    Abstract Purpose: The anatomy of the middle glenohumeral ligament (MGHL) is seldomly described during arthroscopy. The aim of this study was to determine the arthroscopic variants concerning the anatomy of the MGHL.
    Methods: A prospective, observational, single-center study was conducted between June 2016 and June 2017. All patients undergoing a first-time arthroscopy of the shoulder and with no history of prior surgery or trauma of the same shoulder were consecutively enrolled. The variations of the MGHLs shape and of its glenoid and distal insertions were documented during surgery.
    Results: A total 300 patients were included. Surgeries included rotator cuff sutures, tenotomy/tenodesis of the long head of the biceps tendon (LHBT) and subacromial decompression in respectively 31%, 32.7% and 35.5% of cases. The MGHL was absent in 12% of cases, presented a flat structure in 72% of cases, a cord-like shape in 14% and a Buford complex was observed in 1%. Its glenoid insertion was located on the labrum between the superior (SGHL) and inferior (IGHL) in 43% of cases, presented a combined insertion with the LHBT in 7%, with the SGHL in 29% and with both the SGHL and LHBT in 9%. The distal insertion was located in 67% of cases on the subscapularis tendon (SCCT) or the capsule covering it, and on the humeral bone in 21%.
    Conclusion: This study confirms and details the anatomic variants of the MGHL, notably describing the lesser-known variants of its distal insertion, yet arises the question of the exact nature and function of this so-called ligament.

    Abstract Introduction: Critical shoulder angle (CSA)>35° correlates significantly with primary rotator cuff tear and re-tear after repair. Acromial axial orientation varies between individuals. The present study aimed to assess the impact of strictly anterior acromioplasty on CSA.
    Hypothesis: CSA is reduced by arthroscopic anterior acromioplasty.
    Material and methods: A single-center retrospective study included patients receiving isolated arthroscopic acromioplasty between January 2016 and December 2017. Exclusion criteria comprised history of surgery and fracture or dislocation of the shoulder. Pre- and post-operative AP shoulder radiographs were taken. The angle subtended by a line between the inferior and superior edges of the glenoid and a line between the inferior edge of the glenoid and the most lateral point on the acromion was measured.
    Results: One hundred and two patients were included: mean age, 50.5±10.1 years (range, 23-82 years). Mean CSA was 34.7±4.4° (range, 26-48°) preoperatively and 31.7±3.7° (range, 23-40°) postoperatively (p<0.001). In the group with CSA >35°, the decrease was significant: 34.4±3.2° (range, 28-40°) versus 38.9±2.8° (range, 36-48°) (p<0.001), with 64% CSA <35° after isolated anterior acromioplasty.
    Discussion: CSA was decreased by arthroscopic acromioplasty. However, in 36% of cases with preoperative CSA >35°, CSA was not normalized.
    Conclusion: Anterior arthroscopic acromioplasty significantly reduced CSA in the overall population, and especially in case of high CSA.

    Abstract Introduction: Critical shoulder angle (CSA) is measured on AP view in neutral rotation as the angle subtended by the tangent to the glenoid and the lateral edge of the acromion. CSA >35° is a risk factor for primary rotator cuff tear and iterative tear after cuff repair. The aim of the present study was to assess change in CSA following arthroscopic anterior acromioplasty.
    Hypothesis: CSA is decreased by arthroscopic anterior acromioplasty.
    Material and method: A single-center retrospective descriptive observational study included patients undergoing arthroscopic acromioplasty between January 2016 and December 2017, for whom strictly AP pre- and post-operative radiographs were available. Patients with history of surgery, fracture or dislocation in the same shoulder were excluded.
    Results: 102 patients were included; mean age, 50.5 years (range, 23-82 years). Mean preoperative CSA was 34.7±4.4° (range, 26-48°) and postoperative CSA 31.7±3.7° (range, 23-40°) (p<0.001). In the pathological CSA group (>35°), the values were 38.9±2.8° (36-48°) and 34.4±3.2° (28-40°) (p<0.001), with a 64.3% rate of correction after acromioplasty.
    Discussion: CSA decreased after arthroscopic acromioplasty. Even so, 36.7% of pathological CSAs (>35°) were not corrected, with risk of rotator cuff tear.
    Conclusion: Anterior arthroscopic acromioplasty significantly reduced CSA overall, notably in case of high CSA.

    Abstract A strategic theory of suicide assures that any suicide or attempted suicide is a solution to a problem affronted by a person. The basic question is: "who seeks what solution to what problem by killing oneself or by attempting to do it?" This position allows to distinguish between a dozen different meanings of suicidal conducts. By an escape, somebody tries to get out of an unbearable position; depending on the circumstances, the situation is more or less realistic, so that it becomes feasible to question the state of the mind of the subject. Instead, in an appeal, a subject tries to ring a bell, expressing a need to be helped to live. Other meanings are grief, revenge, blackmail, sacrifice, ordeal... The question "who?" can be answered depending on the meanings and the circumstances. One can show the impact of the status of the mind, of age, gender, status, of a series of indicators pertaining to suicide and attempted suicide. Some statistical observations can be explained in this way. In a third point, it is argued that this theory is able to explain how some factors have a bearing on suicide, be it war, the economic cycle, or some more profound cultural patterns, such as nervous breakdown as an ethnic illness or the modern process of individualization. In a final section, a point is made about the possibility of deducing from the theory some kinds of therapy, one bearing on the connections linking the family, the other building on sessions of "problems solving". On the contrary, one has to keep sceptical about preventing suicides.
