    Abstract Dear editor: We have read Dr Riccardo Fondrini and Colleagues commentary on our paper, and we are grateful for their remarks. We herewith would clarify the critical points mentioned about fat reabsorption and survival, along with the risk of periorbital chronic oedema as per the recorded outcomes in the presented case series. Lastly, we have unfortunately to disagree with the commentary's point related to recommended injection depth as it is mandatory to point out that in our experience the superficial injection of nano-fat is a crucial step for lower eyelid retraction management, otherwise a retraction may persist due to the untreated scar in the anterior lamella, thus missing the therapeutic goal that we aim to achieve.

    Abstract Background: When autologous septal cartilage is not enough or even not disposable for graft sculpting in revision rhinoplasty, valid alternatives have to be found. Both autologous and homologous costal cartilage usage has been described in scientific literature. As there is no universally accepted consensus on the cartilage choice to use in these cases, the experiences with the different types of cartilage usage assume significant importance in the rhinoplasty learning process.
    Objectives: This multicenter prospective study outlined an overview of the authors' experience regarding short-term and long-term complications following revision rhinoplasty procedures in which either fresh frozen (FFCC), in-alcohol (IACC) or autologous costal cartilage (ACC) was used.
    Methods: 671 patients undergoing revision rhinoplasty from June 2015 to September 2020 were divided into three groups according to the type of cartilage used (Group1/FFCC with 212 patients, group2/IACC with 239 patients and group3/ACC with 202 cases). Sociodemographic and clinical characteristics and short and long-term complications were described and discussed. A statistical analysis investigating a possible significance of the differences in complication rate was conducted.
    Results: Authors' data outlined a short-term general complication rate of 5.05%, and a long-term complication rate of 7.04%. A statistically significant difference was identified in cartilage warping rate between the homologous cartilages in comparison to ACC.
    Conclusions: FFCC, IACC and ACC can be safely used in revision rhinoplasty with no statistically significant differences regarding short- and long-term complications rate. Cartilage warping rate is significantly higher for ACC in comparison with FFCC and IACC.

    Abstract Background: The aim of this study was to evaluate the use of autologous fat graft injection to correct lower eyelid position METHODS: A retrospective, observational, single blind, case-control study was carried out on 94 patients, presenting with lower eyelid retraction in 159 eyes. In the sub-population with monolateral eyelid retraction, the not affected site has been considered as a control and compared with the outcomes recorded after treatment of the contralateral side Follow-up at 12 months was performed with a subjective assessment carried out by a questionnaire administered to patients while objective result assessment was performed 12 months after surgery by two independent blind examiners.
    Results: The eyelid upward reposition has improved one year after fat grafting in both bilateral (1,52 mm) and unilateral (1,7 mm) population: the latter allowed to statistically validate (P<0.05) the result with respect to the not affected site.
    Conclusions: This is the first paper that highlights the outcomes of sole fat injection in the treatment of lower eyelid retraction: blind objective evaluation of surgical outcomes along with a patient assessment of both functional and aesthetic improvement one year after surgery confirm its efficacy and reliability along with the first case/control outcome evaluation of the technique carried on in the sub-population of 29 patients affected by unilateral lower eyelid retraction that validate the average improvement of the retracted eyelid one year after fat grafting as statistically significant. Nevertheless, longer follow-up periods and a larger sample size are needed to thoroughly confirm surgical outcomes and statistical results.

    Abstract Parkinson's disease (PD) stands as the most prevalent degenerative movement disorder, marked by the degeneration of dopaminergic neurons in the substantia nigra of the midbrain. In this study, we conducted a transcriptome analysis utilizing post mortem mRNA extracted from the substantia nigra of both PD patients and healthy control (CTRL) individuals. Specifically, we acquired eight samples from individuals with PD and six samples from CTRL individuals, with no discernible pathology detected in the latter group. RNA sequencing was conducted using the TapeStation 4200 system from Agilent Technologies. A total of 16,148 transcripts were identified, with 92 mRNAs displaying differential expression between the PD and control groups. Specifically, 33 mRNAs were significantly up-regulated, while 59 mRNAs were down-regulated in PD compared to the controls.     Abstract The complete removal of perfluorocarbon liquid (PFCL) after complex vitreoretinal surgery can be a challenging task. However, it remains a crucial step to avoid several ocular complications associated with retained PFCL. In this report, the authors describe a simple and effective technique to highlight the presence of retained PFCL bubbles in the epiretinal space through negative staining with vital dyes.

    Abstract Workplace health promotion programs and services offered by insurers may play a fundamental role to foster health/well-being and to prevent chronic diseases. To this end, they should be tailored to companies/employees’ requirements and characteristics. In particular, age needs to be taken into account, considering both that young age workers are generally healthy, and that young age is the best period in lifespan to address prevention and instilling healthy behaviors. We employed an anonymous, simple web-based questionnaire (filled out by 1305 employees) which furnishes data regarding lifestyle (nutrition, exercise, smoking, stress, sleep, etc.), some of which were used to build a unique descriptor (Lifestyle Index; 0−100 higher scores being healthier). We considered three subgroups accordingly to age: ≤30; between 30 and 50; >50 years. This study showed age influences lifestyle and stress perception in the working population: the youngest employees (both men and women) presented the worst lifestyle index, particularly in its stress component. This observation may potentially be useful to tailor workplace health promotion programs and to personalize insurance protocols and services offered to employees. The practical message of our study is that in healthy young people focusing only on medical parameters (frequently within normal ranges in this cohort), albeit important, may be not sufficient to foster proactive actions to prevent chronic non-communicable diseases in adult life. Vice versa, driving their attention on current behaviors might elicit their proactive role to improve lifestyle, getting immediate advantages such as well-being improvement and the possibility to best manage stress.

