    Abstract Objective of the study: estimate the prevalence of magnesium deficiency (MD) in women using hormonal contraception (HC), who are enrolled in the MAGYN2 study, determine the clinical features of MD and the efficacy of Magne B6 Forte in replenishing magnesium deficiency and improving the patients’ quality of life.Material and methods: 1,904 women using HC were examined. At stage I,the prevalence of DMwas assessed on the basis of the MDQ questionnaire and/or serum magnesium levels were determined. Then the patients were divided into two groups: I – without magnesium deficiency (n = 996) and II – with identified MD (n = 908) during the study. The following parameters were determined: clinical symptomatology caused by MD, and the women’s quality of life using the WHOQOL-BREF questionnaire. At stage II, patients with MD received Magne B6 Forte for 4 weeks with an assessment of its efficacy. Results: The prevalence of MD in the population of patients taking HA is 47.7%. The most severe manifestations of MD included irritation, increased fatigue, chronic stress state. The clinical efficacy of Magne B6 Forte was confirmed by increased serum magnesium levels (from 0.67 ± 0.09 to 0.79 ± 0.26 mmol/L, p< 0.001), normalization of the MDQ score (from 42.5 ± 11, 4 to 22.7 ± 14.9 points, p< 0.001), double reduction of severity of more than half the symptoms of MD, improved quality of life in the range of 20 to 32% in 5 domains of the WHOQOL-BREF questionnaire. Conclusion: The use of Magne B6 Forte in women using HC helps to reduce symptoms of MD and improve the quality of life.

    Abstract Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a virus that, having crossed species, has caused human disease from 2019 - COrona Virus Disease 2019 (COVID-19). Pregnant women are potentially at high risk of contracting SARS-CoV-2 infection when compared to non-pregnant matched controls. Pregnancy is also complicated with a higher risk of developing severe SARS-CoV-2, including respiratory diseases, admission to the intensive care unit and mortality, even after adjusting for confounding risk factors. Moreover, data on the effect on fetal outcome including preterm delivery and perinatal morbidity are still conflicting, the risk of vertical transmission (i.e., transmission of SARS-CoV-2 from the mother to the fetus or the newborn) is considered low but there is evidence that a significant proportion of placentas where SARS-CoV-2 occurred during pregnancy show histopathological findings suggesting placental hypoperfusion and inflammation. In this review we will present the available data on the effects of SARS-CoV-2 infection on fetal growth and maternal hemodynamics

    Abstract The article tells about major anticoagulant medications used in cancer patients taking into account the pathogenesis of hypercoagulation status in these patients: oral anticoagulants, heparin medications, selective factor Xa-inhibitors. The main advantages of low molecular weight heparins versus unfractionated heparin are described. Recommendations for the prevention of thrombotic complications in cancer patients are provided. Possible complications of heparin therapy are reviewed, and the prevention methods are suggested.

    Abstract Introduction. Disseminated intravascular coagulation (DIC) is an acquired syndrome characterized by intravascular activation of blood clotting without specific localization and arising from various causes. DIC is the most complex and high-priority problem in contemporary clinical medicine including obstetrics and perinatology. DIC significance is related to its peak perinatal mortality, as well as a large rate of complications during childbirth and postpartum period.Aim: to conduct comprehensive data assessment related to DIC pathogenesis and management in pregnant women and newborns.Materials and methods. A search for DIC-related publications within the past 10 years was conducted in international research databases: Scientific Electronic Library Online eLibrary, Google Scholar, ScienceDirect, Cochrane Library, PubMed/Medline. The data regarding DIC diagnostic criteria and markers, as well as current approach to its treatment are presented.Results. DIC may be considered as one of the causes resulting in massive obstetric bleeding. The majority of pregnancy-related complications is manifested as preeclampsia, premature detachment of normally situated placenta and anaphylactoid syndrome of pregnancy. Any massive bleeding should be perceived as hemorrhage resulting from the DIC progression.Conclusion. Knowledge of the DIC pathogenetic mechanisms is necessary for conducting a differential diagnosis and applying timely treatment.

    Abstract COVID-19 is an infectious disease caused by the beta coronavirus SARS-CoV-2, which became widespread worldwide in 2020. In most severe patients, the clinical picture begins with respiratory failure, then progressing to polyorgan failure. The most adverse prognostic sign is the development of coagulopathy. Based on the analysis of clinical data available to date, 71.4% of those who died and 0.6% of those who survived showed signs of explicit disseminated intravascular coagulation syndrome (DIC-syndrome). Monitoring of D-dimer, prothrombin time (PT), platelet and fibrinogen count are important for determining treatment and indications for hospitalization among patients with COVID-19. If these parameters deteriorate, more "aggressive" intensive care should be performed. Lowmolecular weight heparin (LMWH) should be administered to all patients with diagnosed COVID-19 infection (including non-critical patients) who require hospitalization if these patients have no contraindications for LMWH.

