    Abstract Although breed-specific vertebral heart size (VHS) reference ranges have been reported, the relationship between VHS and severity of cardiac enlargement has not been clarified. The objective was to assess the influence of cardiac enlargement on VHS in Chihuahuas with myxomatous mitral valve disease (MMVD). Ten clinically normal Chihuahuas (Normal) and 97 Chihuahuas with MMVD were recruited. Chihuahuas with MMVD were classified according to the values of left atrium to aorta ratio (LA/AO) and left ventricular internal dimension in diastole normalized (LVIDDN). These dogs were allocated into 3 groups: LA1 (LA/AO < 1.6), LA2 (1.6 ≤ LA/AO < 2.0), LA3 (LA/AO ≥ 2.0), and into 2 groups: LV1 (LVIDDN < 1.7), LV2 (LVIDDN ≥ 1.7). Vertebral heart sizes, measured as mean ± SD, were compared among groups. Optimal cutoff values of VHS were determined for mild (LA/AO ≥ 1.6, LVIDDN ≥ 1.7) and severe (LA/AO ≥ 2.0, LVIDDN ≥ 1.7) cardiac enlargement. Vertebral heart sizes (mean ± SD) were Normal: 9.66 ± 0.36, LA1: 10.13 ± 0.64, LA2: 10.87 ± 0.71, LA3: 11.71 ± 0.78, LV1: 10.04 ± 0.71, LV2: 11.21 ± 0.78. LA2-3 had significantly greater VHS than Normal and LA1, whereas LA3 had the greatest VHS. LV2 had significantly greater VHS than Normal and LV1 and a VHS of 10.5 and 11.1 had optimal diagnostic accuracy for identifying mild and severe cardiac enlargement, respectively. In conclusion, VHS increased according to cardiac enlargement in Chihuahuas with MMVD; a VHS of 10.5 and 11.1 might be useful in evaluating the extent of cardiac enlargement.

    Abstract Nucleic acid therapeutics have been attracting attention as novel drug discovery modalities for intractable diseases, including amyotrophic lateral sclerosis. This review provides an overview of the current status and prospects of antisense oligonucleotide treatment for amyotrophic lateral sclerosis. Recently, the results of a phase I/II study using the antisense oligonucleotides Tofersen to treat familial amyotrophic lateral sclerosis with superoxide dismutase 1 mutation have been reported. Intrathecal Tofersen administration resulted in a 36% reduction in superoxide dismutase 1 level in the cerebrospinal fluid. Another report described 2 patients with mutant superoxide dismutase 1 treated with an adeno-associated virus encoding a microRNA targeting superoxide dismutase 1. The first patient, who possessed the fast progressive mutant A5V, received a single intrathecal infusion. Although the patient died of respiratory arrest 16 months after treatment, autopsy findings showed a reduction of >90% in superoxide dismutase 1 level in the spinal cord. Clinical trials on antisense oligonucleotide therapies targeting other major amyotrophic lateral sclerosis-causative genes, fused in sarcoma and chromosome 9 open reading frame 72, are ongoing. To attenuate the pathology of TDP-43, strategies targeting regulators of TDP-43 (ataxin 2) and proteins downstream of TDP-43 (stathmin 2) by antisense oligonucleotides are being developed. The advent of nucleic acid therapeutics has enabled to specifically attack the molecules in the amyotrophic lateral sclerosis pathological cascade, expanding the options for therapeutic targets.     Abstract Double aortic arch is an embryological abnormality of the aortic arch forming a vascular ring. It has been noted that the right recurrent nerve travels differently in patients with a duplicated aortic arch and may be in close proximity to the area of superior mediastinal lymph node dissection in lung cancer. We report a surgical case of a patient with right middle lung cancer associated with a duplicated aortic arch. A 64-year-old man was referred to our hospital because of a nodular shadow in the right lung field noted on chest X-ray during a medical checkup. A transbronchial needle biopsy revealed a diagnosis of adenocarcinoma, and right middle lobe resection and lymph node dissection were performed. When dissecting the superior mediastinal lymph nodes in a patient with an overlapping aortic arch, it was necessary to carefully perform the operation, paying attention to the running of the right recurrent nerve.

    Abstract Vonoprazan (VPZ) has been available in Japan since 2015. Endoscopic features of proton-pump inhibitor (PPI)-related gastric mucosal changes, including fundic gland and hyperplastic polyps, have been observed. However, the relationship between gastric polyps and VPZ remains unclear. A 65-year-old man with reflux esophagitis-associated symptoms refractory to PPI was referred to our hospital. VPZ (20 mg) was administered for 3 weeks, which proved effective. Afterward, VPZ dose was reduced to 10 mg; the reflux symptoms worsened, and 20 mg VPZ was restarted. Afterward, esophagogastroduodenoscopy (EGD) revealed a gradually enlarging gastric polyp in the cardia. After 5 years of VPZ administration, the patient developed a reddish polyp (approximately 10 mm) with a whitish substance in the cardia. Based on the clinical course, the polyp was considered to have enlarged because of the long-term VPZ administration. After being informed of the endoscopic findings, the patient decided to discontinue VPZ. One year after VPZ discontinuation, EGD revealed a shrunken polyp (5 mm). Long-term acid suppression causes hypergastrinemia, which may lead to gastric mucosal changes, including gastric polyps. There are few case reports of a decrease in the number and size of gastric polyps after VPZ discontinuation. Hence, some VPZ-induced endoscopic changes may be reversible.
