    Abstract Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), sometimes referred to as an invisible killer, has changed all of our lives, including the daily practices of medical personnel and the approach to anesthesia. We must wear protective equipment and special suits, aprons, and 2 or 3 pairs of gloves. Our faces are obscured by masks and goggles and shields. We move with difficulty and have limited vision. Communication with patients is limited; they cannot see our facial expressions of smiles or concern. They only hear our voice, which may also be muffled. We look more like robots than someone demonstrating assistance, love, or compassion. We are strangers to our infected patients. When thinking about the type of anesthesia for pregnant patients who have the disease caused by SARS-CoV-2, coronavirus2019 (COVID-19), 3 factors taken into consideration ranked according to importance are the safest method for healthcare providers, ease of application, and difficulties associated with movement and vision. The patient's wishes are secondary. Have priorities changed?

    Abstract INTRODUCTION[|]A preoperative checklists guide was issued in 2008 by the American Society of Anesthesiologists (ASA) to improve patient safety. We aimed to determine the applicability and effectiveness of video-based feedback for the transfer of knowledge into clinical practice by using this guide.[¤]METHODS[|]Ten anesthesia residents were divided into two groups according to their seniority, under two years and over two years. Once the preparation, anesthesia induction and airway management steps were completed, video-based feedback was given according to the ASA checklist. The knowledge level and skills of the residents were evaluated. Two months after the video-based feedback, all steps were repeated in another patient. The results were compared with a checklist score. Each guideline item was analyzed.[¤]RESULTS[|]The ages of participants varied between 27–34 years (28.9+-2.28 years), five were female, and five were male. Results of the pre and post video-based feedback evaluations of the residents were statistically similar in the senior and junior groups. The post-feedback achievement score in both groups was significantly higher than the pre-feedback achievement score (p<0.05).[¤]DISCUSSION AND CONCLUSION[|]It has been shown that the ASA Checklist Procedure before airway management during general anesthesia induction can be successfully put into clinical practice with video-based feedback. We concluded that patient safety could be increased by the integration of this method into the routine anesthesia resident training program.[¤]

    Abstract Objective: Although thoracic computed tomography (T-CT) has high sensitivity in the diagnosis of coronavirus disease-2019 (COVID-19), a specific involvement type or region for COVID- 19 has not yet been identified in images consistent with viral pneumonia. This study determined the relationship between the degree of involvement in T-CT and mortality in patients with COVID- 19. Materials and Methods: One hundred and fifteen COVID-19 patients admitted to our intensive care unit were included in this study. Obtained T-CTs were evaluated according to the T-CT severity scoring system. Patients were divided into two groups as the mortality and the survival group. The two groups were compared in terms of age and gender, the presence and localization of abnormal T-CT findings and total CT score. Results: The total CT score was significantly higher in the mortality group than in the survival group (10.88±5.67 vs 8.53±4.89, p=0.048). Bilateral involvement in the T-CT scan was found to be significantly higher in the mortality group. Conclusion: Total CT scoring has predictive value in determining the survival of patients with a critical COVID-19. Bilateral involvement in the T-CT scans performed in the early period of disease and a total CT score of ≥10 may be indicative of mortality in COVID-19 patients.

    Abstract INTRODUCTION[|]The Joint Commission and the American Medical Association-Convened Physician Consortium for Performance Improvement reported that the blood transfusions are among the top five overused treatments in modern medicine. Optimal management of blood transfusion is one of the most important factors that increase patient safety, and special education is increasing all over the world in this regard. In this retrospective cohort study, our goal was to investigate the effects of periodic consensus meetings and training on perioperative blood transfusion by a team of different branches of medicine.[¤]METHODS[|]Patients over the age of 18 undergoing cardiac surgery and required blood transfusion were included in this study. The transfusions were calculated cumulatively; the change concerning years was determined, as well as side effects and complications associated with transfusion. Patients' cardiac reserves, laboratory values, anticoagulant drug use frequency, transfused blood volume, complications and mortality rates were recorded. The types of surgery, reexploration rate, length of intensive care and hospital stay were recorded.[¤]RESULTS[|]Patients' age, BMI, comorbidity ratio and antimicrobial drug use did not differ between years (p>0.05). The length of intensive care and hospital stay, amount of bleeding, mortality rate did not differ (p> 0.05). Cardiopulmonary Bypass time and Cross Clamp time in 2016, were significantly higher (p<0.05) than in 2014 and 2015. The amount of blood transfusion did not differ significantly (p>0.05). Besides, after 2014, an increase was observed in platelet transfusion. The change in Hb, Htc, platelet and INR did not differ significantly in the pre-postoperative period.[¤]DISCUSSION AND CONCLUSION[|]Training on the restrictive use of blood products did not have a positive influence on blood transfusion in our study. It has been demonstrated that there are challenges in sufficiently transferring the knowledge to the clinical environment.[¤]

    Abstract Accidentally dural puncture during the application of epidural blockage is a rare complication. There are some treatment methods in anesthesia management of this complication. Inserting the epidural needle to another intervertebral disc space, replacing subarachnoid catheter, injection of spinal anesthetic solution wherein epidural needle or implementing general anesthesia are some of the methods that physicians applicate.We report a cesarean delivery patient’s case in which we have composed an accidental dural puncture. We drew back the epidural needle till the level that no free cerebrospinal fluid (CSF) flow is seen. We confirmed this by injection aspiration technique. In this case, we revised the maneuvers of the anesthetist after unintentional dural puncture with protective and therapeutic methods of the most frequent complication postdural puncture headache (PDPH).