    Abstract The pandemic of a new coronavirus infection COVID-19 has become a real challenge to humanity and the medical community and has raised a number of medical and social issues. From the currently available information on clinical cases of COVID-19, it follows that COVID-19 patients in critical condition have a clinical picture of disseminated intravascular coagulation (DIC), septic shock with the development of multiple organ failure, which justifies the anticoagulant therapy use in patients with COVID-19. In addition to isolating virus DNA from biological material and diagnosing SARS-CoV-2, the use of simple and easily accessible laboratory markers in the blood is necessary for the management of patients with COVID. If the activation of coagulation processes is sufficient enough, the consumption of platelets and blood clotting factors can be diagnosed by laboratory methods in the form of prolongation of routine blood clotting tests and increasing thrombocytopenia. Hyperfibrinogenemia, increased D-dimer, prolonged prothrombin time, thrombocytopenia, lymphopenia, leukocytopenia, increased interleukin 6, and ferritin are observed in most COVID-19 patients. The degree of increase in these changes correlates with the severity of the inflammatory process and is a prognostically unfavorable sign. In the article authors discuss laboratory monitoring that plays an essential role in this crisis, contributing to patient screening, diagnosis, as well as monitoring, treatment and rehabilitation.

    Abstract The rapid spread of COVID-19 and the large number of cases put a significant burden on the health systems of any developed country Specialists in natural disasters and military medicine should be involved in the provision of medical care and observance of anti-epidemic measures In some countries, including Italy, they were involved only after the situation was dramatically worsening with many clinical units and hospitals overloaded by infected patients To curb the spread of COVID-19, most countries declared a state of emergency, and unprecedented measures have been taken to strengthen quarantine in suspected or positive symptomatic subjects Nevertheless, the crisis associated with the unexpectedly global scale and tragedy of the pandemic and the inconsistency of actions of both society and individuals and specialized medical services, lead to insufficient effectiveness of the measures taken in a number of regions In the present day, it is vital for every person to change its mindset ― relying on personal responsibility to comply with all recommendations of quarantine and anti-epidemic measures, and to reorganize departments and resources of medical institutions at all levels in order to withstand the spread of infection and at the same time provide all those in need with the necessary and appropriate medical care Particular attention should be paid to the obstetric care service, given that even in normal times, the obstetric hospital is an area of increased responsibility for the life and health of mother and child and future mankind Fulfillment of existing orders, instructions of national and regional committees, international and national protocols and clinical protocols should undoubtedly lead to a positive result, but this requires additional training of medical personnel at all levels The purpose of this review is to propose quick key strategies for reassessing the maternity and neonatal wards/ hospitals based on the experience of health systems and organizations which faced the spread of this new coronavirus;this advice may be applied along with binding tight instructions in obstetric hospitals in order to proactively respond to a likely wave of growth in COVID-19 ;Актуальность Быстрое распространение COVID-19 и большое количество заболевших накладывает значительную нагрузку на системы здравоохранения развитых государств Ð’ ряде стран к оказанию медицинской помощи и соблюдению противоэпидемических мероприятий привлекаются специалисты по стихийным бедствиям и военной медицине, однако в некоторых странах, в том числе в Италии, они были задействованы после того, как многие клинические подразделения и стационары были перегружены Для сдерживания распространения COVID-19 большинство стран объявили чрезвычайное положение, были приняты беспрецедентные меры по усилению карантинных мероприятий Тем не менее кризис, связанный с неожиданно глобальным масштабом и трагичностью пандемии, и несогласованность действий как общества и отдельных граждан, так и специализированных медицинских служб приводят к недостаточной эффективности проводимых мероприятий в ряде регионов Особое внимание следует уделить службе родовспоможения, учитывая, что и в спокойное время акушерский с ‚ационар является зоной повышенной ответственности за жизнь и здоровье матери и ребенка Исполнение действующих приказов, указаний национальных и региональных комитетов, международных/национальных и клинических протоколов, несомненно, приведет к положительному результату, но требует дополнительного обучения и тренировки медицинского персонала всех уровней Цель обзора ― предложить ключевые стратегии, основываясь на опыте регионов, ранее столкнувшихся с распространением нового коронавируса, которые могут быть быстро применены наряду с обязательными к исполнению действующими приказами и распоряжениями в акушерских стационарах, чтобы превентивно отреагировать на вероятную волну роста заболеваемости и носительства COVID-19
