Abstract Problem/Intro: Epidemiological data of injuries and illnesses in equestrian athletes are limited. The objective of this study was to describe the incidence, prevalence, and severity of health problems among a cohort of elite riders. Methods: We conducted a prospective cohort study with 38 elite equestrian athletes over a two-year follow-up period. Health problems were recorded weekly with the Oslo Sports Trauma Research Center Questionnaire on health problems and subsequently categorized into illnesses and injuries according to the recently published International Olympic Committee guidelines. Based on the athletes weekly responses, we calculated health problem prevalence, incidence and severity. Results: A total of 93 health problems were reported including 30 illnesses and 63 injuries resulting in a weekly prevalence of 12.1% (95% CI: 10.5-13.7) for all health problems. For acute injuries, the chest, lumbosacral region, and neck were the most commonly reported body locations. The lumbosacral region was the most commonly reported repetitive injury mechanism body region. Illnesses most often affected the upper respiratory and gastrointestinal body systems. Conclusion: There was a low prevalence of injuries among this cohort of elite athletes. Future efforts that further improve athlete safety should focus on repetitive mechanism injuries and implementation and scale-up of preventive equipment to the broader equestrian population.Key Words: Epidemiology, Acute Injury, Repetitive Injury, Injury Mechanism

Abstract Unlabelled: We follow people who are considered eligible for an elderly care provision. Through the estimation of a duration model we determine when they can be expected to express demand for another care provision. Data are drawn from the registrations of needs assessment agencies in The Netherlands.
Main findings: Age positively effects the probability of a reassessment if initially care at home was assessed. Living alone, access to informal care, and applications initiated by professionals increase transition probabilities. Also initial applications for care at home due to dementia are more likely to be followed by a quick reassessment. The results can aid assessment agencies to set validity terms of assessments, and in planning future supply of services.

Abstract Nineteen white rhinoceroses ( Ceratotherium simum ) were anesthetized with 4 mg of etorphine hydrochloride; 35-40 mg of midazolam; and 7,500 international units of hyaluronidase for dehorning purposes at a game ranch in South Africa, to investigate this anesthetic combination. Median time to recumbency was 548 sec (range 361-787 sec). Good muscle relaxation and no muscle rigidity or tremors were observed in 18 animals, and only 1 individual showed slight tremors. In addition, all animals received butorphanol i.v. 5 min after recumbency at the ratio of 10 mg of butorphanol per 1 mg of etorphine. Blood gas and selected physiologic parameters were measured in the recumbent animal, immediately before and 10 min after the administration of butorphanol. Statistically significant improvements were observed in blood gas physiologic and cardiopulmonary parameters 10 min after the administration of butorphanol, with a reduction in arterial partial pressure of carbon dioxide, systolic blood pressure, and heart rate and an increase in pH, arterial partial pressure of oxygen, oxygen saturation, and respiratory rate (all P < 0.005). After i.v. naltrexone reversal, recovery was uneventful, and median time to walking or running was 110 sec (range 71-247 sec). The results indicate etorphine and midazolam combination is an effective alternative anesthetic protocol and produces good muscle relaxation. Furthermore, i.v. butorphanol was associated with improved blood gas values and cardiopulmonary function for at least 10 min postinjection.
