    Abstract Bipolar disorder (BD) is a high-suicide-risk mental disorder. The purpose of this study was to identify the relationship between temperament and character traits with suicide probability, suicide attempts, and perceived stress level in patients with BD. A total of 39 euthymic patients with bipolar disorder who had a history of suicide attempts and 39 euthymic patients without a history of suicide attempts were included in this study. The sociodemographic and clinical data form, Hamilton Depression Rating Scale (HDRS), Young Mania Rating Scale (YMRS), Structured Clinical Interview for DSM-5-Clinician Version (SCID-5/CV), Temperament and Character Inventory (TCI), Perceived Stress Scale (PSS), and Suicide Probability Scale were used to obtain the data. HDRS, PSS, and SPS scores of the group comprised of patients who attempted suicide were higher than the other group. There was no significant difference between the group of patients who had attempted suicide and the other group in terms of temperament characteristics. In the group of patients who had attempted suicide, self-directedness (SD) and cooperativeness (CO) scores were lower, and the self-transcendence (ST) score was higher than the other group. HA and ST were positively and SD negatively associated with SPS scores. In the regression analysis for suicide risk, the factors most associated with suicide risk were high HDRS and low CO score. Low SD in BD and high ST with CO may be associated with suicide attempts. Alongside low SD, high HA and ST may be associated with suicidal ideation. Treating residual depressive symptoms can reduce the risk of suicide.

    Abstract Tens of thousands of people worldwide became infected with severe acute respiratory syndrome coronavirus-2. Death rate in the general population is about 1%-6%, but this rate rises up to 15% in those with comorbidities. Recent publications showed that the clinical progression of this disease in organ recipients is more destructive, with a fatality rate of up to 14%-25%. We aimed to review the effect of the pandemic on various transplantation patients. Coronavirus disease 2019 (COVID-19) has not only interrupted the lives of waiting list patients'; it has also impacted transplantation strategies, transplant surgeries and broken donation chains. COVID-19 was directly and indirectly accountable for a 73% surplus in mortality of this population as compared to wait listed patients in earlier years. The impact of chronic immunosuppression on outcomes of COVID-19 remains unclear but understanding the immunological mechanisms related to the virus is critically important for the lifetime of transplantation and immune suppressed patients. It is hard to endorse changing anti-rejection therapy, as the existing data evaluation is not adequate to advise substituting tacrolimus with cyclosporine during severe COVID-19 disease.

    Abstract Objective: This study aimed to show the effects of different exercise types on disease activity, pain, functional status, and quality of life in female patients with rheumatoid arthritis with low disease activity or in remission and supported these findings with body composition and muscle measurements.
    Design: This randomized controlled prospective study screened female patients aged 20-50 years with rheumatoid arthritis. The patients were randomized into 12-wk resistance exercise, aerobic exercise, and control groups.
    Results: The mean age of the 66 patients was 42.5 ± 5.6 yrs. In the resistance and aerobic exercise groups compared with the control group, a positive significant difference was found in the pain, disease activity, several subparameters of quality of life, M. gastrocnemius, and M. biceps femoris muscle thickness measurements, and fat mass in the lower limbs before and after treatment ( P < 0.05). Compared with the other groups, the resistance exercise group showed a significant improvement in M. rectus femoris and M. vastus intermedius muscle thickness measurements, whole body fat mass, whole body and lower extremity lean body mass, and timed up and go test when comparing before and after treatment ( P < 0.05).
    Conclusions: In rheumatoid arthritis patients, resistance exercises led to a significant increase in muscle thickness, functional status, lean body mass compared with other exercises; resistance exercises also resulted in a significant reduction in pain and disease activity.

