    Abstract Aim: Caloric vestibular stimulation (CVS) induces the activation of several cortical structures and has been utilized as a neuromodulation technique in the treatment of several disorders. This study purposed to investigate whether CVS would be beneficial in the management of idiopathic tinnitus. Methods: Patients with unilateral idiopathic tinnitus were enrolled in the study. CVS with cold water (study group) and body temperature (control group) irrigation were applied to the symptomatic ear. All patients underwent a standard audiometric examination and visual analogue scale (VAS), the tinnitus handicap inventory (THI) and the Beck depression inventory (BDI) were administered. Results: VAS intensity and disturbance values, mean THI and BDI scores did not show any significant changes in the control group.     Abstract Aim:Leptomeningeal metastasis (LM), which can be seen in all types of cancer, is a rare condition with poor prognosis. Diagnosis and treatment are important components of the management of the disease. We aimed to reveal clues for early diagnosis by examining the clinical and laboratory features of LM.Methods:Sixteen patients who received the diagnosis of LM between 2018 and 2020 were included in the study retrospectively. The cerebrospinal fluid (CSF) samples and cranial magnetic resonance imaging of the patients were examined. In addition, the patients were divided into two groups according to the presence of papilledema.Results:The mean age of the patients was 49 (19-73) and 37.5% were women. The most common histological tumor type was adenocarcinoma. The most common clinical finding was multiple cranial neuropathy. The mean time to the diagnosis of LM was 20 months. Eleven of the 16 patients had solid tumors, five had hematological malignancies. Nine patients had papilledema. Headache was significantly more common in the group with papilledema (p=0.01). The average lifespan after the diagnosis of LM was 8 months (1-48 months). Malignant cells were detected in the CSF cytology of four patients (30%).Conclusion:LM is a condition with poor prognosis and difficult to diagnose in oncology patients, despite all tests being performed under appropriate conditions. Presence of papilledema is thought to contribute to the diagnosis since the evaluation of intracranial in LM patients is a non-invasive evaluation method.

    Abstract Objectives:Hypoxic encephalopathy (HE) after cardiac arrest is a clinical condition that can cause severe neurological sequelae and death. It is important to determine the prognosis in patients with HE for follow up and developing future treatments in the long term. Our aim was to investigate the effects of age, gender, hypothermia, lesion site in brain magnetic resonance imaging (MRI), seizure and electroencephalography (EEG) findings on prognosis in patients with the diagnosis of HE.Materials and Methods:The data of the adult intensive care unit was evaluated retrospectively. A total of 194 patients with HE were included into the study. One hundred and eighteen (60.8%) of the patients were male and 76 (39.2%) were female. The survivors and those who died were identified. Hypothermia therapy, brain MRI findings (cortical and/or subcortical lesion), presence of seizures (myoclonic, generalized, focal) and EEG findings (epileptic activity, status, generalized slowing) were recorded.Results:The mean age was 59.46±1.71 (18-89). Seventy-six (39.2%) patients underwent hypothermia. One hundred and seven (55.2%) patients died. The rate of exitus in elderly patients was significantly higher (p<0.01). The brain MRI of 88 patients showed hypoxic lesions (cortical in 8 patients, subcortical in 21 patients, cortical + subcortical lesion in 59 patients). The survival rate of the patients with cortical or subcortical pathology is significantly higher than patients with both pathology (p<0.01). As a result of multiple regression analysis, a 1-year increase in age, increased the risk of death by 4%, while the presence of pathological findings in both areas in brain MRI increased the risk of death by 2.62 times. There was no significant difference of gender, hypothermia, seizures and pathological EEG findings on survival (p>0.05).Conclusion:Approximately half of the patients included in our study died. The survival rate of the elderly patients and the patients with multiple lesions in brain MRI was low.

    Abstract Aim:It has been shown that motor symptoms, as well as non-motor symptoms, significantly affect the quality of life of patients with idiopathic Parkinson’s disease. In our study, the association of apathy, one of the NMS, with motor and non-motor symptoms was investigated.Methods:Patients with stage 1-3 IPH according to the Hoehn-Yahr Scale (H&Y) were included in the study. Patients who received dementia diagnosis according to the DSM IV criteria were excluded from the study. The patients were evaluated using the Unified Parkinson’s Disease Rating Scale (UPDRS), non-motor symptoms Questionnaire (NMSQ), Geriatric Depression Scale (GDS), and the revised diagnostic criteria for apathy.Results:Apathy was found in 26 of 68 patients who were included in the study. Patients with and without apathy were compared in terms of age, clinical and demographic characteristics, L-dopa equivalent doses, depression, and NMSQ scores. The group of patients with apathy consisted of nine females and 17 males and the mean age was 64.3±7.7 years. The non-apathy group included 25 females and 17 males and the mean age was 66.8±9.1 years. There was no statistically significant difference between the groups in terms of age, education, age at onset, duration of illness and L-dopa equivalent doses. The male-to-female ratio was significantly higher in the apathy group. The mean GDS and NMSQ scores in the apathy group were significantly higher than in the non-apathy group. There was a statistically significant difference between the groups in H&Y stage and total UPDRS and subscales scores. It was noteworthy that the rate of stage 3 patients was higher in the group of patients with apathy. The number of patients having bradykinesia in the apathy group and the number of patients having tremor in the group without apathy was higher.Conclusion:Our study showed that apathy, one of the non-motor symptoms, was more prominent in male gender, associated with UPDRS scores, clinical stage and bradykinesia, and accompanied by depression.

