    Abstract Introduction: Periprosthetic joint infection (PJI) treatment has high failure rates even after two-stage revision. Risk factors for treatment failure after staged revision for PJI are not well defined, nor is it well established how they correlate with the risks of developing an index PJI. Identifying modifiable risk factors may allow preoperative optimization, while identifying non-modifiable risk factors can influence surgical options or advise against further surgery. We performed a systematic review and meta-analysis to better define predictors of treatment failure in two-stage revision for PJI.
    Methods: The Pubmed, Embase, and Scopus databases were searched from their inception in December 1976 to April 15, 2023. Studies comparing patient-related variables between patients successfully treated who had two-staged revision total hip arthroplasty and patients with persistent infections were included. Studies were screened, and two independent reviewers extracted data, while a third resolved discrepancies. Meta-analysis was performed on these data. There were 10,052 unique studies screened, and 21 studies met the inclusion criteria for data extraction.
    Results: There was good-quality evidence that obesity, liver cirrhosis, and previous failed revisions for PJI are non-modifiable risk factors, while intravenous drug use and smoking are modifiable risk factors for treatment failure after two-stage revision for hip PJI. Reoperation between revision stages was also significantly associated with an increased risk of treatment failure. Interestingly, other risk factors for an index PJI including male gender, American Society of Anaesthesiology score, diabetes mellitus and inflammatory arthropathy did not predict treatment failure. Evidence on Charlson Comorbidity Index was limited.
    Discussion: Patients with a smoking history, obesity, intravenous drug use, previous failed revision for PJI, reoperation between stages and LC are more likely to experience TF after two-stage revision THA for PJI. Modifiable risk factors include smoking and IVDU and these patients should be referred to services for cessation as early as possible before two-stage revision THA.

    Abstract Objective: To develop an algorithm that can infer the severity level of a COPD patient's airflow limitation from tidal breathing data that is collected by a wearable device.
    Methods: Data was collected from 25 single visit adult volunteers with a confirmed or suspected diagnosis of chronic obstructive pulmonary disease (COPD). The ground truth airflow limitation severity of each subject was determined by applying the Global Initiative for Chronic Obstructive Lung Disease (GOLD) staging criteria to the subject's spirometry results. Spirometry was performed in a pulmonary function test laboratory under the supervision of trained clinical staff. Separately, the subjects' respiratory signal was measured during quiet breathing, and a classification model was built to infer the subjects' level of airflow limitation from the measured respiratory signal. The classification model was evaluated against the ground truth using leave-one-out testing.
    Results: Severity of airway obstruction was classified as either mild/moderate or severe/very severe with an accuracy of 96.4%.
    Conclusion: Tidal breathing parameters that are measured with a wearable device can be used to distinguish between different levels of airflow limitation in COPD patients.

    Abstract Data on adverse events from research bronchoscopy with bronchoalveolar lavage (BAL) in patients with cystic fibrosis (CF) is lacking. As research bronchoscopy with BAL is useful for isolation of immune cells and investigation of CF lung microbiome, we sought to investigate the safety of bronchoscopy in adult patients with CF. Between November 2016 and September 2019, we performed research bronchoscopies on CF subjects (32) and control subjects (82). Control subjects were nonsmokers without respiratory disease. CF subjects had mild or moderate obstructive lung disease (FEV1 > 50% predicted) and no evidence of recent CF pulmonary exacerbation. There was no significant difference in the age or sex of each cohort. Neither group experienced life threatening adverse events. The number of adverse events was similar between CF and control subjects. The most common adverse events were sore throat and cough, which occurred at similar frequencies in control and CF subjects. Fever and headache occurred more frequently in CF subjects. However, the majority of fevers were seen in CF subjects with FEV1 values below 65% predicted. We found that CF subjects had similar adverse event profiles following research bronchoscopy compared to healthy subjects. While CF subjects had a higher rate of fevers, this adverse event occurred with greater frequency in CF subjects with lower FEV1. Our data demonstrate that research bronchoscopy with BAL is safe in CF subjects and that safety profile is improved if bronchoscopies are limited to subjects with an FEV1 > 65% predicted.
