    Abstract Independentemente do regime político, a exclusão social tem sido uma constante em Moçambique. O artigo propõe uma análise das categorias sociais desenvolvidas pelo colonialismo português nos inícios do século XX, e do seu percurso através dos diferentes debates e projetos políticos que os moçambicanos viveram, tanto durante o período colonial como depois da independência. O artigo procura explicar por que categorias tipicamente coloniais, como régulo ou indígena, reapareceram nos debates de finais do século XX, em pleno processo de transição para a democracia e o capitalismo.

    Abstract Drawing from ethnographic fieldwork in Massinga district, this article presents the building up of traditional authority’s legitimacy as a process linked to southern Africa regional history. Both the colonial State and the mining industry transformed African societies. Rural migrants, despite being confined to the customary sphere, were in fact straddling two different systems of social reproduction, as for successful young proletarians would become respected traditional elders. Thanks to this ambiguous position, each generation of migrants was able to build new and fancy houses. By doing so, they also contributed to renewing the legitimacy of traditional institutions. Once the mines stopped the recruitment of Mozambican young miners, the usual way to renew legitimacy collapsed.

    Abstract As in many African countries, agriculture has been one of the key issues when debating rural development in Mozambique. One of the current concepts in these debates is the so called subsistence agriculture. In this article we propose an historical perspective to look at changes in rural areas of Southern Mozambique before colonialism, during colonialism and after the independence. By focusing on the evolution of rural markets, social inequalities, kin and other hierarchies that structure rural societies, we propose that concepts as subsistence agriculture and family sector, if used without taking into account historical changes, tend to simplify too much ever changing rural complexities. We conclude that history can help in socioeconomic analysis and a better diagnosis for future policies.

    Abstract A grande densidade de população faz da região dos Grandes Lagos um lugar com tendência a processos de centralização política. Neste artigo vamos descrever a evolução dos reinos pré-coloniais em expansão durante o século XIX através da sua experiência colonial e pós-colonial no Uganda. Argumentaremos que a progressiva instauração do Estado colonial no Uganda pressupôs a consolidação de uma tendência para a exclusão política como forma habitual de manutenção do poder, o que conduziu no Uganda pós-colonial a uma crescente espiral de violência. Neste processo as instituições políticas ficaram subordinadas ao exército. The Great Lakes region, as a very densely populated region, has seen the development of some centralization process affecting its bigger kingdoms, which were in militar expansion during the 19th century. In this article we will focus on the colonial and postcolonial experience of these kingdoms inside Uganda. We will point out that the establishment of the colonial state supposed strengthen a bias to political exclusion as a way to keep power. This way of ruling sowed the seeds of the increasing political violence and civil wars experienced in postcolonial Uganda. In this process the state political institutions became subordinated to the army.

    Abstract El gran impacto que supuso el colonialismo para la mayoría de las sociedades africanas no se dio en el vacío, sino sobre la experiencia histórica previa de las sociedades colonizadas. La primera parte de este artículo presenta las limitaciones y los equívocos del adjetivo tradicional en la historia de las sociedades africanas. Al mismo tiempo plantea la necesidad de encontrar formas de nombrar y analizar la complejidad de la historia de las sociedades africanas (sus instituciones, sus valores, sus conflictos, sus narrativas, etc.) tanto antes del colonialismo, como también durante y después del colonialismo. La segunda parte presenta algunos ejemplos que muestran la diversidad de reacciones que hubo en dos etnias vecinas de lo que hoy es el oeste de Kenia: los lúo y los kipsigi. Esta diversidad de reacciones nos indica, al contrario de la visión del pasado africano ofrecida por la intelectualidad colonial, una experiencia histórica muy plural y flexible. Esta diversidad puede darnos pistas para analizar el presente de las sociedades actuales del oeste de Kenia.

    Abstract The importance of how disease and illness are conceptualised lies in the fact that such definition is paramount to understand the boundaries and scope of responsibility associated with medical work. In this paper, we aim to provide an overview of the interplay of these understandings in shaping the nature of medical work, philosophically, and in practice. We first discuss the emergence of the biopsychosocial model as an attempt to both challenge and broaden the traditional biomedical model. Then, we outline the main criticisms associated with the biopsychosocial model and note a range of contributions addressing the shortcomings of the model as initially formulated. Despite recurrent criticisms and uneven uptake, the biopsychosocial model has gone on to influence core aspects of medical practice, education, and research across many areas of medicine. One of these areas is adolescent medicine, which provides a particularly good exemplar to examine the contemporary challenges associated with the practical application of the biopsychosocial model. We conclude that a more optimal use of existing bodies of evidence, bringing together evidence-based methodological advances of the biopsychosocial model and existing evidence on the psychosocial needs associated with specific conditions/populations, can help to bridge the gap between philosophy and practice.