    Abstract Aim: The purpose of this study is to prepare the Turkish version of the International Restless Legs Syndrome (RLS) Study Group Rating Scale and to investigate its validity and reliability. Materials and Methods: A total of 89 individuals with RLS were included. These subjects were divided into two groups: A pilot study group (n=20) and a study group (n=69). The scale was translated into and adjusted for Turkish was applied to the groups. Results: In the analysis conducted to test the factor structure of the scale, it was clearly seen that the items were collected under two factors and that the two factors in total accounted for 68.19% of the variance. The Kaiser-Meyer-Olkin (Kaiser-Meyer-Olkin) coefficient for validity of the scale was 0.84 and Bartlett Sphericity index was significant with p <0.005. The Cronbach's Alpha coefficient, which determined the reliability of the scale, was 0.0896. Conclusions: This study demonstrated that the Turkish version of this scale that is frequently used for scientific and clinical trials is valid.

    Abstract Objective: Leptin and adiponectin are the most important adipocytokines and recent studies showed that they may have role in the etiology of ischemic stroke. However, the effect of adipocytokines on stroke is not yet well understood, and related studies are confusing. The aim of this study was to investigate the effects of serum leptin and adiponectin levels on patients with acute ischemic stroke who were admitted to hospital within 24 hours of stroke. Materials and Methods: A total of 59 patients with acute ischemic stroke and 24 healthy individuals similar in age and sex to the patients were enrolled in the study. The Rankin disability scale was performed in order to detect the severity of stroke. Levels of serum leptin and adiponectin were detected using an enzyme-linked immunosorbent assay. In addition, the outcomes of the patients were calculated through differences between the first day Rankin score and the 30th day Rankin score. Results were compared and the relation with prognosis was investigated. The correlation of leptin and adiponectin levels with stroke severity was analyzed. Results: Thirty-four male and 25 female patients were included in this study. Leptin levels of patients were higher than in the control group. Adiponectin levels did not differ between the patient and control groups. There was no correlation between serum adipocytokine levels and the modified Rankin scale. Conclusion: Serum leptin and adiponectin levels are not associated with the prognosis of ischemic stroke. Even though high levels of leptin may associate with ischemic stroke, this study was not able to show an association with stroke prognosis

    Abstract Background: Establishing a relationship between COVID-19 severity and olfactory dysfunction may be beneficial in patient follow-up. Thus, in this study, we aimed to evaluate the association between self-reported olfactory dysfunction and the clinical stages of COVID-19. Methods: The patients included in this study were divided into three groups according to the severity of the novel coronavirus disease as mild, severe, and critical (life-threatening) patients. Patients were then contacted by phone and asked questions with the help of structured questionnaires that evaluated their general status, sense of smell, taste and compared the data within the three groups. Results: Among the 144 subjects evaluated in the present study (mild, n = 60; moderate, n = 61, severe, n = 23), 70 of the participants were males, and 74 were females. The findings showed that olfactory loss was the most prominent feature of the COVID-19's mild clinical course and the majority of the patients with loss of smell were female and young patients. Conclusion: The findings obtained from clinically mild cases suggest that more olfactory dysfunction, indicating that the effects of viral load alone, is not decisive for olfactory dysfunction.

    Abstract Aim. Autonomic symptoms (AS) are one of the major causes affecting quality of life in Parkinson’s disease (PD) patients and contribute to the disease burden. Our aim is to investigate the occurence of AS and to evaluate the relation between AS and the demographic and clinical features of PD patients. Method. In this study 66 idiopathic PD patients (47 men) were evaluated for the presence of AS during the last month with the SCOPA-AUT (Scales for Outcomes in Parkinson’s Disease; SCOPA) questionnaire, which consists of 23 items in six domains: gastrointestinal (7), urinary (6), cardiovascular (3), thermoregulatory (4), pupillomotor (1), and sexual (2). The maximum score is 69, with a score range per item from 0 (never experiencing the symptom) to 3 (often experiencing the symptom). Associations between AS and duration of disease, age onset of disease, treatment and Hoehn &Yahr stage scores were also studied. Results. The mean (SD) age was 67.1 (9.5), disease duration 5.1 (3.9), age onset of disease 59 (10.9) years and Hoehn &Yahr median stage 1.9 (0.75, range I-IV). The presence of AS in all patients was observed. Mean SCOPA-AUT score was 15.6 (4-32). Total SCOPA-AUT scores were found significantly associated with duration of disease and Hoehn&Yahr stages (p<0.05), in contrast to age and gender. The most leading AS were in the gastorintestinal domain which was associated with the disease severity and duration also (p<0.05). Conclusion. AS were present in all patients even at the disease onset and early stages and increased with the disease severity and duration. For a better quality of life of the patient, it’ s important to evaluate the presence of AS and initiate the appropriate treatment.

    Abstract Pathological laughing and crying (PLC) has an approximately prevelance of 7%–10% among patients with multiple sclerosis (MS). Various damages of prefrontal, temporal cortex, internal capsule, hypothalamus, thalamus, brainstem and the cerebellar pathways may cause pathological laughing or crying. We interviewed 160 MS patients for PLC. Only 5 of our 160 cases complained about pathological laughing or crying. All 5 patients had PLC defined as sudden, involuntary displays of laughing or crying or both, without associated subjective feelings of depression or euphoria. Patient group, consisting 2 women and 3 men had a mean age of 41, mean Expanded Disability Status Scale (EDSS) score of 4.8, disease duration of 12.8 years. In our patients PLC was not associated with disease exacerbations. All PLC patients had numerous brainstem and cerebellar relapses, which caused obvious disabilities including gait ataxia, intention tremor, dysmetria, dysarthria, nystagmus and other cerebellar dysfunctions. All of our patients had so many lesions especially in brainstem because of this we think that the brainstem lesions have crucial role in PLC. This hypothesis needs further investigation especially in functional imaging which may add further information to pathogenesis of PLC. Before the investigation of PLC none of our patients informed us about their complaints of PLC so the physician must be aware of this symptom.
