    Abstract This study focuses on the spatiotemporal dynamics of land use and land cover (LULC) in the Tselinograd district, which is a peri-urban area (PUA) of the metropolis Nur-Sultan. The analysis of LULC dynamics in PUA was carried out based on a supervised classification in the twenty-year interval (1998, 2008, and 2018). During the study period, noticeable changes occurred in the structure of LULC. At the border with the city, the built-up area increased dramatically. In the PUA the area of arable land and forests has grown steadily and pasture land has been declining. That is, there is an intensification of land use due to an increase in the share of arable land in the study area. The main drivers of LULC change in PUA are urban expansion and population growth of the Nur Sultan metropolis. In general, as a result of reasonable economic and legislative measures, the influence of the Urban sprawl of the capital of Kazakhstan on the PUA is still accompanied by a slight effect on its sustainable development. Our approach by using Geo-Information Techniques is useful for the rapid detection of phenomena and processes that can lead to the unstable development of PUA.

    Abstract The question of studying the characteristics of sensitivity / resistance of bacteria isolated from patients with urolithiasis in order to develop optimal therapy and correct complications in patients with a urological profile is relevant. Purpose of the study. Study of the etiological structure and level of antibiotic resistance of pathogens of infectious and inflammatory complications of infections in patients with a urological profile. Methods. The case histories of 345 patients who were treated in the urology department with a diagnosis of urolithiasis, male and female over the age of 18, were studied. The material for bacteriological studies was urine. Results. When analyzing age indicators, it was found that adults over the age of 31-40 years are more likely to be at risk of developing the studied infections. Infectious and inflammatory complications were presented: acute pyelonephritis, acute cystitis. The spectrum of pathogens of infectious and inflammatory complications of the urinary tract in patients after long-distance lithotripsy is mainly represented by gram-negative flora: E. coli, Ps. aeruginosa, Proteus spp. Staphylococcus sapr prevailed among representatives of gram-positive pathogens. The most effective drugs against Ps strains. Aeruginosa proved to be polymyxin, cephalexin. Almost all Klebsiella strains showed high sensitivity to cephalotin and cephalexin. Microorganisms of the genus Proteus showed high sensitivity to gentamicin and cephalexin. Ñonclusion. It is advisable to use the data when choosing the starting empirical antibiotic therapy in the development of the leading complications of distant lithotripsy.

    Title translation Assessment of endogenous intoxication values by oral fluid composition.
    Abstract Objective: To study changes in the parameters of endogenous intoxication using the level of substances of low and medium molecular weight in the oral fluid in patients with catarrhal stomatitis, which got developed after the acute drug poisoning.
    Material and methods: To determine the degree of intoxication, we studied the concentrations of low and medium molecular weight substances (SLMMW) in the oral fluid of patients. We studied the saliva of 20 patients (13 men and 7 women, average age 39 years). The control was done against the oral fluid of 10 healthy individuals. The studies were conducted in a control group consisting of 10 healthy individuals with intact oral mucosa and two groups of patients (20 patients in total) with drug poisoning. The first of these two groups of patients included 8 individuals without pathological changes in the oral mucosa, while the second group consisted of 12 patients diagnosed with catarrhal stomatitis that developed after acute drug poisoning.
    Results: It became known that endogenous intoxication develops in the oral fluid of patients with acute drug poisoning, which is manifested by a statistically significant increase in the content of SLMMW in comparison with practically healthy individuals (15.3%). And the total level of SLMMW in the oral fluid of patients with acute drug stomatitis and catarrhal stomatitis has increased by 42.3% relative to the data of practically healthy individuals.
    Conclusion: Studies have shown the need for monitoring the oral cavity in patients with acute drug poisoning, with the aim of timely detection of pathologies of the oral mucosa in them and the appointment of complex treatment.

    Abstract With an increase in the average life expectancy of the population, some histological types of symptomatic IEMT are more common in elderly and senile patients. Of the 45 patients included in the study, 27 (60%) patients underwent a microneurosurgical resection of the tumor tissue using minimally invasive techniques (minimally invasive group) and 18 (40%) to patients using the classical open method (open group). The duration of operative intervention in both cohorts of respondents is comparable and is 245,4±117,1 min and 261,1±108,6 min for open and minimally invasive groups, respectively (p=0,71). The volume of blood loss in the minimally invasive group of patients (139,6±44,6 ml) was statistically significantly lower than in the open technique group (539,2±127,5 ml) (p<0,01). The total degree of resection of tumor tissue was achieved in 97,4% of patients in the open group and 92,8% in the minimally invasive group (p=0,81). The incidence of recurrence of IEMT in the study cohort of patients was 6,6%. At the same time, in 1 case the relapse of IEMT was verified in an open group of patients and in 2 cases in patients of the minimally invasive group. The average duration of hospitalization of patients of the minimally invasive group was 9,6±2,7 days, and the open group 13,5±3,1 (p<0,01). The comparison of the incidence of adverse events between the two groups of patients did not show significant differences (p=0,61).

