    Abstract During the COVID-19 crisis, local epidemiology units have been forced to manage an increasing number of cases, contacts and outbreaks for which they were not previously prepared or staffed. Under normal circumstances, the efficient study and control of outbreaks and public health alerts requires human and material resources, situation analysis to identify possible causes and present recommendations, coordination with other health system structures, as well as the providing a written report including control and prevention measures implemented and their results or other recommendations. The field study of outbreaks has been systematized in a way that has made it possible to implement quick and effective measures, for the interruption of transmission chains and management of risk situations. To achieve this, a rapid and extraordinary exercise of digital integration, staff mobilization and creation of ad hoc structures needed to be carried out. Solidarity and cooperation between professionals from different administrations has been the pillar that has sustained the surveillance systems. Despite this, obstacles have been found as a result of the lack of coordination, social inequalities, and later, pandemic fatigue, which have reduced adherence and effectiveness of the implemented measures. It has also come to light the need to properly staff these units with trained professionals, and match working conditions and salaries with other health services.

    Abstract Resumen: Durante la crisis por la COVID-19, las unidades periféricas de epidemiología se han visto obligadas a gestionar un volumen de casos, contactos y brotes para el que no estaban dimensionadas ni dotadas. En situaciones ordinarias, el estudio y el control eficiente de los brotes y de las alertas requieren recursos humanos y materiales, análisis de la situación para identificar causas y emitir recomendaciones, coordinación con otras estructuras del sistema sanitario, así como la devolución de los resultados y las actuaciones en un informe y una propuesta de actuaciones. El estudio de brotes sobre el terreno ha evolucionado, alcanzando un grado de sistematización que ha permitido implementar de forma generalizada medidas rápidas y eficaces, para la interrupción de las cadenas de transmisión y la gestión de las situaciones de riesgo. Para conseguirlo, se ha tenido que realizar un ejercicio rápido y extraordinario de integración informática, de movilización de personal y de creación de estructuras ad hoc. La solidaridad y la cooperación entre profesionales de distintas Administraciones ha sido el pilar que ha sostenido el sistema de vigilancia. A pesar de ello, se han detectado obstáculos fruto de la falta de coordinación, las desigualdades sociales en salud y, posteriormente, la fatiga pandémica, que han disminuido la adherencia y la efectividad de las medidas. A su vez, se ha puesto de manifiesto la necesidad de redimensionar estas unidades, dotarlas de profesionales con formación y equiparar las condiciones de trabajo y de remuneración con los servicios de salud.     Abstract Se presenta el caso de un varón de 75 años a quien se realiza una coronariografía por disnea de nueva aparición y bloqueo de rama izquierda con lesión larga, difusa y fuertemente calcificada y estenosis máxima del 90% en coronaria derecha dominante. Se le sometió a una intervención coronaria percutánea (ICP) compleja (técnica de doble guía, ambas hidrofílicas, catéter guía telescopado y dilataciones con balones distensibles y semidistensibles) y se implantaron con éxito 3 stents farmacoactivos en sentido distal a proximal. A las pocas horas, el paciente empezó a quejarse de dolor torácico pleurítico, aunque seguía hemodinámicamente estable y con una exploración física normal. Las pruebas de laboratorio mostraron niveles de troponina I de 8 ng/ml (valores de referencia < 0,012 ng/ml). El ecocardiograma no mostró signos de movimiento regional anómalo, pero sí la presencia de una masa en la aurícula izquierda de 55,3 × 29 mm emergiendo de la pared auricular posterior que casi taponaba por completo la cavidad auricular sin disfunción protésica mitral asociada ni flujo transmitral comprometido. También se confirmó la presencia de derrame pericárdico sugestivo de hemopericardio (figura 1A; vídeo 1 del material adicional). Ante la sospecha de hematoma intramural auricular izquierdo (HIAI) se realizó una.

    Abstract Twenty-six triazole-based derivatives were designed for targeting both PD-L1 (programmed death receptor ligand 1) and VEGFR-2 (vascular endothelial growth factor receptor 2). These compounds were synthetized and biologically evaluated as multitarget inhibitors of VEGFR-2, PD-L1 and c-Myc proteins. The antiproliferative activity of these molecules on several tumor cell lines (HT-29, A-549, and MCF-7) and on the non-tumor cell line HEK-293 was determined.     Abstract Twenty-six triazole-based derivatives were designed for targeting both PD-L1 (programmed death receptor ligand 1) and VEGFR-2 (vascular endothelial growth factor receptor 2). These compounds were synthetized and biologically evaluated as multitarget inhibitors of VEGFR-2, PD-L1 and c-Myc proteins. The antiproliferative activity of these molecules on several tumor cell lines (HT-29, A-549, and MCF-7) and on the non-tumor cell line HEK-293 was determined. The effects on the abovementioned biological targets were evaluated for some selected compounds. Compound 23 , bearing a p -chlorophenyl group, showed better results than sorafenib in regard to the downregulation of VEGFR-2 and a similar effect to BMS-8 on both PD-L1 and c-Myc proteins. The antiangiogenic and antivascular activities of chloro derivatives were also established by endothelial microtube formation assay on Matrigel ® .

