    Abstract Objetivo: conhecer os níveis de depressão e sintomas depressivos em angolanos com HIV. Método: estudo descritivo e misto, com estratégia exploratória sequencial, em período pré-pandêmico em um hospital público de Angola. Realizou-se entrevistas semiestruturadas, com aplicação do Inventário de Depressão de Beck. Para fins de análise, considerou o conceito de depressão a partir da classificação internacional de doenças. Resultados: prevaleceu depressão leve a moderada para ambos os sexos e estado civil. No G1, a maioria além da tristeza ou infelicidade não conseguem suportar a angústia, alguns verbalizam ter consciência de que os choros não os levará a ter vida anterior desta realidade. Agora se sentem irritados o tempo todo e não sentem mais prazer nas coisas como antes. No G2, expressaram ideias e sentimentos suicidas, estavam desanimados quanto ao futuro, percebem-se fracassados mais do que uma pessoa sem a doença, e adiam a tomada de decisão mais do que o costume. Conclusão: embora não tenha prevalecido formas severas (graves) de depressão, a ocorrência de sintomas negativos em variadas esferas da vida pode evoluir, especialmente no contexto angolano.

    Abstract As infecções respiratórias agudas constituem um problema de saúde pública em Angola, pela perda de vidas humanas. Para descrever a sua ocorrência, realizou-se um estudo descritivo longitudinal de Síndromes Gripal, Pneumonia grave em menores de cinco anos e Infecções Respiratórias Agudas em maiores de cinco anos, no período de 2012 - 2019. Analisou-se á morbilidade e mortalidade em tempo e espaço. As fontes estatísticas oficiais foram os relatórios provinciais, fichas de notificação de caso. Foram utilizadas frequência absoluta e relativa, taxas gerais e mudança relativa. Os resultados foram apresentados em tabelas e gráficos. A Síndrome Gripal incrementou o risco de adoecer em 9,2% entre Junho e Agosto nos anos 2018 e 2019, a Pneumonia em menores de cinco anos em 9,5% entre Março a Junho e a Infecção Respiratória Aguda em 4,7% entre Abril e Agosto e o corredor endêmico do país esteve em epidemia destas doenças. O risco de morrer por Infecção respiratória grave em maiores de 5 anos incrementou em 5,7%. O risco da Pneumonia grave em menores de 5 anos diminuiu em14,3%. Nos meses de Maio à Setembro incrementou o risco da morbilidade das doenças respiratórias a nível das províncias e registaram-se surtos epidémicos cíclicos. Neste período a mortalidade e letalidade por Infecções Respiratórias Agudas em maiores de 5 anos e Pneumonia grave em menores de 5 anos aumentou o risco a nível das províncias, em relação ao país. Evidenciou-se que para diminuir a morbilidade e mortalidade é necessário uma vigilância activa destas doenças.

    Abstract Diabetes is common in urban settings in Sub-Saharan Africa. Household food insecurity has been suggested to increase the chance of developing diabetes among adults. The relationship between diabetes and food insecurity has not been explored in Angolan urban settings so far. This case-control (1:2) study investigated the association between diabetes and food insecurity among adults attending six healthcare facilities in Luanda (Angola) between April 2019 and September 2019. All subjects with fasting blood glucose (FBG) levels ≥ 126 mg/dl were included as cases. For each case, the next two subjects with FBG levels < 110 mg/dl were included as controls, to warrant the achievement of the set 1:2 ratio. Food insecurity was assessed using the Food Insecurity Experience Scale (FIES). A total of 663 participants (221 cases and 442 controls) were enrolled in the study. Median FIES raw score was 7 (IQR 1-8) in cases and 5 (IQR 2-8) in controls (p = 0.09). The distribution of FIES levels (0-3; 4-6; 7-8) was different between cases and controls (p < 0.0001), with highest FIES scores (7-8) recorded in 53.0% of cases and 38.2% of controls. Our findings revealed an association between diabetes and severe food insecurity among adults attending healthcare facilities in the capital city of Angola.

