    Abstract Sepsis is associated with high levels of morbidity and mortality. All healthcare professionals have a responsibility to ensure they have sufficient knowledge to effectively screen patients for signs and symptoms of sepsis. In the community setting, screening for sepsis can be challenging, due to the complexity within the patient population and difficulties associated with observation for changes in the patient's condition. The Sepsis Trust community nursing sepsis screening tool provides decision-making support to community healthcare professionals, enabling them to make a rapid assessment for risk factors for sepsis, ensuring a proportionate, consistent and appropriate response. Through implementation of a decision-support tool within the clinical setting, it is likely that patients at risk of sepsis will be identified earlier, and patients will be escalated in a more consistent manner. This process of improving consistency in practice can improve patient outcomes, including mortality, morbidity and overall patient experience.

    Abstract This paper examines debates about the "planning of science" that took place in the 1930s and 1940s in the United States and Britain and argues for their enduring relevance to today's philosophical literature on scientific pursuitworthiness. In addition to proposing desiderata for scientific pursuit that retain philosophical interest, such as social utility, coordination between branches of science, and the comprehensive development of science as a whole, advocates of the planning of science shared a sense of the inextricable relationship between the philosophy of scientific pursuit and the political economy of science, including the structure of science funding and scientific institutions. Critics of the planning of science were alarmed by the movement's socialist political orientation and developed a rival account of scientific pursuit framed by the economic metaphor of the independent businessman or entrepreneur. Taking advantage of the red scare in the United States in the early years of the Cold War, advocates of scientific entrepreneurship seized the upper hand over advocates of science planning during the shaping of the postwar American science policy regime. According to the science-as-entrepreneurship view, the role of science policy was only to make judgments about which individuals seemed best equipped, due to their personal intellectual or moral virtues, to make decisions about pursuitworthiness. The ultimate triumph of this view over the planning of science opened up a gap between the philosophy of pursuit and the political economy of science that persists to the present day.

    Abstract The current study investigates Spanish children's variation between the standard and non-standard forms for second person singular preterit -s (caiste ~ caístes). All second person singular preterit forms were extracted from the spontaneous speech of 78 children in Spain and analyzed for the effects of age, language contact setting, and lexical frequency. Results show that children in contact with Galician and Catalan produce more non-standard than children in non-contact areas like Madrid. Meanwhile, low-frequency verbs (e.g., pillaste) are more likely to occur with the non-standard -s than high-frequency verbs (e.g., fuiste). However, age is not a significant predictor of children's 2sg preterit production. These preliminary findings demonstrate that Spanish children do have the non-standard -s in their speech, and that their 2sg preterit forms are significantly conditioned by language contact and lexical frequency.

Emergency nurses play an essential role in infection prevention and control in this clinical area. The COVID-19 pandemic has refocused the need for emergency nurses to have a good knowledge and understanding of infection control processes and the clinical skills to protect themselves and patients alike.     Abstract Background: Rib fractures are associated with significant morbidity and mortality, and providing adequate analgesia is paramount in preventing early complications from these injuries. Multiple regional anaesthetic techniques can be used to provide analgesia for these injuries; however, few evidence-based guidelines exist for their use. The aim of this study was to establish consensus within an expert group on the selection criteria used when choosing regional anaesthetic techniques for rib fractures.
    Methods: The Delphi technique is a mixed-methods study format which uses a longitudinal survey process to develop consensus opinion amongst an expert group. A three-round modified e-Delphi study was undertaken using an online survey platform. Round one established cohort characteristics and identified key factors considered important by the group when selecting regional anaesthetic techniques for rib fractures. Subsequent rounds used Likert scales and free text comments to rate the participants' level of agreement with various statements generated from the first-round responses. The final consensus threshold was established as at least 70% of respondents stating, 'Strongly Disagree' or 'Disagree' or alternatively 'Agree' or 'Strongly Agree'.
    Results: An expert panel of UK-based consultants in anaesthesia and/or intensive care medicine was recruited. Participants worked in a variety of tertiary- and non-tertiary trauma care settings and were varied in their years of experience, approximate annual rib fracture caseload and preference for various anaesthetic techniques in rib fracture management. 54 participants took part in round one and generated 60 statements which were further analysed in an iterative process involving a total of three rounds. A total of 28 statements ultimately reached the pre-defined threshold for consensus within the expert group.
    Conclusions: This e-Delphi study succeeded in building consensus across multiple statements relating to the selection criteria for regional anaesthesia in patients with rib fractures. These consensus statements can inform clinical practice, guide future research priorities and can be integrated into decision-making pathways across multiple hospital settings.

    Abstract This case report discusses a 46-year-old female with no prior surgical history who presented with severe abdominal pain and generalized tenderness. She was found to have a small bowel obstruction secondary to internal hernia caused by a rare congenital pelvic peritoneal defect in the Pouch of Douglas. She required diagnostic laparoscopy and repair of the pelvic peritoneal defect. Congenital peritoneal defect is an extremely rare cause of small bowel obstruction but should remain a possible differential diagnosis in patients with virgin abdomen presenting with acute abdominal pain.