    Abstract Background: The presence of pathologic Q waves on admission electrocardiogram (ECG) in patients with anterior ST-elevated myocardial infarction (STEMI) has been related to adverse cardiac outcomes. Our study evaluates the prognostic value of QRS complex and Q waves in patients with STEMI undergoing percutaneous coronary intervention.
    Methods: We prospectively analyzed the specific characteristics of QRS complex and pathologic Q waves on admission and on discharge ECG in 144 patients hospitalized for anterior STEMI. We correlated these findings with the development of left ventricular systolic dysfunction (LVSD), appearance of heart failure (HF) or death during follow-up, and levels of several biomarkers obtained 6 months after the index event.
    Results: Multivariate logistic regression analysis showed that QRS width (odds ratios [OR] 1.05, p = .001) on admission ECG and the sum of Q-wave depth (OR 1.06, p = .002) on discharge ECG were independent predictors of LVSD development. Moreover, QRS width on admission ECG was related to an increased risk of HF or death (OR 1.03, p = .026). Regarding biomarkers, QRS width on admission ECG revealed a statistically significant relationship with the levels of NT-pro-BNP at 6 months (0.29, p = .004); the sum of Q-wave depth (0.27, p = .012) and width (0.25, p = .021) on admission ECG was related to the higher levels of hs-cTnI; the sum of the voltages in precordial leads both on admission ECG (-0.26, p = .011) and discharge ECG (0.24, p = .046) was related to the lower levels of parathormone.
    Conclusions: Assessment of QRS complex width and pathologic Q waves on admission and discharge ECGs aids in predicting long-term prognosis in patients with STEMI.

    Abstract Our purpose was to assess a possible association of inflammatory, lipid and mineral metabolism biomarkers with coronary artery ectasia (CAE) and to determine a possible association of this with acute atherotrombotic events (AAT). We studied 270 patients who underwent coronary angiography during an acute coronary syndrome 6 months before. Plasma levels of several biomarkers were assessed, and patients were followed during a median of 5.35 (3.88-6.65) years. Two interventional cardiologists reviewed the coronary angiograms, diagnosing CAE according to previously published criteria in 23 patients (8.5%). Multivariate binary logistic regression analysis was used to search for independent predictors of CAE.     Abstract Our purpose was to assess a possible association of inflammatory, lipid and mineral metabolism biomarkers with coronary artery ectasia (CAE) and to determine a possible association of this with acute atherotrombotic events (AAT). We studied 270 patients who underwent coronary angiography during an acute coronary syndrome 6 months before. Plasma levels of several biomarkers were assessed, and patients were followed during a median of 5.35 (3.88–6.65) years. Two interventional cardiologists reviewed the coronary angiograms, diagnosing CAE according to previously published criteria in 23 patients (8.5%). Multivariate binary logistic regression analysis was used to search for independent predictors of CAE. Multivariate analysis revealed that, aside from gender and a diagnosis of dyslipidemia, only monocyte chemoattractant protein-1 (MCP-1) (OR = 2.25, 95%CI = (1.35–3.76) for each increase of 100 pg/mL, p = 0.001) was independent predictor of CAE, whereas mineral metabolism markers or proprotein convertase subtilisin/kexin type 9 were not. Moreover, CAE was a strong predictor of AAT during follow-up after adjustment for other clinically relevant variables (HR = 2.67, 95%CI = (1.22–5.82), p = 0.013). This is the first report showing that MCP-1 is an independent predictor of CAE, suggesting that CAE and coronary artery disease may share pathogenic mechanisms. Furthermore, CAE was associated with an increased incidence of AAT.

    Abstract No clinical risk score is universally accepted for coronary artery disease. In 603 patients (mean age, 61.2 ± 12.3 yr) with stable coronary artery disease, we investigated the predictive power of clinical risk scores derived from the Framingham, the Long-term Intervention with Pravastatin in Ischemic Disease (LIPID), and the Vienna and Ludwigshafen Coronary Artery Disease (VILCAD) studies. Secondary outcomes were the recurrence of an acute thrombotic event (coronary events, strokes, or transient ischemic attacks), or heart failure or death. The primary outcome was the combination of secondary outcomes. During follow-up (duration, 2.08 ± 0.97 yr), 42 patients had an acute thrombotic event; 22, heart failure or death; and 60, the primary outcome.     Abstract We here report the synthesis of several stilbene derivatives. They show a measurable inhibitory effect on angiogenesis, some of them to a higher degree than resveratrol. Test methods included cell proliferation and tube formation assays using bovine aorta endothelial cells. In addition, it has been confirmed through the reverse transcriptase/polymerase chain reaction experiment that these stilbene derivatives downregulate the expression of the gene related to the production of the angiogenesis factor VEGF in cancer cells.