    Abstract Sickle cell disease (SCD) is a life-threatening blood disorder affecting >500 000 infants annually, mostly in sub-Saharan Africa. Most infants do not have access to an early diagnosis and die early from treatable complications of SCD. Universal newborn screening (NBS) is not yet available in any African country for a variety of reasons, including lack of laboratory capacity, difficulty in tracking affected infants, and the relatively short stay of mothers and newborns at maternity hospitals. Several point-of-care (POC) tests for SCD have been recently developed and validated, but the 2 most well-established tests (Sickle SCAN and HemoTypeSC) have not been rigorously compared with one another. In this study, we aimed to evaluate and compare these 2 POC tests to screen infants aged ≤6 months in Luanda, Angola. Challenging the traditional NBS paradigm, we performed testing not only at maternity centers, but also at vaccination centers across Luanda. We enrolled 2000 babies and performed 1000 tests with each POC test. Both tests demonstrated diagnostic accuracy, with 98.3% of Sickle SCAN results and 95.3% of HemoTypeSC results aligning with the gold standard isoelectric focusing hemoglobin pattern. When the result was provided at the POC, 92% of infants were linked to SCD care compared with 56% in the pilot Angolan NBS program, which used centralized laboratory testing. This study demonstrates the real-world feasibility and accuracy of POC tests to screen infants for SCD in Angola. This study also suggests that including vaccination centers may improve the capture rate for early infant SCD screening programs.

    Abstract Background and aims: The acute tropical infectious disease known as yellow fever (YF) is caused by an arbovirus and is characterized by fever, jaundice, hemorrhage, headache, muscle pain, nausea, vomiting, and fatigue. Angola experienced a yellow fever virus (YFV) outbreak that was documented in December 2015. However, little is known about the outcome of this outbreak. We aimed to demonstrate epidemic features and lessons learned during the YF epidemic in Angola.
    Methods: A total of 4618 blood samples from suspected YF cases were sent to the Instituto Nacional de Investigação em Saúde (INIS), a national referral and public health laboratory, between December 5, 2015, and December 23, 2016. Sample analyses were conducted using enzyme-linked immunosorbent assay (ELISA) and reverse transcription polymerase chain reaction (RT-PCR) assays. Blood samples were sent from 16 out of the 18 provinces of Angola.
    Results: We detected 884 (19.1%) cases that were positive for ELISA, which were confirmed by RT-PCR assay. Considering the positive cases, the incidence among male patients was around three times higher (
    Conclusions: This study demonstrates features of the YF epidemic that occurred in Angola. Also, it demonstrates that YF causes deaths in young people but is preventable by high vaccine coverage. Thus, public health laboratory surveillance must be strengthened to reduce the possibility of emerging and re-emerging human infections.

    Title translation Revisão realista como metodologia para utilização de evidências em políticas de saúde: uma revisão integrativa.
    Abstract Objective: To identify and analyze the concepts of realist review and the recommended methodology for its development in the health area.
    Method: An integrative review which sought theoretical-methodological qualitative or quantitative studies through 11 data sources which analyzed or proposed theories and/or realist review methodology.
    Results: Twenty-three (23) studies, mainly published in the United Kingdom, were included. The realist review aims at explaining what makes a complex policy, program, or intervention work, in which aspects, for whom, in what context, to what extent, and why, by constructing theory, decompressing the functioning mechanisms, complex policy contexts or programs which generate results. Such methodology relies on the involvement of stakeholders in the problem from the early stages of the review process.
    Conclusion: The realist review has the potential to provide relevant and applicable subsidies for researchers, workers, policy makers and health decision makers.

    Abstract A 38-year-old male with established diagnosis of stage IV squamous cell carcinoma of the esophagus treated with chemoradiotherapy (25 sessions of 50 Gy), presented with acute aphagia, thoracic pain, productive cough, and mild hemoptysis. Upon physical examination the right hemithorax presented with crepitations. An initial CT scan showed an esophageal perforation. An upper endoscopy was performed, visualizing the esophageal perforation in the mid third of the esophagus at 26 cm of the dental arcade. It was possible to bypass and intubate the stomach, enabling the placement of a guide wire under endoscopic visualization. Afterwards, a partially covered, self-expandable, metal stent (Wallflex esophageal stent 10 cm/18/23; Boston Scientific) was placed in the esophagus restoring continuity, visualized by fluoroscopy.
