    Abstract The technical complexity of gene expression profiling in routine practice has necessitated the use of surrogate molecular classification of breast cancer, based on immunohistochemical analyses.
    Background and objectives: The aim of this study was to compare the differences between histological and molecular subtypes of invasive lobular carcinoma (ILC) of the breast, in order to be able to predict the behavior and prognosis of the disease, as well as to effectively determine therapy.
    Material and methods: This study included 263 cases of breast ILC diagnosed over a seven-year period. The diagnosis of invasive lobular carcinoma is based on the characteristic growth pattern and phenotype of cancer cells with the respective subtypes: classic, alveolar, solid, tubulolobular, pleomorphic and mixed lobular type. The examined cases were divided into five groups according to molecular classification based on the expression of ER, PR, HER2 and Ki67 immunohistochemical markers.
    Results: It was found that the pleomorphic subtype occurred statistically significantly less often as the luminal A subtype compared to others (
    Conclusions: The results of this study significantly singled out the luminal A subtype, and among them classic ILC, as the subtype with the most favorable expression ratio of the investigated predictive/prognostic immunohistochemical markers.

    Abstract Integral circulant graphs are proposed as models for quantum spin networks that permit a quantum phenomenon called perfect state transfer. Specifically, it is important to know how far information can potentially be transferred between nodes of the quantum networks modelled by integral circulant graphs and this task is related to calculating the maximal diameter of a graph. The integral circulant graph $ICG_n (D)$ has the vertex set $Z_n = \{0, 1, 2, \ldots, n - 1\}$ and vertices $a$ and $b$ are adjacent if $\gcd(a-b,n)\in D$, where $D \subseteq \{d : d \mid n,\ 1\leq d<n\}$. Motivated by the result on the upper bound of the diameter of $ICG_n(D)$ given in [N. Saxena, S. Severini, I. Shparlinski, \textit{Parameters of integral circulant graphs and periodic quantum dynamics}, International Journal of Quantum Information 5 (2007), 417--430], according to which $2|D|+1$ represents one such bound, in this paper we prove that the maximal value of the diameter of the integral circulant graph $ICG_n(D)$ of a given order $n$ with its prime factorization $p_1^{\alpha_1}\cdots p_k^{\alpha_k}$, is equal to $r(n)$ or $r(n)+1$, where $r(n)=k + |\{ i \ | \alpha_i> 1,\ 1\leq i\leq k \}|$, depending on whether $n\not\in 4N+2$ or not, respectively. Furthermore, we show that, for a given order $n$, a divisor set $D$ with $|D|\leq k$ can always be found such that this bound is attained. Finally, we calculate the maximal diameter in the class of integral circulant graphs of a given order $n$ and cardinality of the divisor set $t\leq k$ and characterize all extremal graphs. We actually show that the maximal diameter can have the values $2t$, $2t+1$, $r(n)$ and $r(n)+1$ depending on the values of $t$ and $n$. This way we further improve the upper bound of Saxena, Severini and Shparlinski and we also characterize all graphs whose diameters are equal to $2|D|+1$, thus generalizing a result in that paper.

    Abstract This study investigated the effects of knee joint angle on muscle activation, exerted torque, and whether the knee angle affects the muscle activation−torque ratio. Nine healthy adult male participants participated in the study. They performed maximal voluntary isometric contraction (MVIC) at six (80°, 90°, 100°, 110°, 120°, and 130°) different knee joint angles (i.e., angles between the thigh and shin bones). Their maximal torque was assessed utilizing an isokinetic chair, while their muscle activation (root mean square [RMS]) was assessed using an eight-channel single differential surface EMG sensor. For the purposes of the torque−knee angle relationship and muscle activation−knee angle relationship, the torque and RMS were normalized relative to the maximal value obtained by each participant. To evaluate the muscle activation−torque ratio in function of knee angle, RMS was normalized relative to the corresponding torque obtained at each knee angle. Repeated measure analysis of variance was used to investigate the effects of knee angle on muscle activation, torque, and muscle activation−torque ratio. There was a significant effect of knee joint angle on normalized torque (F = 27.521, p < 0.001), while the activation of vastus lateralis and vastus medialis remained unchanged. The changes in knee angle affected the muscle activation−torque ratio of vastus lateralis (Chi-square = 16.246, p = 0.006) but not the vastus medialis. These results suggest that knee joint angles from 80° to 130° provide a stable milieu for muscle electrification, while mechanical factor such as knee joint angle (i.e., lever arm length) affect the torque output when one needs to contract quadriceps maximally during the isometric contraction.

