    Abstract Proposals to replace the existing mix of public and private health financing with a fully nationalized government system ("Medicare for All") fail to address fundamental problems of high cost, overutilization, and inefficiency. Those proposals are a reaction to the Affordable Care Act, which provided new insurance subsidies but did little to solve the cost problem. Stabilizing the individual insurance market is not the goal. Reform should focus on making the entire insurance market and the health delivery system more efficient and more equitable by promoting consumer choice, market competition, private initiative, and state experimentation.

    Abstract Tetracyclines are among the most commonly used antibiotics for the treatment of bacterial infections and the improvement of agricultural growth and feed efficiency. All compounds in the group of tetracyclines (tetracycline, chlorotetracycline, doxycycline, and oxytetracycline) are excreted in an unchanged form in urine at a rate of more than 70%. They enter the aquatic environment in altered and unaltered forms which affect aquatic micro- and macroorganisms. This study reviews the occurrence, fate, and removal techniques of tetracycline contamination in Europe. The average level of tetracycline contamination in water ranged from 0 to 20 ng/L. However, data regarding environmental contamination by tetracyclines are still insufficient. Despite the constant presence and impact of tetracyclines in the environment, there are no legal restrictions regarding the discharge of tetracyclines into the aquatic environment. To address these challenges, various removal techniques, including advanced oxidation, adsorption, and UV treatment, are being critically evaluated and compared. The summarized data contributes to a better understanding of the current state of Europe's waters and provides insight into potential strategies for future environmental management and policy development. Further research on the pollution and effects of tetracyclines in aquatic environments is therefore required.

    Abstract The Affordable Care Act expanded health insurance coverage in the United States but did little to address the structural problems that plague the U.S. health care system. Controlling cost while maintaining or improving access to quality care requires a more fundamental reform based on market principles. Such an approach means aligning the financial incentives of patients and providers to promote smarter spending. It also requires better information and more flexible regulation to promote well-functioning competitive markets. Key elements of these reforms include setting reasonable limits on subsidies for Medicare, Medicaid, and private health insurance; modernizing the Medicare program and adopting reforms that promote competition between traditional Medicare and Medicare Advantage; allowing greater flexibility for states in running their Medicaid programs; enacting smarter regulations to protect consumers without imposing greater inefficiency on the health market; and promoting more direct consumer involvement in all phases of their health and health care. These changes will challenge academic medical centers as a new era of creativity and competition emerges in the health care market.

    Abstract American Health & Drug Benefits ™ has been created to act as an ideological melting pot focusing on health and drug benefit decision makers, as well as those who may affect or may be affected by those decisions. By engaging in conversations with payors, regulators, employers, and other stakeholders, our journal hopes to enable decision makers to view the impact of benefit designs from as many perspectives as possible. Through this open dialogue, we hope that better decisions may be made, and that the greater healthcare marketplace will be positively impacted. In following our editorial mission, American Health & Drug Benefits ™ has sought thought leaders who have influenced and will continue to influence the healthcare marketplace. During a fall visit to Washington, DC, Dr. Joseph Antos was kind enough to host a visit to the American Enterprise Institute and provide his thoughts to Robert Henry, editor-in-chief, on how the Centers for Medicare & Medicaid Services (CMS) exerts its influence on drug coverage in the wake of the Medicare Modernization Act (MMA) and in the face of evidence-based medicine standards. Following a chronology of CMS's role from its inception to current events, Dr. Antos offers a lively insight into CMS's strategy and its tactical effects on the American healthcare system.

    Abstract Tungsten-based materials are the most prospective candidates for plasma-facing components of future fusion devices, such as DEMO. W-based composites and graded layers can serve as stress-relieving interlayers for the joints between plasma-facing armor and the cooling or structural parts. Coating/cladding techniques offer the advantages of eliminating the joining step and the ability to coat large areas, even on nonplanar shapes. In this work, W + Cu and W + Ni composites were prepared by pulsed plasma transferred arc (PTA) cladding on several different substrates. Optimization of the process was carried out with respect to powder mixture composition and process parameters like arc current, plasma gas composition, and traverse velocity. Dense claddings of several millimeters thickness and various W content were achieved. Moreover, multilayers with W content gradually varying from 47 to 92% were formed. The structure, compositional profiles, and thermal properties of the claddings were characterized.

    Abstract In the wake of the Supreme Court decision, states should not rush to expand eligibility for Medicaid. They cannot be certain that the federal support promised in the Patient Protection and Affordable Care Act will remain available, and a better deal might be possible after the election. Adding millions more to Medicaid rolls will exacerbate existing problems of access to providers. A more humane policy would give everyone--even the poor --a choice of health plans.