    Abstract When acute appendicitis is present and lately treated, further complications will occur against patients. This case report describes an unusual presentation of acute appendicitis in a young patient and demonstrates a unique late compli- cation of perforated appendicitis. A 15 year old female acute appendicitis patient had lapara- tomy accompanied systemic inflammatory re- sponse syndrome (SIRS) symptoms. After treatment of 6 days in the intensive care unit (ICU), the patient was healed. We emphasize the importance of early treatment for acute appen- dicitis and septic complications.

    Abstract Since myomectomy throughout cesarean deliv- ery may lead to hemorrhage and uterinal atony, it is not recommended. But, myomectomy has been reported during cesarean section in recent studies. We presented a patient with large in- tramural myoma who was diagnosed at 34 weeks of pregnancy and operated with an unavoidable cesarean-combined myomectomy. A 33-year-old unpursued primigravida was referred to emer- gency department with abdominal pain and amenorrhea of 34 weeks duration. A sonographic diagnosis of myoma in pregnancy was made. Cesarean section was required for fetal distress and alive 2300 g weighted male infant with Ap- gar score of 6 at one minute, was born. As uterine incision could not be closed because of the myoma, myomectomy was performed dur- ing cesarean section unavoidably. A single 970 g and 15 × 18 cm sized myoma was removed. The physical examinations were unremarkable in the postoperative period. Although there are case series that have demonstrated the safety of myomectomy during cesarean section, we con-cluded that myomectomy during cesarean section is not a safe procedure accept inevitable situa-tions.

    Abstract A field experiment was conducted to evaluate the integrated nutrients effect on growth, yield and quality of maize (Zea mays L.) during spring, 2009, at the Agronomic Research Area, University of Agriculture, Faisalabad. The ex-periment was laid out in Randomized Complete Block Design (RCBD) having three replications with following treat-ments: T1 (control), T2(recommended NPK @ 200-120-125 kg ha-1), T3 [single spray of multinutrient (a solution mixture of micronutrients i.e; Zn = 2%, Fe = 1%, B = 1%, Mn = 1%, Cu = 0.2% and macronutrients N = 1%, K2O = 2%, S = 2%) @ 1.25Lha-1], T4 (recommended NPK @ 200-120-125 kg ha-1 + single spray of multinutrient @ 1.25L ha-1), T5 (recom-mended NPK @ 200-120-125 kg ha-1 + two spray of multinutrient @1.25Lha-1) and T6 (recommended NPK @ 200-120-125 kg ha-1+ three spray of multinutrient @ 1.25Lha-1).The recommended dose of NPK in addition with single spray of Multi-nutrients substationally improved all growth parameters, ear characteristics and also enhanced macronutrients use efficiency up to 11.5% which induced significant increase in grain yield as compared to control and also in the treatment where recommended dose of NPK was applied alone. The quality parameter of maize (oil contents) significantly improved by foliar application of multinutrients solution but recommended dose of fertilizer in addition to single spray of Multi-nutrients was economical.

    Abstract Background: Opioid and local anesthetic infusion by an epidural catheter is widely used as a postoperative pain management method after major abdominal surgeries. There are several agents nowadays to provide sufficient analgesia. The agents which cause less side effects but better quality of analgesia are more valuable. We aimed to postoperatively compare the analgesic, hemodynamic and arrhythmogenic effects of epidural levobupivacaine-fentanyl and bupivacaine-fentanyl solutions. Methods: Fifty patients were scheduled to undergo major abdominal surgery in this clinical trial. The parameters were recorded pre- and post-operatively. In Group I (n=25), bupivacaine with fentanyl solution and in Group II (n=25), levobupivacaine with fentanyl solution was infused via epidural patient-controlled analgesia (PCA). According to the preoperative and postoperative holter recording reports, the arrhythmogenic effects were examined in four catagories: ventricular arrhythmia (VA), supraventricular arrhythmia (SVA), atrioventricular conduction abnormalities and pauses longer than two seconds. Results: Mean visual analog scale (VAS) values of groups did not differ at all time. They were 6 at the end of the surgery (0. Min, p = 0.622). The scores were 5 in Group I and 4 in Group II in 30. min (p = 0.301). The frequency of SVA was higher in bupivacaine group. Conclusions: The results of our study suggest that same concentration of epidural levobupivacaine and bupivacaine with fentanyl provide stable postoperative analgesia and both were found safe for the patients undergoing major abdominal surgery.
