Abstract	Study design: Secondary analysis of cross-sectional data from the ALLRISC cohort study. Objectives: To investigate the prevalence of obesity and its association with time since injury (TSI) and physical activity (PA) in wheelchair users with long-standing (TSI > 10 years) spinal cord injury (SCI). Setting: Community, The Netherlands. Methods: Wheelchair users with SCI (N = 282) in TSI strata (10-19, 20-29, and ≥30 years) and divided in meeting SCI-specific exercise guidelines or not. Waist circumference (WC) and body mass index (BMI) were assessed. Participants were classified as being obese (WC > 102 cm for men, WC > 88 cm for women; BMI ≥ 25 kg/m Results: Almost half of the participants (45-47%) were classified as obese. TSI was significantly associated with obesity, the odds of being obese were 1.4 higher when having a 10 years longer TSI. Furthermore, the odds of being obese were 2.0 lower for participants who were meeting the exercise guidelines. Conclusions: The prevalence of obesity is high in people with long-standing SCI. Those with a longer TSI and individuals who do not meet the exercise guidelines are more likely to be obese and need to be targeted for weight management interventions.

Abstract	Management and restoration of mangrove forests to protect coasts are promoted in many countries, including Indonesia. Indonesian mangrove forests are actively restored and managed by local communities for their ecosystem services, including coastal protection. Whether community-based mangrove management (CBMM) is effective is still debated. Our study analysed the effectiveness of different CBMM practices in four Central Javan communities by analysing the capacity of their mangrove forests to protect against coastal hazards. We used complementary interviews, field assessments and literature reviews to collect the necessary information. The overall CBMM performance and success significantly differed for each community's mangrove rehabilitation effort and the resulting coastal protection service. Of the four communities, Bedono performed best in terms of mangrove coverage, forest structure and restored coastal protection service. This is explained by multiple factors, such as application of long-term and integrated CBMM approaches, involving appropriate maintenance and additional measures to reduce wave energy. Our results can help governments, practitioners and communities to better understand the factors that contribute to CBMM's success and failure when restoring and managing mangrove forests and protecting coasts.

Abstract	Efforts to rehabilitate degraded mangrove ecosystems in Indonesia started in the 1960s and have recently received increased attention. Rehabilitation efforts have been mainly conducted through community-based mangrove management (CBMM) programs, aimed at restoring important services such as food provisioning and coastal protection, and for biodiversity conservation. Our study assessed the effectiveness of CBMM strategies to conserve biodiversity in four adjacent Indonesian coastal villages (Sriwulan, Bedono, Timbulsloko, Surodadi) in Central Java. For this, we used complementary methodologies combining participatory resource mapping, semi-structured interviews, questionnaire-based interviews, field assessments and literature review. This yielded detailed information on mangrove rehabilitation activities, management approaches and the impacts of mangrove rehabilitation on biodiversity in the four villages. Our analysis focussed on mangrove forest structure and diversity and macro invertebrate diversity.The overall comparison of management performance shows a higher achievement of CBMM applied in Bedono in terms of a larger mangrove diversity and net reforestation coverage, also supporting a higher macrobenthic faunal diversity compared to the other villages. The main contributing factors were a) the longer-term funding and maintenance, b) the greater acceptance for protective legislation, c) the higher levels of public support, d) the fact that more species of mangroves were used, e) the much larger spatial scale of mangrove restoration, and f) the presence of additional measures to reduce wave action in highly eroded areas. The results revealed key determinants of success when restoring mangroves for the purpose of biodiversity conservation and the influence of different CBMM approaches.

Abstract	Illness cognitions (IC) influence how a patient adapts to a chronic disease. The aim was (1) to determine if training for a handcycling mountain time trial (HandbikeBattle) improves IC and (2) to identify factors associated with IC change scores. Persons with a chronic disability (N = 220; including N = 151 with spinal cord disorder) trained 5 months and participated in the time trial. The IC Questionnaire measured helplessness, acceptance, perceived benefits and was assessed before training (T1), after training (T2), and four months after the event (T3). Age, sex, body mass index (BMI), time since injury (TSI), disability characteristics, self-efficacy, mental health (MH) and musculoskeletal pain were obtained at T1. Abstract	Purpose: To investigate: (1) changes in body satisfaction during five months of handcycle training and one year after the training period; (2) whether longitudinal changes are dependent on sex, waist circumference and severity of the physical impairment; (3) associations between changes in physical capacity or body composition, and body satisfaction. Materials and methods: Individuals ( Results: Multilevel regression analyses showed that body satisfaction significantly increased during the training period and significantly decreased back to pre-training levels at follow-up. Individuals with more severe impairments showed a larger decrease at T4. Improvements in physical capacity and waist circumference were significantly associated with improvements in body satisfaction. Conclusions: Body satisfaction significantly increased during the training period, but significantly decreased during follow-up. Additional efforts might be necessary to keep individuals engaged in long-term exercise.

Abstract	Background: Over the past decades, opioid prescriptions have increased in the Netherlands. The Dutch general practitioners' guideline on pain was recently updated and now aims to reduce opioid prescriptions and high-risk opioid use for non-cancer pain. The guideline, however, lacks practical measures for implementation. Objective: This study aims to determine practical components for a tool that should assist Dutch primary care prescribers and implements the recently updated guideline to reduce opioid prescriptions and high-risk use. Methods: A modified Delphi approach was used. The practical components for the tool were identified based on systematic reviews, qualitative studies, and Dutch primary care guidelines. Suggested components were divided into Part A, containing components designed to reduce opioid initiation and stimulate short-term use, and Part B, containing components designed to reduce opioid use among patients on long-term opioid treatment. During three rounds, a multidisciplinary panel of 21 experts assessed the content, usability, and feasibility of these components by adding, deleting, and adapting components until consensus was reached on the outlines of an opioid reduction tool. Results: The resulting Part A consisted of six components, namely education, opioid decision tree, risk assessment, agreements on dosage and duration of use, guidance and follow-up, and interdisciplinary collaboration. The resulting Part B consisted of five components, namely education, patient identification, risk assessment, motivation, and tapering. Conclusions: In this pragmatic Delphi study, components for an opioid reduction tool for Dutch primary care-givers are identified. These components need further development, and the final tool should be tested in an implementation study.

Abstract	Objective: Self-regulation, participation and health-related quality of life are important rehabilitation outcomes. The aim of this study was to explore associations between these outcomes in a multi-diagnostic and heterogenic group of former rehabilitation patients. Methods: This cross-sectional survey used the Self-Regulation Assessment (SeRA), Utrecht Scale for Evaluation of Rehabilitation-participation (USERParticipation) and the Patient-Reported-Outcome-Measurement-System (PROMIS) ability and PROMIS satisfaction with participation in social roles, and the EuroQol-5L-5D and PROMIS-10 Global Health. Regression analyses, controlling for demographic and condition-related factors, were performed. Results: Respondents (n = 563) had a mean age of 56.5 (standard deviation (SD) 12.7) years. The largest diagnostic groups were chronic pain disorder and brain injury. In addition to demographic and condition-related factors, self-regulation subscales explained 0-15% of the variance in participation outcome scores, and 0-22% of the variance in HRQoL outcome scores. Self-regulation subscales explained up to 22% of the variance in satisfaction subscales of participation (USER-Participation and PROMIS) and the mental health subscale of the PROMIS-10. Self-regulation subscales explained up to 11% of the restriction and frequency subscales of participation (USER-Participation) and the physical health subscale of the PROMIS-10. Conclusion: Self-regulation is more strongly associated with outcomes such as satisfaction with participation and mental health compared with outcomes such as restrictions in participation and physical health.
