Abstract Aim: To determine the efficacy of a combination of injectable-platelet-rich fibrin and type-1 collagen particles on the healing of through-and-through periapical bone defect and subsequent closure of bony window.
Methodology: The clinical trial was registered in ClinicalTrials.gov (NCT04391725). Thirty-eight individuals with radiographic evidence of periapical radiolucency in maxillary anterior teeth and confirmed loss of palatal cortical plates in cone beam computed tomographic imaging were randomly assigned to either the experimental group (n = 19) or the control group (n = 19). A mixture of i-PRF and collagen as a graft was applied to the defect in adjunct to periapical surgery in the experimental group. No guided bone regeneration procedures were used in the control group. The healing was evaluated using Molven's (2D) and modified PENN 3D (3D) criteria. Percentage reduction of the buccal and palatal bony window area, and complete closure of through-and-through periapical bony window (tunnel defect) were assessed using Radiant Diacom viewer software (Version 4.0.2). The reduction in the periapical lesion area and volume was measured using Corel DRAW and ITK Snap software.
Results: Thirty-four participants (18 and 16 in the experimental and control groups respectively) reported for follow-up at 12 months. There was 96.9% and 97.96% reduction of buccal bony window area in the experimental and control groups respectively. Similarly, palatal window showed 99.03% and 100% reduction in the experimental and control groups respectively. No significant difference in both buccal and palatal window reduction was noticed between the groups. A total of 14 cases (seven in the experimental group and seven in the control group) showed complete closure of through-and-through bony window. No significant difference in clinical, 2D and 3D radiographic healing, percentage reduction in area and volume was observed between the experimental and control groups (p > .05). Neither the area nor the volume of lesion, and the size of buccal or palatal window had significant effect on healing of through-and-through defects.
Conclusion: Endodontic microsurgery results in high success rate in large periapical lesions with through-and-through communication with more than 80% reduction in volume of lesion and size of both buccal and palatal window after 1 year. A mixture of type-1 collagen particles and i-PRF, adjunct to periapical micro-surgery did not improve the healing in through-and-through periapical defects.

Abstract Background: Chhattisgarh is one of the worst performing states of India in terms of maternal mortality ratio figures. Slums carry the unfortunate burden of maximum maternal deaths. Birth Preparedness and Complication Readiness(BPCR) advocates planning of birth and being ready for any obstetric complications. Aim: To evaluate the BPCR index among first and second trimester pregnant women and those delivered within 6 months living in urban slums of Raipur city. Methods: A population-based, cross-sectional descriptive study was conducted among 160 pregnant and 70 recently delivered women residing in urban slums of Raipur city selected by multistage random sampling using an interview method. BPCR index was calculated using Johns Hopkins Program for International Education in Gynaecology and Obstetrics guidelines. BPCR components were correlated for any association with sociodemographic variables of the participants using Z test. Results: BPCR index came out to be 34.1%, which was very low. Factors associated with ≥4 antenatal care visits were general category women (P = 0.0375) and homemakers (P = 0.0057). Identifying skilled provider was higher in educated women (P = 0.0001), those with educated husbands (P = 0.0085), working (P = 0.0167) and with higher socioeconomic status (P = 0.0013). Saving money for childbirth was more among general category (P = 0.0067), educated (P = 0.0094) and women with low socioeconomic status (P = 0.0004). Conclusions: BPCR index of the study area was found to be very low.

Abstract A reliable climate dataset is the backbone for modelling the essential processes of the water cycle and predicting future conditions. Although a number of gridded climate datasets are available for the North American content which provide reasonable estimates of climatic conditions in the region, there are inherent inconsistencies in these available climate datasets (e.g., spatially and temporally varying data accuracies, meteorological parameters, lengths of records, spatial coverage, temporal resolution, etc.). These inconsistencies raise questions as to which datasets are the most suitable for the study area and how to systematically combine these datasets to produce a reliable climate dataset for climate studies and hydrological modelling. This study suggests a framework called the REFerence Reliability Evaluation System (REFRES) that systematically ranks multiple climate datasets to generate a hybrid climate dataset for a region. To demonstrate the usefulness of the proposed framework, REFRES was applied to produce a historical hybrid climate dataset for the Athabasca River basin (ARB) in Alberta, Canada. A proxy validation was also conducted to prove the applicability of the generated hybrid climate datasets to hydrologic simulations. This study evaluated five climate datasets, including the station-based gridded climate datasets ANUSPLIN (Australia National University Spline), Alberta Township, and the Pacific Climate Impacts Consortium's (PCIC) PNWNAmet (PCIC NorthWest North America meteorological dataset), a multi-source gridded dataset (Canadian Precipitation Analysis; CaPA), and a reanalysis-based dataset (North American Regional Reanalysis; NARR). The results showed that the gridded climate interpolated from station data performed better than multi-source- and reanalysis-based climate datasets. For the Athabasca River basin, Township and ANUSPLIN were ranked first for precipitation and temperature, respectively. The proxy validation also confirmed the utility of hybrid climate datasets in hydrologic ...