    Abstract Introduction: Aim of this study was to explore the survival predictive factors and treatment outcomes in a cohort of SGC patients treated at a single center over a period of 25 years.
    Materials and methods: Patients who had received primary treatment for SGC were enroled. Outcomes evaluated were: overall survival (OS), disease specific survival (DSS), recurrence free survival (RFS), locoregional recurrence free survival (LRFS) and distant metastasis free survival (DFS).
    Results: A total of 40 patients with SGC were enroled in the study. The most common tumor was the adenoid cystic carcinoma (60% of cases). Cumulative OS for 5-and 10-year follow up period was 81% and 60%, respectively. Thirteen patients (32.5%) developed distant metastases during follow-up. Nodal status, high-grade histology, tumor stage and adjuvant radiation-therapy (RT) were significant variables on multivariate analysis for survival and treatment outcomes.
    Conclusions: Submandibular gland carcinomas represent rare and heterogenous tumor group regarding histological appearance and locoregional and distant metastatic potential. Tumor histological grade, AJCC tumor stage and nodal status were the strongest predictive factors for survival and treatment outcomes. RT improved OS and locoregional treatment outcome, but not DFS. Elective neck dissection (END) could be beneficial for selected cases of SGC. Superselective neck dissection of levels I-IIa may be the level of dissection for END. Distant metastases were the main cause of death and treatment failure. Prognostic factors for poor DMFS were AJCC stage III and IV, high tumor grade and nodal status.

    Abstract Aim: This study aimed to assess the impact of COVID-19 infection on cardiac surgery outcomes in patients who contracted COVID-19 peri-operatively or had recently recovered from COVID-19.
    Methods: The study prospectively enrolled 95 patients scheduled for cardiac surgery who had recently recovered from COVID-19. This formed the post-COVID-19 group. The other group consisted of 25 patients who contracted COVID-19 peri-operatively. Patients were followed for all-cause mortality as the primary endpoint and postoperative course complications as the secondary endpoint. Data were compared to a historical cohort of 280 non-COVID-19 patients.
    Results: The peri-operative COVID-19 group exhibited a significantly higher prevalence of primary outcome all-cause mortality (28%), compared with 4.3% in the controls (
    Conclusion: Patients who contracted COVID-19 peri-operatively had an increased rate of mortality and postoperative complications, while cardiac surgery in the recently recovered COVID-19 group was associated with a higher incidence of pulmonary embolism and atrial fibrillation.

    Abstract Objective: Risk factors for epilepsy in meningioma patients are not yet clearly defined, however, seizure freedom is a significant factor for quality of life after surgery.
    Methods: We performed a retrospective study of the 333 adult patients who received surgery for supratentorial meningioma at our center. Various clinical, radiological, and surgical variables were included in the multivariate regression, and the outcomes measured were the occurrence of seizure(s) preoperatively, during the hospitalization, and during the follow-up period.
    Results: A total of 89 (26.7%) patients experienced preoperative seizures, of whom 62.9% were seizure free after the surgery. Of 244 patients without epilepsy before surgery, 11.9% had at least one seizure postoperatively. In total, 63 of our patients (18.9%) experienced seizures after the surgery, of whom 20 had refractory epilepsy. Multivariate analysis identified the following predictors of preoperative seizures: the absence of headache (OR: 0.23, CI: 2.55-8.50), the presence of significant peritumoral edema (OR: 4.35, CI: 2.57-7.35), and younger age (OR: 0.97 per year increase, CI: 0.95-0.99). Factors associated with early postoperative seizures were: younger age (OR: 0.96 per year increase, CI: 0.93-0.99) and the presence of preoperative seizures (OR: 2.73, CI: 1.13-6.57), while the presence of preoperative seizures (OR: 4.73, CI: 2.05-10.92), tumor progression (OR: 5.38, CI: 2.25-12.89), and neurological worsening (OR: 5.21 CI: 1.72-15.81) were significant for late postoperative seizures.
    Significance: Our results from a single-center meningioma cohort confirm, in general, data from some previous studies regarding patients' characteristics for both preoperative and overall postoperative epilepsy. Besides previously described risk factors, younger age was important for preoperative and early postoperative seizures. Epilepsy is common in patients with recurrence of meningioma, but the variables of significance for refractory seizures in these patients require further examination.

