    Abstract Aim:The neutrophil-lymphocyte ratio (NLR) is a parameter that has been shown to be effective as a prognostic marker in many solid tumors. It is aimed to investigate NLR and the systemic immune-inflammation index (SII) in predicting the overall survival and lymph node positivity in bladder cancer (BC).Methods:The retrospective study included patients that underwent radical cystoprostatectomy/radical cystectomy (RC) due to muscle-invasive bladder cancer (MIBC), high-grade T1 BC, or carcinoma in situ in our clinic between January 2010 and March 2020. All the patients had no history of preoperative metastasis, lymph nodes, chemotherapy, hematological malignancies, and preoperative urinary tract infection. Data on neutrophil, lymphocyte, platelet, and hemoglobin levels and total white blood cell counts were retrieved from clinical records and data on disease stage and lymph node positivity were retrieved from pathology reports. Follow-up times and survival times were recorded.Results:The 213 patients comprised 196 (92%) men and 17 (8%) women with a mean age of 63.17±11.25 years. Lymph node positivity was detected in 49 (23%) patients. The mean overall survival time was 75.04, 63.77, and 84.4 months in all patients, patients with lymph node positivity, and patients with lymph node negativity, respectively. No significant difference was found between patients with lymph node positivity and negativity with regard to NLR and SII values (p=0.975 and p=0.745, respectively). In the receiver operating characteristics (ROC) analysis NLR and SII had no significant effect in predicting lymph node positivity [Area under the ROC curve (AUC) 0.499 (95% confidence interval (CI): 0.403-0.594) and AUC 0.485 (95% CI: 0.394-0.575), respectively] and in predicting overall survival [AUC 0.423 (95% CI: 0.346-0.501, p=0.056) and AUC 0.435 (95% CI: 0.357-0.514, p=0.107), respectively].Conclusion:The results indicated that SII and NLR are not sufficient to predict lymph node positivity and survival in patients with organ-confined BC.

    Abstract Objective:Falls are a significant cause of morbidity and mortality, particularly in hospitalized patients. In this study, we aimed to evaluate the risk of falls in the patients hospitalized in a urology clinic.Materials and Methods:The study included patients that were hospitalized in a urology clinic between November and December 2018. All the patients were administered both Itaki Fall Risk scale (IFS) and Morse Fall scale (MFS). Demographic and clinical characteristics, history of falls within the past six months, type of surgery, primary diagnosis (urooncological vs non-urooncological), and the department/unit from which the patients were referred [outpatient clinic vs emergency service/intensive care unit (ICU)] were recorded.Results:The risk of falls was significantly higher in the patients with urooncological diseases compared to the patients with non-urooncological diseases (p<0.001) and in the patients referred from emergency service/ICU compared to the patients referred from outpatient clinics (p<0.001). Moreover, the patients with a history of falls within the past six months were found to be at higher risk of falls compared to the patients with no history of falls both on IFS and MFS (p=0.002, p=0.01, respectively).Conclusion:Hospitalized patients should be closely followed due to the risk of falls, particularly the cancer patients and the patients referred from the emergency service or ICU.

    Abstract Objective:Bacillus Calmette-Guérin (BCG) limited availability, cost, and allergic response in some patients limit its use, leading to the search for alternative agents. We aimed to present the outcomes of intermediate- and high-risk non-muscle invasive bladder cancers treated with bladder wall thermochemotherapy (BWT) system via a single-arm prospective study.Materials and Methods:Overall, 44 patients were enrolled and subjected to a treatment with mitomycin-C and BWT for 6 weeks, followed by a monthly maintenance for up to 1 year. The recurrence rate, progression rate, recurrence-free survival, progression-free survival, and side effects were evaluated.Results:In the intermediate- and high-risk groups, the median follow-up duration was 14 and 17 months; recurrence rates were 28.6% and 23.3%; progression rates were 7.1% and 23.3%; the recurrence free survival at the 24th month were 78.6% and 80.0% and the progression free survival at the 24th month were 92.6% and 76.7%, respectively. All the patients suffered from dysuria lasting for 2-3 days; five of the 442 BWT application developed allergic reactions requiring treatments with oral agents.Conclusion:BWT showed comparable recurrence and progression rates, but seemed safer than BCG when our findings were compared to those of published studies. It could be an alternative for individuals who are not suitable for BCG or in a case of BCG unavailability.

