    Abstract The eggs laid by the orb web spider Trichonephila clavata must overwinter in bitterly freezing and dry conditions before hatching, but there does not seem to be any protection like a compact silk case covering the entire eggmass. Instead, the surface of the eggmass is completely coated with a milky coating called chorionic microspheres (CM). Therefore, we investigated the fine structural characteristics of CM to demonstrate their ecological importance. Although the diameter of CM in outer eggmass exhibits a significant variation, the chorionic surface is coated with a single layer of CM, characterized by a consistent diameter of approximately 2.3 µm. The surface structure of aggregated CM shows short papillary projections demonstrating segmental adhesion of mucous components. CM is insoluble in water but partially soluble in anhydrous ethanol, and its spherical structure is completely decomposed by hexafluoroisopropanol (HFIP), a strong organic solvent. Since our fine structural observations clearly show that CM is not derived from vitellogenic or choriogenetic processes, the CM adhesive coatings during ovipositional process appears to be equivalent to cocoon silk for various protective functions in silken eggcase.

    Abstract The fighter pilots exposed to high gravitational (G) acceleration must perform anti-G maneuvers similar to the Valsalva maneuver. However, the effects of high-G acceleration and anti-G maneuvers on cardiac function have rarely been studied. This study aimed to investigate the effects of high-G forces on cardiac function of fighter pilots. Fighter pilots who underwent regular health check-ups and echocardiography were included (n = 29; 100% men, 41 ± 10 years old; mean flight time, 1821 ± 1186 h). Trainees who had not experienced any flights were included in the control group (n = 16; 100% men, 36 ± 17 years old). Echocardiographic data included left ventricular chamber size, systolic and diastolic functions, right ventricular systolic pressure (RVSP), inferior vena cava (IVC) collapsibility, and tricuspid annular plane systolic excursion (TAPSE). No significant differences in left ventricular ejection fraction, RVSP, or IVC collapsibility were observed between two groups. In the multivariate linear regression analysis with total flight time as an independent continuous variable for fighter pilots, TAPSE was positively correlated with total flight time. The experience of fighter pilots who were exposed to high-G acceleration forces and anti-G maneuvers did not cause cardiac structural changes, but the exposure might be associated with right heart function changes.

    Abstract The ultimate goal of cleft palate repair is to achieve an intact palate with the separation of the oral and nasal cavities. However, some patients develop an oronasal fistula in the secondary palate after palatoplasty. Postoperatively, a secondary palatal oronasal fistula may develop, leading to functional problems. In this study, we describe a patient with recurrent oronasal fistula and alveolar cleft with multiple failed previous reconstructions at another clinic. The oronasal fistula and alveolar cleft were repaired using a tongue flap and an iliac bone graft, respectively. The patient demonstrated excellent clinical progress with no recurrence of the oronasal fistula at the 1-year follow-up.

    Abstract Statistical and analytical methods using artificial intelligence approaches such as machine learning (ML) are increasingly being applied to the field of pediatrics, particularly to neonatology. This study compared the representative ML analysis and the logistic regression (LR), which is a traditional statistical analysis method, using them to predict mortality of very low birth weight infants (VLBWI). We included 7472 VLBWI data from a nationwide Korean neonatal network. Eleven predictor variables (neonatal factors: male sex, gestational age, 5 min Apgar scores, body temperature, and resuscitation at birth; maternal factors: diabetes mellitus, hypertension, chorioamnionitis, premature rupture of membranes, antenatal steroid, and cesarean delivery) were selected based on clinical impact and statistical analysis. We compared the predicted mortality between ML methods—such as artificial neural network (ANN), random forest (RF), and support vector machine (SVM)—and LR with a randomly selected training set (80%) and a test set (20%). The model performances of area under the receiver operating curve (95% confidence interval) equaled LR 0.841 (0.811−0.872), ANN 0.845 (0.815−0.875), and RF 0.826 (0.795−0.858). The exception was SVM 0.631 (0.578−0.683). No statistically significant differences were observed between the performance of LR, ANN, and RF (i.e., p > 0.05). However, the SVM model was lower (p < 0.01). We suggest that VLBWI mortality prediction using ML methods would yield the same prediction rate as the traditional statistical LR method and may be suitable for predicting mortality. However, low prediction rates are observed in certain ML methods; hence, further research is needed on these limitations and selecting an appropriate method.
