    Abstract Spinal cord compression may lead to pain that is sometimes directed to areas far below the compression level. In certain cases, it may present as sciatica pain, knee pain or low back pain (LBP). These types of pain are called tract pain or funicular pain. Tract pain because of cervical spondylotic myelopathy (CSM) may lead to delays in the diagnosis and treatment of CSM in some cases, and sometimes unnecessary medical and surgical treatments. This study evaluated the results of four patients who presented to the outpatient clinic with complaints of LBP accompanying CSM findings. This study aims to present the improvement in low back pain as a result of anterior cervical microdiscectomy and cage procedure in four patients who presented with tract pain because of CSM, which is a rare condition.

    Abstract Purpose: Intracranial aneurysms are the most common reason of the subarachnoid hemorrhages and cause significant rate of morbidity and mortality. In this article, we aimed to share the results of our intracranial aneurysm surgery. Material and Methods: The results have been obtained by scanning the data of the patients who operated or treated by the reason of SAH and intracranial aneurysm, in our hospital between the years 2003 and 2015. Results: The 344 subarachnoid hemorrhage cases admitted to Sanko University Konukoglu Hospital and 313 intracranial aneurysms were identified on 270 (78.5%) cases and 311 aneurysms were surgically clipped. There were single aneurysm on 232 (85.9%) and multiple aneurysms on 38 (14.1%) cases. Clinically vasospasm occurred in 19.6% of cases. There was a strong correlation between Hunt and Hess grade on admission and vasospasm development. The mortality of single and multiple aneurysm were 12.5% and 26.5% respectively. The overall mortality and morbidity rates were 14.4% and 21.5% respectively . Conclusion: Multiple aneurysms, poor grade and intracerebral hematomas are poor prognostic factors. The patients who have subarachnoid hemorrhage should be monitored by an experienced team of neurosurgery and intensive care and should be operated on as soon as possible because of rehemorrhage risk. The surgical clipping as a gold standard is still an affective and successful method. [Cukurova Med J 2016; 41(4.000): 732-743]

    Abstract Object: Since the atlantoaxial region have critical neurovascular anatomy and limited bone surface for fusion, the application and choice of salvage fixation techniques are highly important. To discuss alternative posterior atlantoaxial fixation surgery techniques.
    Methods: We retrospectively surgical records of 22 patients that posterior atlantoaxial fixation techniques were applied.
    Results: The patients included 11 males and 11 females (mean age: 65.7 years). The fracture type that caused instability is type 2 odontoid fractures (22). In six of these patients alternative stabilization techniques were applied due to anatomical variations, huge venous bleeding and iatrogenic trauma of the screw entry points during surgery.
    Conclusions: Owing to anatomical variations, intraoperative challenges, and/or instrumentation failures, performing alternative surgical fixation technique is an important factor that affects the success of stabilization of the atlantoaxial region. Knowledge of salvage techniques especially during the learning curve is vitally important. Surgeons should adapt to intraoperative surgical challenges as required.

    Abstract Bu yazıda nadir bir durum olan basiller arter anevrizmasının kitle etkisinin yol açtığı obstrüktif hidrosefalili bir olgu bildirilmiştir. Hidrosefaliye yol açmış yüksek yerleşimli basiller tepe de gelişen dev multilobüle anevrizma, klipleme ve lamina terminalis fenstrasyonu ile başarılı bir şekilde tedavi edilmiştir. Anevrizmaya bağlı gelişen hidrosefali tedavisinin yönetimi tartışmalıdır. Ventriculoperitoneal shunt ve endoskopik third ventrikülostomi tedavi seçenekleri arasındadır. Hidrosefaliye yol açan yüksek yerleşimli basiller tepe anevrizmasının mikroşirürjikal olarak anevrizmanın klipajı ve lamina terminalis fenestrasyonu hem anevrizma hem de hidrosefali tedavisinde en güvenli ve fizyolojik yöntemdir.

    Abstract Aim: Bone mineral density (BMD) generally assesses fracture risk in the elderly but is not included in assessment of vertebral fracture status. In this study we aimed to investigate spinal alignment and pelvic orientation in patients with osteoporosis and identify indicators of vertebral fractures (VFs).Methods: Seventy patients above 50 years of age with osteoporosis were included in this retrospective cohort study. Patients were allocated to two groups comprising 29 patients with and 41 patients without VFs. Demographic and clinical characteristics and back pain scores evaluated by Visual Analogue Scale were obtained by scanning patient files. Sagittal vertebral axis (SVA), spinal and pelvic parameters were evaluated with lateral radiography. All parameters and their effect of VFs were compared in both groups. Results: Femoral neck BMD, sacral slope, lumbar lordosis, and pain scores were significantly different in patients with and without VFs (P=0.016, P=0.032, P=0.010, P<0.001, respectively). However, no significant difference was observed in terms of lumbar spine BMD, pelvic tilt, pelvic incidence, and thoracic kyphosis (P=0.394, P=0.313, P=0.258, P=0.341, respectively). Sacral slope and lumbar lordosis were positively correlated in patients with and without VFs (r=0.54, P=0.003 and r=0.50, P=0.001, respectively). SVA>50 mm and pain scores were predictors of VFs according to results of logistic regression. Conclusion: The spinal deformity in patients with osteoporosis may be explained by the spinal parameters. In our study, we concluded that pain and sagittal imbalance in osteoporosis patients are important parameters for vertebral fractures.

    Abstract Removing a broken pedicle screw is not always easy. Different methods and tools have been developed to remove the broken screw. Preserving the pedicle is an advantage for re-instrumentation. The head of the broken screw can be modified with the aid of a high-speed drill and the screw can be removed using a fractured screw removal tool without any problems. In addition, performing this procedure under the microscope reduces the risk of injury in the surrounding dura mater and the root. We believe that this simple technique can be conveniently used while removing a broken pedicle screw.

    Abstract Benign veya malign karakterli üçüncü ventrikül yerleşimli kitleler farklı klinik bulgular ile presente olabilirler. Bu çalışmamızda üçüncü ventrikül yerleşimli 3 kolloid kist ve nadir olan bir kavernöz hemanjiomdan oluşan olgularımız değerlendirilmiştir. Olguların klinik, radyolojik bulguları ve interhemisferik transkallosal transforaminal cerrahi yaklaşımın sonuçları ile beraber kavernöz hemangiomun sık rastlanmayan bu yerleşimi tartışılmıştır. Ventrikül anterioruna yerleşmiş kolloid kist ve kavernöz hemanjiom gibi benign lezyonlarda interhemisferik trankallozal ve transforaminal yaklaşım total eksizyon için güvenli bir yoldur.
