    Abstract Science and the production of scientific knowledge are changing, the parts are not the objective, it is the whole and its interrelationships. Health faces new challenges that cannot be explained by reductionist conceptions. The attempt to group various conditions in non-communicable diseases that cause three quarters of deaths in the world and in Cuba is recognized as one of the great challenges of sustainable development. Many of them, despite their heterogeneity and multi-causality, share common risk -and protective- factors, which can be reversed with integrated actions. The frequent comorbidity of these diseases with other entities, polymorbidity, and the role of aging in their appearance are discussed. The presence of the chronic inflammatory state of the endothelium and the dysfunction of this organ are highlighted as a possible final common path of the pathogenesis, evolution and subsequent clinical expression of many of the so-called non-communicable diseases, true systemic syndromes. A group of recommendations are given to improve its prevention and control, with emphasis on the community participation of all, where the social sciences can contribute a lot to achieve better results, without forgetting individual or group susceptibility to suffering certain non-communicable diseases. It is considered that managers, health professionals, formal and informal leaders, people, groups and communities must be trained and updated in concepts and approaches to these diseases to achieve their purposes.

    Abstract Due to the favorable prognoses in relation to the control of the COVID-19 pandemic in Cuba, a stage of "new normality" was announced, with a progressive "de-escalation" of measures that had been implemented to prevent the disease. This has been possible thanks to the joint efforts of the whole of society, with its highest authorities ahead, the capacity and dedication of health professionals organized in a vigorous national health system, obtaining and applying effective native drugs and the massive immunization against SARS-COV-2, with our Cuban Abdala and Soberanas vaccines. Comments are made on a group of lessons learned during the pandemic, on the existence of many doubts and questions still unanswered, on the new risks -especially for the most vulnerable- including the so-called post-COVID syndrome, when events are held more massive outbreaks of different kinds, as well as alerting to the frequent appearance of new coronavirus’ variants and recent "waves" of patients in different countries. The current situation of COVID-19 in Cuba and the suggested recommendations for this stage are briefly addressed.

    Abstract After having suffered an acute infectious disease, a group of people suffer from very varied clinical manifestations, which can be transitory or permanent. Such is the case of COVID-19, in which a multitude of general symptoms and organs and systems have been reported, in a not inconsiderable number of people who have previously been infected with the SARS-CoV-2 virus. The World Health Organization issued on October 6, 2021 an official clinical definition of post-COVID-19 disease, agreed after a global consultation and published with the purpose of facilitating the treatment of the sick. Regarding this definition, comments are made about its implications for patients, health services and society in general.

    Abstract High blood pressure is a recognized health problem. It affects more than 30 % of the adult population in the world. It is considered the most important risk factor for cardiovascular diseases, especially coronary heart disease and cerebrovascular disease; chronic kidney disease, heart failure and dementia, contributes to global morbidity and mortality. In Cuba, its high prevalence has been proven, a proportion that increases with age. With the arrival of the COVID-19 pandemic, arterial hypertension has been reinforced its leading role, which it already had as a “tracer condition” of chronic non-communicable diseases in health services, now also as an important marker in the most cases of severe respiratory infection SARS-CoV-2. Maintaining adequate blood pressure figures is an important recommendation for maintaining people's health, also now in the face of the COVID-19 pandemic

    Abstract This work reviews the evolution of Cuban medical publications, from the point of view of the author and his professional experience in Cienfuegos. The efforts, the main results obtained and the factors which contributed to them are stated with emphasis in the first four decades. It is recognized the significant increase in the number of Cuban digital publications in the whole country, the determinant role played by Infomed, starting on the Internet age and the revolution of the Information and Communication Technologies. Although this has not been accompanied by an increase in the global consistent quality of the Cuban medical publications and there are still dissatisfactions and gaps in the issue regarding the potentials that there exits. The author insists in the necessity of providing adequate priority and training of all to write the proper quality the results of the Cuban Medical Science and its divulgation in adequately acknowledged publications.

    Abstract The article relates the first associations with scientific purposes that worked in Cienfuegos during the second half of the 19th century, with emphasis on the Medical Association of Cienfuegos and the Medical-Pharmaceutical Center of Cienfuegos and its relations with the Royal Academy of Medical Sciences, Physical and Natural of Havana, in addition the first professionals from Cienfuegos who worked as Academic “reporters" are mentioned. It refers to the constitution and integration of the first Provincial Technical Advisory Council of Cienfuegos in 1981, and the current, governed by the Ministry of Science, Technology and Environment since its emergence in 1994 to the present times. Professionals who have been chosen to represent Cienfuegos in the Cuban Academy of Sciences in the different periods and the foundation of the Alternative Office of the Academy of Sciences in Cienfuegos in the year 2000, as well as the sponsor institution of the Hospital Dr Gustavo Aldereguía Lima, in 2004, are mentioned. It explains the frequent participation of other Academics -especially those living in the capital- in various activities in the province. The constitution of the Provincial Branch of the Cuban Academy of Sciences in Cienfuegos on April 25, 2019, and the challenges that lie ahead are discussed.

    Abstract Convalescence is the last stage of the "natural history of a disease" - if death does not occur - and it is often not given full attention, so it is not idle to review some of its basic concepts. One year after the onset of the unprecedented SARS-CoV-2 pandemic, the gaze has turned to this evolutionary stage of the patients and to the identification and monitoring of clinical manifestations and possible effect. With the purpose of deepening the importance of this stage in COVID-19 patients and specifying its main characteristics, this work was carried out to contribute to its better understanding with updated information and favor the results in its management. The concepts of clinical discharge and epidemiological discharge and the need to assess the previous diagnosis are addressed. It insists on the factors that can influence convalescence: the characteristics of each person (biological, psychological, social, cultural and spiritual); the "state of seriousness" that the patient had in the acute phase of the disease and whether there was a need for admission to intensive care; adverse effects of medications and other procedures carried out during care and comorbidities.