    Abstract Objective:Periprostatic nerve block (PPNB) and intrarectal local anesthesia (IRLA) are two of the most commonly used methods for pain control in the transrectal ultrasonography (TRUS)-guided prostate needle biopsy procedure, which is the standard method for the diagnosis of prostate cancer. This study compared the efficacy of IRLA alone and with PPNB using lidocaine or prilocaine.Materials and Methods:A total of 64 patients with suspicious rectal examination symptoms and/or serum prostate-specific antigen (PSA) elevation underwent standard 12-core TRUS-guided prostate needle biopsy. The patients were divided into 3 groups: IRLA with lidocaine gel only (group 1), IRLA with lidocaine gel plus PPSB with 2% lidocaine injection (group 2), and IRLA with lidocaine gel plus PPSB with 2% prilocaine injection (group 3). Patients’ pain levels were assessed by visual analog scale (VAS) immediately (VAS-1) and at 45 minutes (VAS-2) after prostate biopsy. In addition, serum PSA levels, age, body mass index (BMI), prostate volume, and cancer detection rates of the patients were recorded.Results:There were no statistical differences between the groups in terms of total PSA, age, BMI, prostate volume, or cancer detection rates. According to VAS-1 score, group 1 had more severe pain compared to the other groups, while there was no significant difference between groups 2 and 3. When VAS-2 score was examined, group 1 experienced the most severe pain, while group 3 was the least painful group. When asked which biopsy step was the most painful, all groups reported that introduction of ultrasound probe into the rectum was the most painful.Conclusion:The combination of IRLA and PPNB seems to be a more effective method than IRLA alone for pain control during TRUS-guided prostate biopsy. Prilocaine and lidocaine have comparable onset but the effect of prilocaine lasts longer. Therefore, prilocaine is a preferable agent for prostate biopsy under TRUS guidance.

    Abstract Grayanotoxin (GTX)-III is a Na-channel neurotoxin. Grayanotoxins can be found in the nectar, pollen, and other plant parts of the Rhododendron genus plants from the Ericaceae family. It is widely believed that honey produced from these plants, which are concentrated in the Black Sea region, is traditionally characterized as enhancing sexual performance. It is thought that the effective factor is dose for this compound, which has both beneficial and toxic effects reported. Therefore, it is aimed to evaluate the histological, immunohistochemical, and biochemical effects of acute and chronic impact of GTX-III in different doses on testes tissue in this study. For this purpose, 100 Sprague-Dawley male rats were divided into 5 separate groups for acute and chronic research. While dose groups were (control, 0.1, 0.2, 0.4, ve 0.8 μg/kg/bw) for experimental groups, a single dose (i.p.) was administered for acute impact whereas the same doses were administered daily for 3 weeks to assess chronic effect. At the end of the experiment, Johnsen testicular biopsy scoring was performed on testicular tissue samples, seminiferous tubule diameters were measured, and apoptotic cells were evaluated by TUNEL method. Testosterone, LH, and FSH levels were measured by ELISA method in serum and tissue specimens. It was found that Johnsen score of acute doses was significantly lower than the control group, and the diameter of the seminiferous tubules decreased significantly in acute and chronic dose-administered groups compared to the control. Hemorrhage, epithelial shedding, irregularity in seminiferous epithelium, and vacuolization were observed in acute and chronic dose-administered groups, and increase in apoptotic cells was determined. Hormone levels varied depending on the dose. In conclusion, it was found that dose-dependent acute and chronic effects of GTX-III are different, and this factor should be taken into account in studies to be carried out due to the adverse effects of high doses.