Abstract Cytogenetic anomalies involving the 3q26 chromosomal region are rare in acute myeloid leukemia (AML). There is no such description of these anomalies from the Indian sub-continent. A total of 174 AML patients were admitted to our hospital for therapy between January 2001 and January 2008. Cytogenetic studies could be done in 115 patients; which revealed three cases with 3q26 anomalies. All were males. In the first two cases, the anomaly was detected in all the metaphases. The common features seen were the presence of only mild thrombocytopenia (relatively high platelet counts when assessed against the background of AML with high blast percentages), monosomy 7, myeloperoxidase positive blasts, mild eosinophilia, and poor therapeutic response. In the third case, the chromosome 3 anomaly was present in only one metaphase. Such an anomaly has not been reported. Only the third patient responded to induction therapy but subsequently relapsed after being in complete remission for 15 months. 3q26 anomalies are associated with monosomy 7, relatively higher platelet counts at diagnosis as compared with other non-3q rearranged AML's and poor prognosis. The precise mechanisms underlying leukemogenesis need to be elucidated and better treatments devised since these patients respond poorly to therapy.

Abstract Oral lichen planus (OLP), a chronic inflammatory disease that affects the oral mucosa, may cause pain or discomfort along with increased burning sensation. There are numerous treatment modalities available that have several side effects. The aim of this study was to determine the effectiveness of Ayurvedic management for oral Lichen Planus. Two sources of Internet were used to search appropriate articles which satisfied the purpose of study including PubMed and Google scholar. Studies which were done on Indian population, PubMed search including articles that were published between January 1, 2000 and October 31, 2019, Studies published in English or which could be translated to English, randomized clinical trials, controlled clinical trials and longitudinal studies with essential data on Ayurvedic approach were included. Patients having Oral Lichen Planus where ayurvedic approach was used for treatment. Out of 09 articles, included in systematic review, in almost all the studies improvement in pain, erythema and burning sensation were reported in score scale. Administration of topical herbal antioxidants and topical application of herbal medicines prove to be effective approach in minimizing the signs and symptoms of Oral Lichen Planus. Long-term use of steroids has many side effects while the herbal medicines almost have none. So, herbal medicines can be used for better efficacy with least side effects for OLP.

Abstract The population dynamics of two species that is governed by the deterministic Lotka-Volterra model concerned with the interaction of predator and prey is investigated in this article as an application of homotopy analysis method. The analytical approximate solution in the form of convergent infinite series is obtained by considering the time-fractional derivatives in the Caputo sense. The simulation of obtained results exhibit the effect of variation in fractional parameters, auxiliary parameters and auxiliary linear operators on the mass concentration of both the biological species which in turn affects the structure of the system.

Abstract Background: Preeclampsia is a condition that is characterized by hypertension and proteinuria occurring after 28 weeks of gestation. It complicates 5%–8% of all pregnancies. Lactate dehydrogenase (LDH) is an intracellular enzyme and its level is increased in preeclamptic women due to cellular death. Preeclampsia is a multisystem disorder and leads to a lot of cellular death. It carries substantial risks for both fetus and mother with a subsequent increase in the perinatal and maternal morbidity and mortality. Aim: The present study was planned to estimate and compare the serum LDH levels in women with preeclampsia and normal pregnant women and to correlate LDH levels with maternal and perinatal outcome in preeclampsia. Materials and Methods: This observational prospective study was conducted on 200 antenatal women. Women were divided into two groups, namely Group I (n = 100) comprised of women with preeclampsia (study group) and it was further subdivided into three categories on the basis of LDH levels: A (n = 53): <600 IU, B (n = 27): 600–800 IU, and C (n = 20): >800 IU and Group II (n = 100, control) comprised of normotensive pregnant women. LDH levels of both the groups were compared, and association of maternal and perinatal outcome was assessed in relation to LDH levels. Results: With increased severity of preeclampsia, rise in the LDH levels was observed. Severely preeclamptic women with LDH levels >800 IU/l showed a significant increase in incidence of eclampsia, abruption, hemolysis, elevated liver enzymes, and low platelet count syndrome, disseminated intravascular coagulation, and intensive care unit (ICU)/respiratory ICU transfer as compared to women who had lower levels <600 IU/l. A significant difference was observed among sepsis, mortality, and neonatal deaths between subgroups of preeclampsia according to the levels of LDH. Conclusion: LDH levels are associated with severity of preeclampsia and occurrence of maternal and fetal complications.