    Abstract Objective To map and explore existing evidence on the use of digital technology to deliver healthcare services with explicit consideration of health inequalities in UK settings. Methods We searched six bibliographic databases, and the National Health Service (NHS) websites of each UK nation (England, Scotland, Wales, Northern Ireland). Restrictions were applied on publication date (2013–2021) and publication language (English). Records were independently screened against eligibility criteria by pairs of reviewers from the team. Articles reporting relevant qualitative and/or quantitative research were included. Data were synthesised narratively. Results Eleven articles, reporting data from nine interventions, were included. Articles reported findings from quantitative (n = 5), qualitative (n = 5), and mixed-methods (n = 1) studies. Study settings were mainly community based, with only one hospital based. Two interventions targeted service users, and seven interventions targeted healthcare providers. Two studies were explicitly and directly aimed at (and designed for) addressing health inequalities, with the remaining studies addressing them indirectly (e.g. study population can be classed as disadvantaged). Seven articles reported data on implementation outcomes (acceptability, appropriateness, and feasibility) and four articles reported data on effectiveness outcomes, with only one intervention demonstrating cost-effectiveness. Conclusions It is not yet clear if digital health interventions/services in the UK work for those most at risk of health inequalities. The current evidence base is significantly underdeveloped, and research/intervention efforts have been largely driven by healthcare provider/system needs, rather than those of service users. Digital health interventions can help address health inequalities, but a range of barriers persist, alongside a potential for exacerbation of health inequalities.

    Abstract Abstract Background Post-stroke physical activity reduces disability and risk of further stroke. When stroke rehabilitation ends, some people feel abandoned by services and struggle to undertake physical activities that support recovery and health. The aim of this study was to codesign a novel text message intervention to promote physical activity among people with stroke and provide support when formal rehabilitation ends. This manuscript describes the intervention development processes that will inform future pilot and feasibility studies. Methods The planned intervention was a series of text messages to be sent in a predetermined sequence to people with stroke at the end of rehabilitation. The intervention, underpinned by behaviour change theory and using salient behaviour change techniques, would provide daily messages offering encouragement and support for the uptake and maintenance of physical activity following stroke. The intervention was codesigned by a Collaborative Working Group, comprised of people with stroke, rehabilitation therapists, representatives from stroke charities and academics. A four-step framework was used to design the intervention: formative research on physical activity post-stroke, creation of the behaviour change text message intervention, pre-testing and refinement. Formative research included a review of the scientific evidence and interviews with community-dwelling people with stroke. Data generated were used by the Collaborative Working Group to identify topics to be addressed in the intervention. These were mapped to constructs of the Health Action Process Approach, and salient behaviour change techniques to deliver the intervention were identified. The intervention was rendered into a series of text messages to be delivered over 12 weeks. The draft intervention was revised and refined through an iterative process including review by people with stroke, their spouses, rehabilitation therapists and experts in the field of stroke.     Abstract Este artigo propõe um itinerário pela história local de Quême, que nos levará a entender a transição do Hosi Zunguze para o regulado Zunguze. Este processo de substituição explica-se pela tentativa de sobrepor, à ordenação territorial existente naquele reino (assente no rito e no parentesco), umas estruturas administrativas assentes, por sua vez, na escrita e nos mapas. A análise das relações de poder vigentes hoje em dia em Quême permite-nos sugerir que, apesar das múltiplas mudanças experienciadas ao longo deste processo, o parentesco e o rito continuam a gerar mais confiança que a administração pública. This article takes the reader on a journey through the local history of Quême, helping us to understand the transition from the Hosi Zunguze to the regulado Zunguze. This process of substitution can be explained as an attempt to impose word and map-based administrative organization on the existing ritual and kinship-based territorial arrangements in that kingdom. An analysis of present-day power relationships in Quême leads us to suggest that, despite the many changes which have occurred throughout this process, greater trust continues to be placed in kinship and ritual than in the state administration.