    Abstract This study was investigated whether parental consanguinity in males has an effect on or relationship with some infer tile subgroups and some semen and hormone parameters. The charts of 2651 infertile males were evaluated retrospec tively for parental consanguinity ratios, sperm counts, motility parameters and hormonal values from the records of 2651 infertile males. In 1260 eligible males the first cousin parental consanguinity ratio was 22.6%. In 119 males with nonobstructive azoospermic (NOA) and 430 males with normal sperm counts, the ratios were 34.5% and 20.9%, re spectively (p = 0.002). In the NOA group the parental consanguinity ratios were 27.1% (23/85) and 52.9% (18/34) in males with FSH values of >7.6 and < 7.6 mIU/ml, respectively (p = 0.007). In males with normal sperm counts if the parents were first cousins, both sperm counts and motility parameters were significantly reduced when compared with the others. To our knowledge, this is the first study of consanguinity ratios among some infertile subgroups. In males with parental consanguinity lower sperm counts and motility ratios in normozoospermic males and lower FSH levels in the NOA group might show a relation with some genetically transmitted defects.

    Abstract Aim:It is known that the Coronavirus disease-2019 (COVID-19) does not only affect the respiratory system in the body, but also affects many vital systems. In this study, we aimed to investigate polymerase chain reaction (PCR) positivity rates in urine samples of patients with COVID-19 infection and to evaluate the effectiveness of positron emission tomography/computed tomography (PET/CT) in demonstrating renal involvement in patients with urinary system involvement findings.Methods:Patients who had positive COVID-19 PCR test and were hospitalized in Erciyes University pandemic wards due to COVID-19 infection between June 2020 and December 2020 were included in this prospective study. A urine PCR test was applied to all patients. In addition, PET/CT was performed in patients with no known malignancy, clean urine culture, but suspected COVID-19 urinary system involvement.Results:A total of 66 patients with a mean age of 45.4±9.1 years were included in the study. PET/CT was performed at the same time in 6 of these patients with suspected urinary system involvement. Only 1 (1.5%) of 66 patients had a positive urine PCR test. No abnormal genitourinary PET/CT findings were found in any of the patients.Conclusion:Urine PCR positivity is very rare in patients with COVID-19 infection. In addition, according to our results, it can be said that PET/CT is not an effective imaging method to show COVID-19 urinary system involvement.

    Abstract Subperiosteal localization of aneurysmal bone cysts is relatively rare compared to the other localizations in the bones. The mass can cause symptoms dependent upon compression and edema of the surroundings tissues. In this study, we present a 14-year-old girl with a vaguely painful, firm, immobile, enlarging mass measuring 4x4x3 cm on the distal-anterior part of her left humerus. The mass, which was first realized one year before, was removed by the extraperiosteal-intralesional route using tourniquet.

    Abstract Objective: To examine the effect of warm and cold ischemia on functions of the operated kidney in cases with a normal contralateral kidney undergoing nephron sparing surgery. Methods: This study enrolled 40 patients with a normal contralateral kidney and without a renal function threatening risk factor, who were operated with NSS. The patients were randomized at admission. They were divided into 2 equal groups as warm and cold ischemia. An ice application for 10 minutes was done to cold ischemia group after clamping renal artery. Renal functions were evaluated with Technesium -99m -Dimercaptosuccinic Acid (DMSA) and serum creatinine at the preoperative and postoperative (day 1, day 15, month 6, and month 12) period. Statistical analysis was done with Mann Whitney U test, Wilcoxon Signed Rank test, and Fredman test. A p value below 0.05 was considered statistically significant. Results: There were no significant dif ferences between the groups in terms of age, body mass index, ischemia time, tumor size, amount of hemorrhage, and procedure time. Both groups had a significantly higher DMSA uptake at the preoperative period compared with the postoperative period (postoperative day 1, day 15, month 6, and month 12) (p < 0 . 001). However, both groups had similar DMSA uptake results at the postoperative period. Preoperative and postoperative creatinine levels were not significantly different from each other in both groups. Conclusion: Based on tumor localization, nephron sparing surgery without use of superficial cooling appears as a viable option for small renal masses.
