    Abstract Malgré l'ouverture proclamée des Canadiens face à la diversité ethnique et culturelle, l'histoire canadienne n'en est pas moins marquée par la discrimination systématique. Cet ouvrage expose la ténacité juridique de cette discrimination par l'entremise d'un examen de six arrêts judiciaires déterminants entre 1900 et 1950 qui démontrent comment le système juridique canadien fut complice de la discrimination raciale. Les cas retenus font exemples des diverses façons dont le racisme a opéré dans les différents environnements juridiques du Canada. On y retrouve ceux d'Eliza Sero, qui a présenté en 1921 une revendication à la souveraineté Mohawk, de Wanduta, un Heyoka de la nation Dakota, qui visait à faire reconnaître son droit de célébrer la traditionnelle danse des herbes sacrées en 1903, d'Ira Johnson, qui a eu à subir le courroux du Ku Klux Klan en raison de son désir de contracter un mariage mixte en 1930, de Yee Clun, un restaurateur canadien d'origine chinoise à qui l'on avait refusé le droit d'employer des femmes blanches en 1924 et de Viola Desmond, qui avait été empêchée par le personnel d'un cinéma de s'asseoir dans une section réservée aux Blancs en 1946. De la couleur des lois illustre l'ambiguïté opérationnelle ainsi que l'étonnante et sournoise persévérance du racisme à l'oeuvre dans le système juridique canadien. De la couleur des lois est la traduction française de Colour-Coded: A Legal History of Racism in Canada (University of Toronto Press, 1999), qui a été gagnant du prix Joseph Brant en 2002

    Abstract The fabrication of microdevices for fluidic control often requires the use of flexible diaphragms in a way that requires cleanroom equipment and compromises performance. We use a CO 2 laser to perform the standard ablative techniques of cutting and engraving materials, but we also apply a method that we call laser placement. This allows us to fabricate precisely-positioned and precisely-sized, isolated diaphragms. This in turn enables the rapid prototyping of integrated multilayer microfluidic devices to form complex structures without the need for manual positioning or cleanroom equipment. The fabrication process is also remarkably rapid and capable of being scaled to manufacturing levels of production. We explore the use of these devices to construct a compact system of peristaltic pumps that can form water in oil droplets without the use of the non-pulsatile pumping systems typically required. Many devices can be fabricated at a time on a sheet by sheet basis with a fabrication process that, to our knowledge, is the fastest reported to date for devices of this type (requiring only 3 h). Moreover, this system is unusually compact and self-contained.

    Title translation Percutaneous implantation of an aortic prosthetic valve on a degenerated mitral bioprosthesis. Report of one case.
    Abstract Reintervention of a mitral degenerated bioprosthesis has a high surgical risk, especially in elderly patients with multiple comorbidities. We report a 74 years old female with two previous cardiac surgical procedures and a new structural mitral bioprosthesis deterioration with severe mitral regurgitation. Considering her high-surgical risk, a fully percutaneous treatment was performed with a balloon-expandable aortic valve in mitral position (valve-in-valve) through a transseptal approach with a favorable outcome. This technique is an attractive and effective option with a relatively low rate of complications that could solve this challenging and complex disease.

    Abstract Dental care professionals (DCPs) are thought to be at enhanced risk of occupational exposure to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). However, robust data to support this from large-scale seroepidemiological studies are lacking. We report a longitudinal seroprevalence analysis of antibodies to SARS-CoV-2 spike glycoprotein, with baseline sampling prior to large-scale practice reopening in July 2020 and follow-up postimplementation of new public health guidance on infection prevention control (IPC) and enhanced personal protective equipment (PPE). In total, 1,507 West Midlands DCPs were recruited into this study in June 2020. Baseline seroprevalence was determined using a combined IgGAM enzyme-linked immunosorbent assay and the cohort followed longitudinally for 6 mo until January/February 2021 through the second wave of the coronavirus disease 2019 pandemic in the United Kingdom and vaccination commencement. Baseline seroprevalence was 16.3%, compared to estimates in the regional population of 6% to 7%. Seropositivity was retained in over 70% of participants at 3- and 6-mo follow-up and conferred a 75% reduced risk of infection. Nonwhite ethnicity and living in areas of greater deprivation were associated with increased baseline seroprevalence. During follow-up, no polymerase chain reaction-proven infections occurred in individuals with a baseline anti-SARS-CoV-2 IgG level greater than 147.6 IU/ml with respect to the World Health Organization international standard 20-136. After vaccination, antibody responses were more rapid and of higher magnitude in those individuals who were seropositive at baseline. Natural infection with SARS-CoV-2 prior to enhanced PPE was significantly higher in DCPs than the regional population. Natural infection leads to a serological response that remains detectable in over 70% of individuals 6 mo after initial sampling and 9 mo from the peak of the first wave of the pandemic. This response is associated with protection from future infection. Even if serological responses wane, a single dose of the Pfizer-BioNTech 162b vaccine is associated with an antibody response indicative of immunological memory.

    Abstract Introduction: Historically, concerns about complications following parathyroid surgery, such as airway compromise, bleeding and hypocalcemia, have precluded its consideration as a short-stay surgical procedure. Recent advancements in perioperative care have resulted in several publications demonstrating that parathyroidectomy can be safely performed as a short-stay procedure.
    Objectives: The aim of the present study was to describe the process of implementing a short-stay protocol focusing on preoperative patient education and postoperative calcium management for those undergoing surgery for primary hyperparathyroidism (PHP).
    Method: A retrospective audit of consecutive parathyroidectomies performed for PHP over the period between 2010 and 2013 was performed. A short-stay protocol (SSP) was introduced focusing on postoperative calcium management. Results were reaudited over the period between 2013 and 2015.
    Results: Consecutive parathyroidectomies in 76 patients were included in the study. A total of 42 patients underwent parathyroidectomy prior to the introduction of the protocol. A total of 26.2% of these patients were symptomatic from hypercalcemia. A total of 40 out of 42 (95.2%) patients had a biochemical cure. A total of 36 out of 42 (85.7%) cases were due to parathyroid adenomas. A total of 34 patients underwent surgery following the introduction of the protocol. A total of 13 out of 34 (38.2%) of the patients had symptomatic hypercalcemia. A total of 33 out of 34 (97.1%) had a biochemical cure. A total of 32 out of 34 (94.1%) cases were due to parathyroid adenomas. The length of stay decreased from a median of 3 days (range 2–9 days; mean 3.32) preprotocol to a median of 2 days (range 2–3 days; mean 2.16) postprotocol ( p  < 0.0001) with no difference in the 30-day unplanned readmission rate (4.8 versus 2.9%; p  = 0.999).
    Conclusions: The postoperative length of stay after parathyroidectomy for PHP can be safely reduced through patient education and by rationalizing postoperative calcium management without adversely affecting outcomes.