    Abstract As facial aging occurs, aesthetic changes of the nasal tip and the senile upper lip include tip ptosis, increase in skin length, loss of vermillion height and Cupid's bow, and vertical wrinkles.Regardless of the rejuvenation procedure, the lips are not to be considered as a single anatomical entity, instead they should be placed in a strong correlation with the nose. The aim of this study is therefore to demonstrate the effectiveness of the association of primary closed rhinoplasty together with indirect subnasal lip-lifting technique.A total of 45 patients were enrolled in this study and underwent primary closed rhinoplasty with indirect subnasal lip-lifting. The result showed an overall average nasolabial angle width reduction of 10.9% and lip length shortening of 23.5%, as assessed 1 year after the surgical procedure. When comparing before and after photographs and using the Subjective Global Aesthetic Improvement Scale (sGAIS), the overall satisfaction was rated 4.4 on 5, as "much improved."The authors show that lips should not be considered as a single anatomical entity but that they indeed must be placed in a strong correlation with the nose since a droopy nasal tip is likely to be considered unattractive and is often associated with the appearance of a long nose with covered philtrum and upper lip.Tip rotation to a normal nasolabial angle width could grant a beneficial impact on overall nose aesthetics; however, it may not fully balance the midface profile as it is the "lip position-to-incisal show" ratio that defines beauty, whereas the balance among forehead, nasal tip, upper lip, and chin is what defines the contour of harmonious profiles.This is the first patient series to show that the combination of closed rhinoplasty and indirect lip-lifting is a highly effective, safe, and reliable procedure to address profile rejuvenation.

    Abstract Purpose: To describe the development and management of ocular decompression retinopathy after cataract surgery in a patient with chronic severe HLA-B27 associated anterior uveitis.
    Methods: Case report.
    Results: A 45-year-old woman affected by ankylosing spondylitis and HLA-B27 associated anterior uveitis was referred for left eye pain and inflammation one week after phacoemulsification and intraocular lens implantation. She had a history of anterior uveitis complicated by iris bombé for posterior synechiae and cataract. Intraocular pressure (IOP) prior surgery was 14 mmHg. Clinical examination after cataract surgery showed intense inflammation in the anterior chamber and marked hypotony. Fundus examination revealed the presence of ocular decompression retinopathy. High-dose corticosteroids were supplemented to control the uveitis, with a progressive IOP increase and resolution of retinal hemorrhages over the following months.
    Conclusion: Ocular decompression retinopathy may present after cataract surgery in patients with complicated HLA-B27 associated anterior uveitis who develop severe post-surgical hypotony and inflammation. Close monitoring of IOP and intraocular inflammation are warranted to prevent severe complications after cataract surgery in these patients.

    Abstract O aleitamento materno contém todos os nutrientes necessários para o recém-nascido e deve ser o único alimento oferecido para a criança até os seis meses de vida, segundo recomendações do Ministério da Saúde (MS), tendo em vista as vantagens proporcionadas por esta prática. Este estudo teve por objetivo identificar, na literatura científica, publicações sobre as orientações que são fornecidas às mães durante a alta hospitalar pelos profissionais enfermeiros. Trata-se de uma pesquisa descritiva, de revisão de literatura, do tipo integrativa, segundo Cooper. A coleta de dados ocorreu nos meses de fevereiro a abril de 2020, norteada pela seguinte questão: a orientação sobre o aleitamento materno na alta hospitalar está acontecendo e de que forma acontece? Os dados foram analisados por temas conforme proposto por Bardin. Os principais resultados demonstraram que a maioria das puérperas não estão recebendo as orientações como é preconizado pelo MS e, quando recebidas, não são suficientes para atender as demandas apresentadas. Concluímos que é importante a participação da enfermagem no parto e na alta hospitalar oferecendo o apoio necessário para a puérpera com empatia e serviços qualificados, minimizando, desta forma, o desmame precoce.

    Abstract Purpose: To report a case of fungal endogenous endophthalmitis from Cladosporium sphaerospermum in a patient with juvenile idiopathic arthritis receiving chronic immunosuppressive therapy with golimumab (tumor necrosis factor-α blocker).
    Methods: Case report and review of the literature.
    Results: A 34-year-old woman receiving chronic immunosuppressive therapy with golimumab for juvenile idiopathic arthritis was referred for unilateral visual loss and ocular pain. Worsening conditions after corticosteroid therapy and raised serum beta-D-glucan levels pointed to an infectious fungal etiology. Panfungal polymerase chain reaction-based genetic sequencing on vitreous specimens obtained during vitrectomy detected C. sphaerospermum. The patient management combined surgical treatment and systemic and intravitreal voriconazole.
    Conclusion: Endogenous fungal endophthalmitis can be a rare complication in patients undergoing chronic immunosuppressive therapy (including golimumab) without other predisposing factors. Prompt diagnosis and appropriate treatment are the keys to preserve vision.
