    Abstract This book aids clinicians in supporting and caring for transgender and gender-diverse children and adolescents – youth who are born into an incongruent body. A recent study using data from 19 states reported that 1.8% of American youth identified as transgender. Many people who are transgender will experience gender dysphoria, the intense emotional distress that is caused by a discrepancy between a person's gender identity and their sex assigned at birth. In this compact volume, the authors discuss the variety of domains involved in addressing gender dysmorphia: social, psychological, medical, and legislative/advocacy. They provide clear and concise information on the types and timing of gender-affirming medications and surgical interventions and offer useful suggestions for making interactions in the clinic and the clinical space inclusive for transgender and gender-diverse youth.     Abstract Lung health reflects the inequities of our society. Asthma and chronic obstructive pulmonary disease are 2 lung conditions commonly treated in general clinical practice; each imposes a disproportionate burden on disadvantaged patients. Numerous factors mediate disparities in lung health, including air pollution, allergen exposures, tobacco, and respiratory infections. Members of racial/ethnic minorities and those of low socioeconomic status also have inferior access to high-quality medical care, compounding disparities in disease burden. Physicians can work against disparities in their practice, but wide-ranging policy reforms to achieve better air quality, housing, workplace safety, and healthcare for all are needed to achieve equity in lung health.

    Abstract We are in the middle of an epidemic involving pediatric and adolescent throwing athletes. Too many young athletes have elbow injuries, and it is unnerving how young surgical indications can present. Increased competition has led to increased demands before athletes have reached their full adult size. Evidence shows that higher ball velocity, higher shoulder external rotation angle, and higher arm speed are associated with increased medial elbow torque and elbow injury in this population. Pediatric and adolescent athletes should not try to throw as hard as possible, and weighted baseball training should be banned for youth athletes.

    Abstract On May 12, 2022, Senator Bernie Sanders held a hearing in the U.S. Senate Budget Committee on Medicare for All legislation. These were the first such hearings in the U.S. Senate. In testimony presented to the Budget Committee, I argued that the achievement of Medicare for All was a medical and moral imperative. I explored the problem of uninsurance, noting that 30 million Americans remain uninsured at a cost of more than 30,000 deaths annually. I contended that improving the quality of coverage was equally crucial, describing how some 41 million Americans remain underinsured at a grave cost to their health and financial wellbeing. Finally, I examined the economics of Medicare for All reform, and showed how the reduction of the enormous administrative waste in American healthcare could save hundreds of billions of dollars a year. Medicare for All, I concluded, is the one health reform that could expand and improve coverage for all while simultaneously controlling costs.

    Abstract Vasorin (VASN), a transmembrane protein heavily expressed in endothelial cells, has garnered recent interest due to its key role in vascular development and pathology. The oligomeric state of VASN is a crucial piece of knowledge given that receptor clustering is a frequent regulatory mechanism in downstream signaling activation and amplification. However, documentation of VASN oligomerization is currently absent. In this brief report, we describe the measurement of VASN oligomerization in its native membranous environment, leveraging a class of fluorescence fluctuation spectroscopy. Our investigation revealed that the majority of VASN resides in a monomeric state, while a minority of VASN forms homodimers in the cellular membrane. This result raises the intriguing possibility that ligand-independent clustering of VASN may play a role in transforming growth factor signaling.
