against preferred objectives by sector.