    Abstract Oral squamous cell carcinoma (OSCC) is the eighth most common type of cancer in the world. Knowledge of prognostic factors of survival in OSCC is key. Several clinical and pathological prognostic factors have been investigated to develop a prognostic model of survival for patients with oral cancer. The present study focused on the association between pathological tumor volume (PTV) and overall survival time in patients with OSCC, regardless of cervical nodal status. The present study was a prospective study and covered 65 consecutive patients who received surgical treatment for oral cancer. The PTV was calculated according to dimensions of the postoperative specimen. Other pathological parameters as perineural and perivascular tumor spreading and extra-nodular propagation were also determined. The data were analyzed using the IBM SPSS 25.0 software. Cox PH regression model was built to analyze association between the PTV and survival time. Survival time was defined as the period from surgery to a target event or last contact. The results of the present study showed that PTV >4.24 cm

    Abstract Adopting adequate operational and safety measures is a significant part of the investigation of transient processes of hydropower plants (HPPs), both throughout the design stage of prospective plants and throughout the planning stage of the HPPs envisaged for refurbishment/uprating. One of the surge tank (ST) types, insufficiently researched so far, with a potential positive techno-economical influence, is the semi-pneumatic surge tank (SPST). Since the detailed analyses of SPST have never been performed or published before, the SPSTs have been examined here by presenting theoretical considerations, a newly developed mathematical model and comprehensive numerical simulations. The aim is to improve the performance of the open-air variations of STs. Multiple numerical simulations for a specific case-study HPP have been performed with the conclusions about the peculiarities and benefits of the SPST implementation. Firstly, numerical results for the existing as-built surge tank and corresponding alternative SPST have been compared, in order to verify the developed model. Variations of the main SPST constructive parameters have been analyzed, with the aim to determine the sensitivity of particular influences on transient processes versus its geometry alterations. The conducted analyses show that the SPST application brings HPP transient behaviour improvement compared to open-air ST. Dimensions of the new surge tank with specific aeration orifice could be smaller than the ones previously defined: both the diameter of the optimized upper and lower chamber and the core could be decreased by 20% and 12.5%, respectively.

    Abstract Background: In the pursuit of a better understanding of biodiversity, evolutionary biologists rely on the study of phylogenetic relationships to illustrate the course of evolution. The relationships among natural organisms, depicted in the shape of phylogenetic trees, not only help to understand evolutionary history but also have a wide range of additional applications in science. One of the most challenging problems that arise when building phylogenetic trees is the presence of missing biological data. More specifically, the possibility of inferring wrong phylogenetic trees increases proportionally to the amount of missing values in the input data. Although there are methods proposed to deal with this issue, their applicability and accuracy is often restricted by different constraints.
    Results: We propose a framework, called PhyloMissForest, to impute missing entries in phylogenetic distance matrices and infer accurate evolutionary relationships. PhyloMissForest is built upon a random forest structure that infers the missing entries of the input data, based on the known parts of it. PhyloMissForest contributes with a robust and configurable framework that incorporates multiple search strategies and machine learning, complemented by phylogenetic techniques, to provide a more accurate inference of lost phylogenetic distances. We evaluate our framework by examining three real-world datasets, two DNA-based sequence alignments and one containing amino acid data, and two additional instances with simulated DNA data. Moreover, we follow a design of experiments methodology to define the hyperparameter values of our algorithm, which is a concise method, preferable in comparison to the well-known exhaustive parameters search. By varying the percentages of missing data from 5% to 60%, we generally outperform the state-of-the-art alternative imputation techniques in the tests conducted on real DNA data. In addition, significant improvements in execution time are observed for the amino acid instance. The results observed on simulated data also denote the attainment of improved imputations when dealing with large percentages of missing data.
    Conclusions: By merging multiple search strategies, machine learning, and phylogenetic techniques, PhyloMissForest provides a highly customizable and robust framework for phylogenetic missing data imputation, with significant topological accuracy and effective speedups over the state of the art.

    Abstract Introduction. Ectopic cervical thymus (ECT) occurs as a result of incomplete migration of the thymic primordia during embryogenesis. In the majority of cases, ECT is asymptomatic; however, in 10% of patients, there are different kinds of symptoms. Case report. A four-month-old baby boy was referred to our clinic for an evaluation of a growing large mass on the right side of the neck, present since birth. Physical examination revealed a solid, painless, soft, moderately mobile mass of irregular round shape localized on the right side of the neck, in front of the sternocleidomastoid muscle, below the parotid gland, and above the carotid lodge. The skin above the mass was unchanged. The dimensions of the mass were 40 × 32 × 15 mm. Based on the clinical and ultrasonographic findings, as well as the findings of the magnetic resonance imaging, it was suspected that the mass was an ECT. The mass was removed by surgical excision. The pathohistology report confirmed the presence of an ECT, with Hassall’s corpuscles in the medulla. The postoperative course went smoothly, and the wound healed well. During the regular clinical, immunological, and ultrasound follow-ups over a period of six months, normal findings were registered. Conclusion. Congenital ECT is a rare congenital anomaly that must be, however, taken into account when considering the differential diagnosis of cervical tumor masses.