    Abstract For the last 10 years, due to the improvements of diagnostic methods of neurosurgical diseases and to the expansion of technical possibility of surgical manipulations, which were not used on the brain before, new surgical access to tumors of the brain was established. From 2000 to 2011, 72 patients with tumors of anterior, middle, and posterior parts of the brain in the Neurosurgical Clinic of Samarkand State Medical Institute were treated. In this work, we decided to share our experience on removal of giant meningioma, which was growing from the base bone and cavernous sinus, with spreading to the external and internal parts of the middle brain on the left side. Patient K suffered from moderate headaches for 8 years. Then this patient had ptosis that gradually developed, and he also had limitation of the left eyeball. In addition to this, he had worsening vision in the left eye with reduction of the vision field from the left. The keenness was 0.5 for the left eye. Surgery was performed using the fronto-temporo-orbito-zygomatic access. The reason for this approach was the dense connection of the tumor with the cavernous sinus. We could not ablate it. After 7–10 postoperative days, the patient's general neurological symptoms were gone. Ptosis converted into half ptosis. Paresis of the nerves abducens and trochlearis became better. After 17 days post-treatment, the patient was discharged from the hospital. We prescribed radial therapy for him.

    Abstract The purpose of the study was to compare the clinical and radiological efficacy of 2-level anterior cervical discectomy and fusion (ACDF) and 1-level anterior cervical corpectomy and fusion (ACCF) in the treatment of two-segment degenerative stenosis of the cervical spine in elderly patients. The retrospective study included 74 elderly patients (over 60 years old), two cohorts were identified: the 1st (n=38) - patients who underwent staged 2-level ACDF; the 2nd (n=36) - patients operated on using the technique of 1-level ACCF. For comparative analysis, we used general information about patients (gender, age, body mass index, physical status according to ASA), characteristics of surgical interventions (time of surgery, volume of blood loss), features of the postoperative period, clinical data, radiological outcomes, and the presence of complications. Analysis of the results was carried out in a minimum period of 36 months. As a result, it was found that 2-level ACDF in the treatment of two-segment degenerative stenosis of the cervical spine in older patients is associated with less blood loss and the level of local pain syndrome. At the same time, better clinical outcomes and fewer revision interventions after 1-level ACCF were recorded in the long-term postoperative period.

    Abstract In recent years, one of the major challenges for financial institutions is the retention of their customers using new methodologies of reliable and profitable segmentation. In the field of banking, the approach of offering all of the services to all the existing customers at the same time does not always work. However, being aware of what to sell, when to sell and whom to sell makes a huge difference in the conversion rate of the customers responding to new services and buying new products. In this paper, we used RFM technique and various clustering algorithms applied to the real customer data of one of the largest private banks of Azerbaijan.

    Abstract Background: The skills required for neurosurgical operations using microsurgical techniques in a deep operating field are difficult to master in the operating room without risk to patients. Although there are many microsurgical training models, most do not use a skull model to simulate a deep field. To solve this problem, 3D models were created to provide increased training in the laboratory before the operating room, improving patient safety.
    Methods: A patient's head was scanned using computed tomography. The data were reconstructed and converted into a standard 3D printing file. The skull was printed with several openings to simulate common surgical approaches. These models were then used to create a deep operating field while practicing on a chicken thigh (femoral artery anastomosis) and on a rat (abdominal aortic anastomosis).
    Results: The advantages of practicing with the 3D printed models were clearly demonstrated by our trainees, including appropriate hand position on the skull, becoming comfortable with the depth of the anastomosis, and simulating proper skull angle and rigid fixation. One limitation is the absence of intracranial structures, which is being explored in future work.
    Conclusion: This neurosurgical model can improve microsurgery training by recapitulating the depth of a real operating field. Improved training can lead to increased accuracy and efficiency of surgical procedures, thereby minimizing the risk to patients.

    Title translation Results of minimally invasive lumbar fusion in professional athletes: a single-center retrospective study.
    Abstract Professional athletes have a high basic level of physical fitness and significant motivation for returning to previous rhythm of functional activity within the shortest period after trauma without deterioration of baseline level of sportsmanship. Despite the well-presented results of lumbar spine fusion in general population, these data among professional athletes are not well understood.
    Objective: To analyze the results of minimally invasive lumbar fusion in professional athletes.
    Material and methods: A retrospective study included 27 athletes (19 men and 8 women) aged 29 (26; 34) years after minimally invasive lumbar spine decompression and fusion via anterior, lateral and posterior approaches. Total lumbar lordosis, degenerative changes in adjacent segment, area of multifidus muscle, perioperative complications and timing of sports rehabilitation were analyzed within 3-5 (mean 4) years after surgery.
    Results: We found an increase of total lumbar lordosis from 35.2° to 44.1° (
    Conclusion: Minimally invasive short-segment lumbar interbody fusion in professional athletes is characterized by low risk of fixation element failure, no significant degeneration of adjacent level and postoperative muscle atrophy. This procedure ensures early rehabilitation and returning to sports.