    Abstract In the present study, the effects of several food colorings, namely (Ponceau 4R (E 124), Tartrazine (E 102), and Pea green (E 102-E 133), were investigated in vivo using the wing spot test, SMART (somatic mutation and recombination test), in Drosophila melanogaster. Food colorings are the food additives, which are used for improving the appearance of food and beverages. In SMART, multiple wing hair (mwh), flare (flr3), and beaded serrate (BdS) marker genes on the third-largest chromosome of Drosophila are used. The genotoxic effects of the food colorings on the imaginal disc cells that will develop into the wing spot cells during the embryonic development of Drosophila heterozygous larvae and the genotypic changes caused by mutation or recombination in somatic cells also play a role in the formation of mutant spots in the wings. Classes by mutant clones are as follows: small single spots containing 1-2 mwh, large single spots containing ≥3 mwh or ≥4 flr3, and twin spots containing adjacent mwh and flr3 cells (GRAF et al., 1984). Negative control medium was prepared with distilled water, while positive control medium was prepared with 1 mM EMS (ethyl methane sulfonate). According to results obtained from SMART, Ponceau 4R, Tartrazine, and Pea green demonstrated significant results in trans-heterozygous flies (mwh/flr3) for inducing the mutant wing spots compared to control groups at 25 mg/ml, 50 mg/ml, and 75 mg/ml exposure concentrations. On the other hand, Ponceau 4R, Tartrazine, and Pea green yielded significant results for inducing the mutant wing spots in balancer-heterozygous flies (mwh/TM3) at 25 mg/ml, 50 mg/ml, and 75 mg/ml exposure concentrations. The numbers of mutant wing spots were increased by all three colorings depending on the concentration (X2= df=3, P<0.001). It was also determined that these numbers were significantly higher than the flies in the negative control medium and it suggests that these food colorings have genotoxic effects.     Abstract Objectives: To study intrathecal kappa free light chain (KFLC) synthesis in people living with HIV (PLWH) in comparison with multiple sclerosis (MS).
    Methods: Cross-sectional analysis including 56 untreated and 150 well treated PLWH, and compared with 58 controls, and 223 MS patients.
    Results: Elevated serum/cerebrospinal fluid (CSF) IgG and KFLC indices were observed in untreated PLWH. Seventy percent of untreated PLWH had KFLC index above 6.1, a threshold associated with clinically isolated syndrome/MS diagnosis. No association was found between KFCL index and CSF markers of neuronal injury in either PLWH or MS patients.
    Conclusions: HIV-related immune system dysfunction is often associated with an elevated KFLC index akin to those observed in MS. HIV infection should be considered as a differential diagnosis for patients presenting with neurological symptoms and increased intrathecal immunoglobulin synthesis.

    Abstract Background: Our aim was to examine the prevalence and characteristics of difficult-to-treat HIV in the current Swedish HIV cohort and to compare treatment outcomes between people with difficult and non-difficult-to-treat HIV.
    Methods: In this cross-sectional analysis of the Swedish HIV cohort, we identified all people with HIV currently in active care in 2023 from the national register InfCareHIV. We defined five categories of difficult-to-treat HIV: 1) advanced resistance, 2) four-drug regimen, 3) salvage therapy, 4) virologic failure within the past 12 months, and 5) ≥ 2 regimen switches following virologic failure since 2008. People classified as having difficult-to-treat HIV were compared with non-difficult for background characteristics as well as treatment outcomes (viral suppression and self-reported physical and psychological health).
    Results: Nine percent of the Swedish HIV cohort in 2023 (n = 8531) met at least one criterion for difficult-to-treat HIV. Most of them had ≥ 2 regimen switches (6%), and the other categories of difficult-to-treat HIV were rare (1-2% of the entire cohort). Compared with non-difficult, people with difficult-to-treat HIV were older, had an earlier first year of positive HIV test and lower CD4 counts, and were more often female. The viral suppression rate among people with difficult-to-treat HIV was 84% compared with 95% for non-difficult (p = 0.001). People with difficult-to-treat HIV reported worse physical (but not psychological) health, and this remained statistically significant after adjustment for age, sex, and transmission group.
    Conclusions: Although 9% of the HIV cohort in Sweden in 2023 were classified as having difficult-to-treat HIV, a large proportion of these were virally suppressed, and challenges such as advanced resistance and need for salvage therapy are rare in the current Swedish cohort.

    Abstract INTRODUCTION: Although it is known that mean platelet volume (MPV), red blood cell distribution width (RDW), neutrophil-to-lymphocyte ratio (NLR) and lymphocyte-to-monocyte ratio (LMR) are predictive for arterial trombosis and adverse events, it is not clear whether they help in the diagnosis of deep vein thrombosis. The aim of the study was to evaluate it. For this purpose MPV, RDW, NLR and LMR were measured and compared between in patients with DVT and control groups. METHODS: In a retrospective study, a total of 144 subjects (77 patients with DVT, 67 control) were examined. Mean platelet volume, RDW, NLR and LMR were calculated and compared between the groups. It was investigated whether there was a correlation between D-dimer values and MPV, RDW, NLR, LMR. RESULTS: Mean platelet volume, RDW and NLR values were significantly higher in the patient group than in the control group (9.68+-1.89 vs. 8.9+-1.01, p=0.003, 12.77+-3.67 vs. 11.15+-2.16, p=0.002 and 1.91+-0.84 vs. 1.51+-0.54, p=0.001, respectively). On the other hand LMR was lower in the patient group (6.27+-3.14 vs. 8.85+-3.92, p<0.001). There was a positive correlation between D-dimer and MPV, RDW, NLR while there was a negative correlation between D-dimer and LMR (r=0.693, p<0.001, r=0.896, p<0.001, r=0.798, p<0.001 and r = -0.287, p=0.011, respectively). DISCUSSION AND CONCLUSION: Mean platelet volume, RDW and NLR increase but LMR decrease in patients with DVT. These results suggesting that these markers may help in the diagnosis of acute DVT.
